비글견의 하악에 식립된 즉시 식립형 자연치아 형태 지르코니아 임플란트의 골유착 평가

Abstract

Objectives: Immediate root-analog zirrconia implant system was used as alternative to solve the aesthetic problem of titanium implant and the weakness of current surgical concepts and implant shape. There are various factors behind implant failures, but we focused on implant design which was needed to establish the root-analog technique designed to provide primary stability. So, the aim of this study was to evaluate the osseointegration of immediate root-analog zirconia implant in the mandible of beagles.

Materials and methods: The mandible of three male beagle dogs was examined by CT scans and then the 3D image for imitating natural tooth was reconstructed to fabricate root-analog zirconia implants. Each beagle received the immediate root-analog zirconia implant at location of the premolars in the mandible. After 3-month healing period, Micro CT examination and histomorphometric analyses were performed.

Results: The immediate root-analog zirconia implants were inserted in a total 42 dental roots and seven implants failed. After 3-month healing period, implant retention showed success in 35 root-analog zirconia implants, 23 of which had an average BIC of 18.7±15.2% and the osseointegration were confirmed.

Conclusions: It was found that the average BIC values for the immediate root-analog zirconia implant is lower than those of the conventional commercial implant. Further study need to achieve successful osseointegration of the immediate root-analog zirconia implant.

목적: 티타늄 임플란트가 가지는 심미적 문제와 기존의 원통형 임플란트 술식의 단점을 대안으로 한 임플란트 시스템을 적용하였다. 임플란트 실패에는 여러 요인이 존재하지만, 디자인의 측면에 초점을 두고 발치와(拔齒窪)와 긴밀한 즉시 식립형 자연치아 형태 임플란트를 통해 초기 안전성을 부여하는 기술 개념을 확인하고자 하였다. 따라서 본 연구에서는 비글견의 하악에 즉시 식립형 자연치아 형태 지르코니아 임플란트를 식립하여 골유착(Osseointegration)을 평가하고자 한다.

방법: 즉시 식립형 자연치아 형태 지르코니아 임플란트 제작을 위해 비글견의 하악을 CT로 촬영하였고, 치아 구조를 본뜬 3차원 영상을 획득하였다. 획득한 영상을 바탕으로 임플란트를 제작하여 3마리의 수컷 비글견 하악 소구치 양쪽에 식립하였고, 3개월의 치유과정 후 조직학적 평가와 Micro CT 분석을 수행하였다.

결과: 비글견의 하악 소구치 부위에 총 42개의 즉시 식립형 자연치아 형태 임플란트를 식립하였고, 이 중 7개의 임플란트는 발치와와 긴밀성 부족으로 식립에 실패했다. 3개월의 치유 과정 후에는 식립에 성공한 35개의 임플란트 중 23개의 임플란트에서 골유착을 확인하였고, 평균적인 BIC 값은 18.7±15.2 %로 나타났다.

결론: 즉시 식립형 자연치아 형태 지르코니아 임플란트의 BIC 값은 기존의 상용 임플란트의 BIC 값과 비교해 낮게 나타났다. 골유착의 우수성을 평가하기 위해서는 아직 더 많은 연구가 필요할 것으로 보인다.
