전기방사법을 통해 제조한 PCL/m-PEG/VEGF 소구경 혈관 도관의 물리적 특성 : Physical characteristics of electrospun PCL/m-PEG/VEGF small diameter vascular graft

Abstract
서론: Poly(Є-caprolactone) (PCL)과 methoxy-polyethylglycol (m-PEG)및 vascular endothelial growth factor (VEGF)를 재료로 전기방사법을 통해 제작한 소구경 인조 혈관 도관의 물리적 특성을 분석하고 소동물 이식 모델에 적용하기 위하여 필요한 개선점을 알아보고자 하였다.

대상 및 방법: 합성 고분자 중합체인 PCL/m-PEG를 재료로 전기방사법을 통해 소구경 인조 혈관 도관을 제조하여 전자현미경으로 도관 표면의 구조적 특성을 관찰하고 인장 응력, 인장 변형, 봉합의 적절성 등 기계적 특성을 조사하였다. Sprague-Dawley (SD) rat의 복부 대동맥에 이식하여 소동물 모델에 적용 가능 여부를 확인하였다.

결과: PCL/m-PEG/VEGF를 이용해 제조한 인조 혈관 도관은 전자 현미경상 다공성의 구조를 보였으며 도관 벽의 두께는 평균 407.56 ± 23.13 ㎛, 도관의 섬유 두께는 평균 6.02 ± 2.3 ㎛였고 미세 다공의 평균 직경은 7.73 ± 4.89 ㎛ 였다. 도관의 인장 응력은 평균 13.94±1.41 ㎫, 인장 변형은 91.67 ± 17.68 % 였다. 봉합사 지지력 (Suture retention strength)은 평균 0.41 ± 0.08 N 이었다. 복부 대동맥을 치환한 SD rat 30마리 중 28마리는 수술 중 혹은 48시간 이내에 사망하였고 2주 이상 생존한 2마리에서는 도관의 구조적 안정성이 유지됨을 확인하였다.

결론: PCL/m-PEG/VEGF으로 제작한 소구경 인조 혈관 도관은 양호한 구조적 특징과 인장력 및 생체 내 안정성을 보였다. 그러나 임상 적용 가능성 연구를 위한 소동물 모델 이식 실험을 성공적으로 수행하기 위해서는 봉합 용이성과 지지력이 개선되어야 하고 안정적인 이식 술기에 대한 연구가 선행되어야 할 것으로 사료된다.
Introduction: This study evaluated the physical characteristics of a small diameter vascular graft made of high molecular compounds with electrospinning method. The feasibility of application to a small animal experimental model was also evaluated.
Methods: We fabricated small diameter vascular graft composed of poly-Є-caprolactone (PCL), methoxy-polyethylglycol (m-PEG) and vascular endothelial growth factor (VEGF) by electrospinning method and investigated the morphologic features in scanning electron microscopy, tensile strength, tensile strain, suturability of graft. The operative outcome and postoperative gross and histologic findings were assessed after implanting the graft into the abdominal aorta of Sprague-Dawley (SD) rats.
Results: Electrospun PCL/m-PEG/VEGF small diameter vascular graft was found to maintain the porous nanostructure and showed mean 407.56 ± 23.13 ㎛ wall thickness, mean 6.02 ± 2.3 ㎛ nanofiber thickness, mean 7.73 ± 4.89 ㎛ diameter-sized pores. Mean tensile strength and mean tensile strain were 13.94±1.41 MPa and 91.67 ± 17.68 %, respectively. Suture retension strength was mean 0.41 ± 0.08 N. Among the 30 SD rats in which the prosthetic graft replaced the abdominal aorta, twenty-eight died during or within 48 hours after the operation. In two animals which survived more than 2 weeks, the implanted grafts remained stable without structural degradation.
Conclusion: Electrospun small diameter vascular graft made of PCL/m-PEG/VEGF showed acceptable structural features, tensile strength and in vivo stability. However, for wider and successful in vivo application to small animal models, suture retention strength should be improved and a reproducible implantation technique needs to be established.
