Abstract
Introduction

Kallistatin has anti-inflammatory effect by various mechanisms and it also attenuates endothelial dysfunction through endothelial nitric oxide synthase (eNOS)-associated pathways. The objective of this study was to investigate the mechanism of action of kallistatin in endothelial cell experimental model mimicking sepsis and to investigate whether serum levels of kallistatin, and biomarkers associated with endothelial dysfunction are associated with clinical outcomes in patients with septic shock



Materials and methods

Human umbilical vein endothelial cells (HUVECs) were divided into five groups for experiments: cells without treatment (negative control group), cells transfected with control small interfering ribonucleic acid (siRNA) only (control siRNA group), cells transfected with control siRNA and treated with lipopolysaccharide (LPS) (control siRNA + LPS group), cells transfected with SERPINA4 siRNA (SERPINA4 siRNA group) and cells transfected with SERPINA4 siRNA and treated with LPS (SERPINA4 siRNA + LPS group). Cell viability, kallistatin level and endothelial nitric oxide synthase (eNOS) level in the culture medium were measured for each group. For clinical investigation, serum levels of kallistatin, vascular cell adhesion molecule-1 (VCAM-1), and E-selectin were measured by enzyme-linked immunosorbent assay using blood samples obtained at admission, at 24 h, and at 72 h after admission from the patients with septic shock and from healthy volunteers. The primary outcome for the clinical investigation was 28-day mortality.



Results

In the SERPINA4 siRNA group, kallistatin level in the culture medium was decreased compared with the negative control group (2.5 ± 0.7 pg/mL vs. 21.1 ± 2.1 pg/mL, P < 0.001) and the control siRNA group (20.5 ±3.3 pg/mL, P < 0.001). Kallistatin level in the medium was lower in the control siRNA + LPS group compared with the control siRNA group (12.0 ± 2.3 pg/mL vs. 21.3 ± 3.6 pg/mL, P = 0.046). Kallistatin level in the medium was markedly lower in the SERPINA4 siRNA group (3.2 ± 1.4 pg/mL, P < 0.001) and the SERPINA4 siRNA + LPS group (3.6 ± 1.9 pg/mL, P < 0.001) compared with the control siRNA group, and there was no significant difference in kallistatin level in the medium between the two groups (P = 0.999). Relative cell viability was lower in the SERPINA4 siRNA + LPS group (58.0 ± 3.7%) compared with the negative control group (100 ± 7.9%, P = 0.001), the control siRNA group (98.2 ± 9.0%, P = 0.002) and the SERPINA4 siRNA group (93.3 ± 4.7%, P = 0.007). eNOS levels in the medium were lower in the control siRNA + LPS group (4.1 ± 0.1 ng/mL, P = 0.041) and the SERPINA4 siRNA group (4.1 ± 0.1 ng/mL, P = 0.041) compared with the control siRNA group (4.5 ± 0.1 ng/mL). eNOS level in the medium was lower in the SERPINA4 siRNA + LPS group (3.7 ± 0.1 ng/mL) compared with the negative control group (4.5 ± 0.1 ng/mL, P < 0.001), the control siRNA group (P < 0.001), the control siRNA + LPS group (P = 0.021) and the SERPINA4 siRNA group (P = 0.021).

Fifty-eight survivors, fourteen non-survivors, and six healthy volunteers were enrolled for the clinical investigation. Serum kallistatin level was lower and serum VCAM-1 and E-selectin levels were higher in patients at admission compared with healthy volunteers. Compared with survivors, serum kallistatin levels were lower in non-survivors at all time points (4.4 μg/mL [2.9–6.1] vs. 2.5 μg/mL [2.1–5.0], P = 0.019 at admission
4.3 μg/mL [3.3–5.2] vs. 3.2 μg/mL [2.2–3.8], P = 0.004 at 24 h
3.1 μg/mL [2.5–4.2] vs. 2.3 μg/mL [1.7–3.1], P = 0.012 at 72 h), while VCAM-1 and E-selectin levels showed no difference. In the multivariable logistic regression analysis, serum kallistatin level at 24 h was independently associated with 28-day mortality (odds ratio, 0.29
95% confidence interval, 0.08–0.69, P = 0.024).



