항관절염 식물의약 PG201의 항염증과 항산화 활성에 대한 분자 기전 : Molecular Characterization of Anti-inflammatory and Anti-oxidative Stress Activities of PG201, An Anti-arthritic Botanical Formulation

Abstract
PG201은 12가지 식물의 에탄올 추출물로서 강력한 관절염 치료 효과를 나타내었다. PG201은 콜라겐으로 유도한 류마티스성 관절염 마우스 모델과 콜라게네이즈로 유도시킨 골관절염 토끼 모델에서 관절염 증상을 효과적으로 완화시켰으며 [Shin et al., Rheumatology, 2003. 42(5): p. 665-72; Park et al., Biochem Biophys Res Commun, 2005. 331(4): p. 1469-77], 골관절염 환자를 대상으로 한 임상 3상 시험에서도 기존에 사용되어 온 관절염 치료제인 세레브렉스에 비하여 더 우수한 치료 효과를 나타내었다. 그러나 아직까지 PG201의 관절염 치료 효과에 대한 작용 원리는 자세히 밝혀지지 않았다. 이번 연구는 PG201의 활성에 대한 분자세포생물학적인 메커니즘을 이해하고, 또다른 염증성 질환에도 치료 효과를 나타내는지 조사하기 위해 수행되었다.
PG201은 여러 가지 식물에서 얻은 추출물로서 구성 성분이 매우 복잡하며 아직 유효 성분을 알지 못하기 때문에 일관된 품질의 연구 시료를 만드는 것이 매우 중요하였다. 따라서 본격적인 연구의 시작에 앞서 생물학적 활성을 이용한 cell-based bioassay를 확립하여 PG201의 품질을 검증하였다. 이를 위해 마우스 대식 세포주인 Raw264.7에서 PG201이 염증성 매개체인 IL (interleukin)-1β와 NO (nitric oxide)의 생산을 억제하는 활성을 이용하였다. 생물학적 활성을 정량적으로 분석하기 위해 IC50값을 지표로 설정하였다. PG201 표준배치의 IC50값과 비교하여 그 값이 20 % 이내에 형성될 경우 동등한 활성을 가진 배치로 판정하였다. 이렇게 확립된 cell-based bioassay를 이용하여 서로 다른 시기에 만든 시료들에 대한 품질 검증, 에탄올 추출 시간에 따른 활성 비교, 분획 연구 등에 다양하게 활용하였다.
PG201이 다양한 염증성 분자들의 발현을 조절하는 메커니즘을 알아보기 위하여 단백질, RNA, 전사인자, 신호전달 경로 등에 미치는 영향을 Raw264.7 세포에서 조사하였다. PG201은 IL-1β와 NO 뿐만 아니라 IL-6, CCL2 [chemokine (C-C motif) ligand 2], PGE2 (prostaglandin E2) 등 염증성 분자들의 생산을 억제하였다. Northern blot 분석 결과 PG201은 LPS (lipopolysaccharide) 자극에 의해 증가하는 IL-1β, IL-6, CCL2 사이토카인 발현을 RNA 수준에서 감소시켰다. PG201이 이 유전자들의 RNA 생산을 어떻게 조절했는지를 이해하기 위해 관련된 전사인자에 미치는 영향을 조사한 결과, PG201이 AP-1 (activator protein-1)과 CREB (cAMP response element-binding) 전사인자의 활성을 억제하여 RNA 생산을 감소시킨다는 것을 밝혔다. 그러나 PG201은 이 전사인자들을 활성화시키는 것으로 알려진 MAPKs (mitogen-activated protein kinases) 신호전달 경로에는 영향을 미치지 않는 것으로 나타났다. 또한 PG201은 프로스타글란딘의 생산에 중요한 효소인 cPLA2 (cytosolic phospholipase A2)의 발현을 단백질 수준에서 억제하였고 그 결과로 PGE2의 생산이 감소된다는 것을 밝혔다. 이러한 결과들을 종합해 볼 때 PG201은 다양한 사이토카인과 효소의 발현을 여러 단계에서 조절함으로써 항염증 활성을 나타내는 것으로 보인다.
PG201은 ROS (reactive oxygen species)와 NO의 생산을 억제하여 강력한 항산화 활성을 나타내었으며, HO-1 (heme oxygenase-1), BlvrB (biliverdin reductase B), GCLM (glutamate cysteine ligase, modifier), SOD2 (superoxide dismutase 2), catalase 등 항산화 유전자들의 발현을 증가시켰다. 또한 iNOS (inducible nitric oxide synthase)의 발현을 단백질 수준에서 조절하여 NO의 생산을 억제 하였는데, siRNA를 이용한 실험을 통해 PG201이 NO의 생산을 억제하는데 HO-1이 관여한다는 것을 밝혔다.
