병원약사 업무 형태별 입원환자 처방 검토 기록 분석 및 약사의 역할 고찰

Abstract

병원약사의 역할은 조제 중심에서 환자 치료에 좀 더 적극적으로 관여하는 약료 (pharmaceutical care) 중심으로 점차 변화되어 가고 있다. 하지만 현행 의료 체계에서는 여전히 조제가 약사의 책임에 많은 비중을 차지하고 있다. 조제는 광범위하게 처방검토, 조제, 조제약 감사로 구분되며, 조제 전 처방검토는 처방전 내에 포함된 약에 대해서만 이루어지고, 표준 용량, 용법, 처방전에 있는 약물간의 상호작용 및 중복 등에 대한 검토만 가능하다. 그러나, 환자 측면에서 정확한 처방 검토를 하기 위해서는 실제 환자의 질병 상태, 신기능, 환자의 입원 전 투약 내역, 자가약 복용, 실제 약물 투약 기록을 종합적으로 확인하는 과정이 반드시 필요하다. 서울대학교병원에서는 컴퓨터 시스템을 이용하여 환자에게 처방을 입력하는 EMR 을 도입하여 사용하고 있으며, 보건복지부의 시행령으로 의사가 약처방을 입력할 때 1차적으로 보완하는 프로그램인 CDSS를 활용하고 있으나, 이는 법적으로 최소한의 내용만 반영하고 있으므로 의사가 최종 입력한 처방이 약제부로 접수된 후에는 조제를 시행하여 환자에게 투약되기 전에 약사가 환자의 약력을 확인하여 처방의 적절성을 검토하는 과정이 반드시 필요하다. 종합적인 환자의 약물요법에 관여하는 가장 좋은 방법으로는 진료팀의 일원으로서 활동하면서 환자의 전반적인 약물요법을 모니터링 하는 것으로 알려져 있으며, 장기이식, 중환자, 종양 환자, 흉부외과 환자 등 다양한 분야에서 임상 약사가 이러한 역할을 수행하고 있다. 그러나 현실적인 인력 문제로 많은 질환군의 환자에서 이러한 임상약사의 업무를 시행하는 것에는 한계가 있으므로 조제 약사의 처방검토를 강화하여 좀 더 많은 환자에 대하여 안전한 약물요법에 약사가 기여할 필요가 있다. 이에 따라 입원 환자 조제를 담당하는 약사가 이러한 종합적인 내용을 근거로 처방검토를 시행할 수 있는 처방검토 프로그램을 개발하여 사용하게 되었으며, 본 연구에서는 이 처방검토 프로그램 도입 이전 조제약사의 처방 검토 기록과 이 프로그램을 사용하여 검토한 병동약사의 중재 기록, 그리고 팀의료의 일원인 임상약사의 처방검토 기록을 분석하여 병원약사의 역할 변화의 필요성을 고찰하고자 하였다. 본 연구에서는 업무 형태가 다른 세 군의 처방검토 기록에 대하여처방 오류 유형, 약사 추천유형, 오류의 중요도를 분석한 결과 세 군에서 각각 다른 분포를 보였다. 특히 오류의 중요도에 있어서 큰 차이를 나타낸 것을 볼 때 입원환자의 치료 효과에 있어서 팀의료의 일원으로서 약사의 역할은 매우 중요하다고 할 수 있다. 병원약사의 주 업무인 조제를 담당하는 약사는 처방의 적절성을 먼저 검토하는 것이 기본이므로 환자의 약력과 질병상태를 확인할 수 있는 처방검토 프로그램을 활용하여 적극적으로 중재할 수 있는 업무 형태로 전환하여 약사의 임상적 지식을 환자 치료효과에 반영할 수 있어야 하며, 조제 자동화 등 약사의 업무 패러다임을 변화 시킴으로서 팀의료의 일원으로 약사가 더 많이 임상활동을 할 수 있는 환경을 조성해야 할 것이다.

The hospital pharmacists role has changed steadily and turning away from dispensing functions toward active involvement in pharmaceutical care. But, dispensing function is still given much more weight on in the responsibility of pharmacists in the present healthcare system. Dispensing includes order verification, dispensing of drugs, double checking of dispensed medications in a wide range. Order clarification by reviewing of prescription before dispensing is only on medications in the sheet and pharmacists can review on common daily dosage, drug interactions and duplications in the prescription. However, to clarify more preciously through reviewing prescriptions for patients, pharmacists are required to check the patients general condition, kidney function, medication history before hospitalization, self medication use and records of administrations comprehensively. Seoul National University Hospital has active computerized physician order entry (CPOE) systems, EMR, and using CDSS program when physicians enter the order complements possible medication error. However, it is an only first line complementing system when ordering on the basis of the act by Ministry of Health and Welfare, so additionaly, before dispensing and administering order, pharmacists have to review the appropriateness of the order received in the pharmacy and clarify. Pharmacists are an essential resource in safe medication use, that participation of pharmacists on rounds improves medication safety, and that pharmacist-physician-patient collaboration is important. Many clinical pharmacists act in specialty areas such as organ transplantion, intensive care unit, oncology, nutrition, and anticoagulation service. Neverthless, insufficient manpower is the matter of facts on the realistic conditions and the limitation of expanding the action region of clinical pharmacists. And so, intensifying verification of the prescribed orders by dispensing pharmacists can contribute on the further better medication therapies towards much more patients. Therefore, the program of reviewing order by inpatient dispensing pharmacists was developed. The purpose of this study was to analize the record of intervention by pharmacists who didnt use this program, the record of intervention by pharmacists who did use this program and the record of intervention by clinical pharmacists participated on rounds and discuss on the necessity of the change of role of hospital pharmacists. In this study, considerably different results were showed in three groups of the records on the types of prescription error, the types of pharmacists recommendation and the significance of error. In view of the results so far achieved especially in the significance of error, the role of clinical pharmacists participated on rounds is much important for the therapeutic effect on inpatients. The addition of clinical pharmacist services collaboratively in the care of inpatients generally resulted in improved care and interacting with the health care team on patient rounds, interviewing patients, reconciling medications, and providing patient discharge counseling and follow-up all resulted in improved outcomes. So, continuing efforts on effectiveness all kinds of works of hospital pharmacists, such as automation of dispensing, are necessary.