Conclusions

In the SERPINA4 siRNA group, kallistatin level in the medium was decreased compared with the negative control group and the control siRNA group. In the SERPINA4 siRNA + LPS group, kallistatin level in the medium was decreased compared with the negative control group, cell viability was lower compared with the negative control group, the control siRNA group and the SERPINA4 siRNA group and eNOS level in the medium was lower compared with the negative control group, the control siRNA group, the control siRNA + LPS group and the SERPINA4 siRNA group. Lower serum kallistatin level at 24 h was independently associated with 28-day mortality in patients with septic shock, but the levels of VCAM-1 or E-selectin were not.
연구 목적

혈관내피 기능 장애는 패혈성 쇼크의 여러 중요한 병태생리 중 하나이다. 칼리스타틴은 체내에서 다양한 기전을 통하여 항염증 작용을 나타내며, endothelial nitric oxide synthase (eNOS)와 연관된 경로를 통하여 혈관내피 기능 장애를 감쇄하는 것으로 알려져 있다. 본 연구는 패혈증을 모방한 혈관내피 세포 모델에서 칼리스타틴이 혈관내피 생존력에 작용하는 기전을 확인하고, 패혈성 쇼크 환자에서 혈중 칼리스타틴 농도와 혈관내피 기능 장애와 연관된 표지자의 농도가 임상적인 예후와 연관성이 있는지 확인하고자 진행되었다.



연구 재료 및 방법

실험 연구를 위해 인간 제대정맥 내피세포(human umbilical vein endothelial cell, HUVEC)를 다섯 군[아무 처리도 시행하지 않은 군(음성 대조군), control 짧은 간섭 리보핵산(small interfering ribonucleic acid, siRNA) 형질주입만 시행한 군(control siRNA군), control siRNA 형질주입과 지질다당류(lipopolysaccharide, LPS) 처리를 시행한 군(control siRNA + LPS군), SERPINA4 siRNA 형질주입만 시행한 군(SERPINA4 siRNA군), 그리고 SERPINA4 siRNA 형질주입과 지질다당류 처리를 시행한 군(SERPINA4 siRNA + LPS군)]으로 나누었다. 각 군별로 세포 생존력, 세포 배양액 내 칼리스타틴 농도, 그리고 eNOS 농도를 측정하였다.

또한 패혈성 쇼크로 중환자실에 입원한 환자로부터 입원 시점, 입원 24 시간 후, 입원 72 시간 후 채혈한 혈액과 건강한 자원자로부터 채혈한 혈액을 이용하여 효소결합면역흡착검사(enzyme-linked immunosorbent assay, ELISA)를 통해 혈중 칼리스타틴, vascular cell adhesion molecule-1 (VCAM-1), E-selectin 농도를 측정한 임상 연구를 진행하였다. 임상 연구의 일차 결과 변수는 28일 사망으로 설정하였다.



