천식 질환이 있는 노인의 염증성 T 세포 및 면역 조절 T 세포의 분석 : The phenotypic analysis of inflammatory T cells and immune regulatory T cells in elderly asthma patients

Abstract
사람의 경우 노화에 의해 CD4, CD8 T 세포들의 수와 아형의 비율, 분비하는 cytokine 등이 달라져 면역 반응에 변화가 생기는 면역 노화 현상이 일어나게 된다. 이번 연구에서는 대표적인 염증성 질환인 천식이 노화라는 인자에 의해 그 특성이 달라지는지 염증성 T 세포와 면역 조절 T 세포의 phenotype을 중심으로 분석해보았다. 이를 위해 노인 천식 환자 50명, 노인 정상인 16명, 성인 천식 환자 30명, 성인 정상인 20명의 혈액 샘플에서 말초혈액단핵세포를 분리한 후 ICS 방법을 통해 CD4+ T 세포와 γδT 세포를 분석하였다.
염증성 T 세포의 분석은 IFN-γ, IL-4, IL-17, IL-9, IL-13, IL-17F, IL-22 등의 cytokine을 분비하는 세포의 비율로 분석하였다. 그 결과 천식의 염증 반응을 매개한다고 알려진 IL-4, IL-9, IL-13이 노인 천식 환자에서 증가되어 있었고, 최근 병원성이 입증된 비전형 cytokine 분비 T 세포 중 IL-9+Th17 세포, IFN-γ+Th17 세포가 노인 천식 환자에서 증가되어 있었으며, 과도한 Th2 염증반응시 생성된다고 알려진 Th2 inhibitory 세포도 노인 천식 환자에서 증가되어 있었다. 또한 천식의 병인세포로 작용할 수 있는 IL-4+γδT 세포와 함께 IL-17+γδT 세포도 노인 천식 환자에서 증가되어 있었다.
다음으로 천식의 염증 반응을 억제할 수 있다고 알려진 면역 조절 T 세포에 대해 IL-10 cytokine과 Foxp3 전사인자의 발현을 통해 분석해보았다. 그 결과 Foxp3를 발현하는 조절 T 세포는 정상 노인에서만 증가되어 나타난 반면, IL-10을 분비하는 CD4+ T 세포는 노인 천식 환자에서 매우 증가되어 있었다. 그리고 노인에서 면역 억제 기능이 감소된 cytokine 분비 조절 T 세포가 증가되어 있다고 알려져 있어 IFN-γ, IL-4, IL-13, IL-17 등의 cytokine을 분비하는 조절 T 세포를 확인해본 결과, IL-4를 분비하는 조절 T 세포가 노인 천식 환자에서 증가되어 있었다.
이러한 염증성 T 세포와 조절 T 세포의 분석을 통해 노화라는 인자에 의해 달라진 노인 천식의 특성을 T 세포 수준에서 규명할 수 있었으며, 성인 천식과는 다른 노인 천식의 발생 기전을 설명할 수 있을 뿐 아니라 기존의 천식 치료법이 잘 적용되지 않는 노인층의 천식 치료법의 타겟을 설정하는 데 기여할 수 있을 것이라 생각된다.
The number of CD4, CD8 T cells, and the frequencies of helper T cell subsets and their cytokine production are changed in the elderly along with the immunosenescence. In this study, we investigated the characteristics of elderly asthma through the analysis of phenotypes of inflammatory and regulatory T cells.
For the analysis of inflammatory T cells, we analyzed CD4 T cells secreting inflammatory cytokines such as IFN-γ, IL-4, IL-17, IL-9, IL-13, IL-17F, and IL-22. As a result, the frequencies of CD4 T cells secreting IL-4, IL-9, IL-13 were increased in the peripheral blood of elderly asthma patients. Unexpectedly, the pathogenic atypical cytokine secreting T cells such as IL-9+Th17 cells, IFN-γ+ Th17 cells, and Th2 inhibitory cells were also increased in the elderly asthma patients. Together with IL-4 producing γδ T cells which are the suspicious pathogenic cells of asthma, γδ T cells secreting IL-17 were also increased in the elderly asthma patients.
For the analysis of immune regulatory T cells, we analyzed the expression of IL-10 and the transcription factor Foxp3. Although Foxp3+ regulatory T cells were increased only in the elderly non-asthma patients, regulatory T cells expressing IL-10 were increased in the elderly asthma patients. Moreover, regulatory T cells secreting IL-4 were increased in the elderly asthma patients.
This study would provide a novel explanation for the differences observed between young asthma and elderly asthma, as well as the mechanism for the development of elderly asthma. Consequently, this study may contribute to establish a strategy for the treatment of elderly asthma patients who are incurable by current methods of asthma treatment.