연구 결과

SERPINA4 siRNA군에서는 음성 대조군(2.5 ± 0.7 pg/mL 대 21.1 ± 2.1 pg/mL, P < 0.001)이나 control siRNA 군(20.5 ±3.3 pg/mL, P < 0.001)과 비교했을 때 세포 배양액 내 칼리스타틴 농도가 감소하였다. Control siRNA + LPS군의 세포 배양액 내 칼리스타틴 농도는 control siRNA 군과 비교했을 때 유의하게 감소하였다(12.0 ± 2.3 pg/mL 대 21.3 ± 3.6 pg/mL, P = 0.046). SERPINA4 siRNA군(3.2 ± 1.4 pg/mL, P < 0.001)과 SERPINA4 siRNA + LPS군(3.6 ± 1.9 pg/mL, P < 0.001)의 세포 배양액 내 칼리스타틴 농도는 control siRNA군과 비교했을 때 매우 낮게 측정되었으며, 두 군 사이에 유의한 칼리스타틴 농도 차이는 관찰되지 않았다(P = 0.999). SERPINA4 siRNA + LPS군의 24 시간 생존력(58.0 ± 3.7%)은 음성 대조군(100 ± 7.9%, P = 0.001), control siRNA군(98.2 ± 9.0%, P = 0.002), 그리고 SERPINA4 siRNA 군(93.3 ± 4.7%, P = 0.007)과 비교했을 때 유의하게 감소하였다. Control siRNA + LPS군(4.1 ± 0.1 ng/mL, P = 0.041)과 SERPINA4 siRNA군(4.1 ± 0.1 ng/mL, P = 0.041)의 세포 배양액 내 eNOS 농도는 control siRNA군(4.5 ± 0.1 ng/mL)과 비교했을 때 낮게 측정되었다. SERPINA4 siRNA + LPS군의 세포 배양액 내 eNOS 농도(3.7 ± 0.1 ng/mL)는 음성 대조군(4.5 ± 0.1 ng/mL, P < 0.001), control siRNA군(P < 0.001), control siRNA + LPS군(P = 0.021), 그리고 SERPINA4 siRNA군(P = 0.021)에 비해 유의하게 낮게 측정되었다.

임상 연구에는 총 72 명의 환자(생존군 58 명, 사망군 14 명)와 6 명의 건강한 자원자가 연구에 참여하였다. 건강한 자원자에 비해 환자군에서는 입원 당시 혈중 칼리스타틴 농도가 낮게, 혈중 VCAM-1과 E-selectin 농도가 높게 측정되었다. 생존군과 비교했을 때 사망군에서는 세 시점 모두에서 혈중 칼리스타틴 농도가 낮게 측정되었다[입원 시, 4.4 μg/mL (2.9–6.1 μg/mL) 대 2.5 μg/mL (2.1–5.0 μg/mL), P = 0.019
24 시간 후, 4.3 μg/mL (3.3–5.2 μg/mL) 대 3.2 μg/mL (2.2–3.8 μg/mL), P = 0.004
72 시간 후, 3.1 μg/mL (2.5–4.2 μg/mL) 대 2.3 μg/mL (1.7–3.1 μg/mL), P = 0.012]. 반면 혈중 VCAM-1이나 E-selectin 농도는 두 군 사이에 유의한 차이가 관찰되지 않았다. 다변수 로지스틱 회귀 분석에서, 입원 24 시간 후 측정한 혈중 칼리스타틴 농도는 28 일 사망과 독립적인 연관성이 관찰되었다(교차비, 0.29
95% 신뢰구간, 0.08–0.69, P = 0.024).



결론

SERPINA4 siRNA군에서는 음성 대조군이나 control siRNA군과 비교했을 때 세포 배양액 내 칼리스타틴 농도가 유의하게 감소하였다. SERPINA4 siRNA + LPS군에서는 음성 대조군과 비교했을 때 세포 배양액 내 칼리스타틴 농도가 유의하게 감소하였고, 음성 대조군, control siRNA군, 그리고 SERPINA4 siRNA군과 비교했을 때 세포 생존력이 감소하였으며, 음성 대조군, control siRNA군, control siRNA + LPS군, 그리고 SERPINA4 siRNA군과 비교했을 때 세포 배양액 내 eNOS 농도가 유의하게 감소하였다. 패혈성 쇼크 환자에서 입원 24 시간 후 측정된 낮은 혈중 칼리스타틴 농도는 28 일 사망과 독립적인 연관성이 관찰되었으나, 혈중 VCAM-1이나 E-selectin 농도는 28 일 사망과의 연관성이 관찰되지 않았다.