PG201이 HO-1의 발현을 조절하는 메커니즘에 대하여 상세하게 연구하였다. Northern blot 분석 결과 PG201은 HO-1의 발현을 RNA 수준에서 조절하였다. Reporter gene assay 실험을 통해 HO-1 프로모터 상에 존재하는 E1, E2 인핸서와 StRE (stress response element)가 HO-1의 발현에 중요한 역할을 한다는 것이 관찰되었다. Nrf2 knockdown 마우스를 이용한 실험을 통해 Nrf2 전사인자가 HO-1 발현에 필수적으로 작용함을 확인하였다. PI3K (phosphatidylinositol 3-kinase) 신호전달 단백질에 대한 저해제인 LY294002를 처리하였을 경우, PG201에 의해 증가하였던 Nrf2의 핵 이동과 HO-1의 발현이 억제되었다. 이를 통해 PG201은 PI3K/Akt 신호전달경로를 통해 Nrf2를 활성화 시키고 HO-1의 발현을 증가시킨다는 것을 밝혔다.
위와 같은 연구들을 통해 PG201이 강력한 항염증 및 항산화 활성을 가지고 있음을 알 수 있었다. 이러한 사실을 바탕으로 PG201은 관절염뿐만 아니라 다른 염증성 질환에도 효과를 나타낼 수 있을 것이라 생각되었다. 이를 조사하기 위하여 PG201이 염증성 자가면역 질환의 일종인 다발성 경화증에서 예방 혹은 치료 효과가 있는지 조사해 보았다. 다발성 경화증 동물 모델인 EAE (experimental autoimmune encephalomyelitis) 마우스에 PG201을 투여하였을 경우 마비 증상이 완화되었으며, 중추신경계로의 염증 세포 침윤 (infiltration)이 억제되었다. 마우스의 척수를 분석해본 결과 PG201은 EAE의 발병에 가장 중요하다고 알려진 IFN (interferon)-γ, IL-17, GM-CSF (granulocyte-macrophage colony-stimulating factor)의 RNA 발현을 강력하게 억제하였다. 비장 세포를 분리하여 MOG (myelin oligodendrocyte glycoprotein)로 재자극 하면서 배양하였을 때에도, PG201을 투여한 마우스로부터 분리한 비장세포에서는 IFN-γ와 IL-17의 발현이 억제되었다. CD4+ T 세포를 이용한 in vitro 분화 실험을 통해 PG201이 Th1 세포와 Th17 세포로의 분화를 억제한다는 사실을 밝혔다. 또한 PG201은 다발성 경화증 모델의 척수에서 TNF-α, IL-1β, IL-6, CCL2 등 염증성 사이토카인의 RNA 발현을 억제하였고, BV-2 세포주를 이용한 실험을 통해 PG201이 미세아교세포에 의한 염증성 분자들의 생산을 억제할 수 있다는 결과를 얻었다. 이러한 결과는 PG201이 다발성 경화증에서도 치료효과를 나타낼 가능성을 가지고 있음을 의미한다.
결론적으로 PG201은 다양한 종류의 분자에 영향을 미쳐서 강력한 항염증 및 항산화 활성을 나타내며 관절염과 다발성 경화증에서의 치료 효과는 이러한 성격을 통해 이뤄진다는 것을 알 수 있었다. PG201의 다양한 활성을 감안할 때 이 천연물 신약은 다양한 염증 질환에 대해 안전하고 효율적인 치료제로 사용될 수 있는 잠재력을 가지고 있음을 의미한다.
PG201, an ethanol extract from a mixture of 12 herbs, has been shown to contain anti-arthritic activities. It was previously reported that it produced significant therapeutic effects in both rheumatoid and osteoarthritis animal models [Shin et al., Rheumatology, 2003. 42(5): p. 665-72; Park et al., Biochem Biophys Res Commun, 2005. 331(4): p. 1469-77]. Furthermore, data from phase III clinical trials with OA patients showed that PG201 could generate more pain relieving effects than Cerebrex. However, the molecular mechanisms underlying such anti-arthritic effects of PG201 were not yet clearly understood. The present thesis research was performed to unravel the molecular and cell biological mechanisms of PG201, and to test its effects in other inflammatory diseases.
PG201 is a complex mixture and its active compound(s) has not been identified. Therefore, it was important to prepare PG201 in a consistent manner and thus to devise the method by which the quality of PG201, as a research and clinical reagent, was measured and controlled. In this study, the quality of PG201 was measured prior to all experiments, using cell-based bioassays. PG201 decreased the level of IL (interleukin)-1β and NO (nitric oxide), the major inflammatory molecules in the pathogenesis of arthritis, in Raw264.7 mouse macrophage cell line. Therefore, the suppressive effect of PG201 on these molecules was measured using cell-based bioassays involving this cell line. Only when the IC50 values from different batches fell within 20% of those from the reference reagent, the prepared samples were employed in the experiments. These bioassays were successfully used to control the quality of PG201 prepared at different times, for example, to compare the biological activities between batches from different ethanol extraction periods, and to study different fractions of PG201.
To investigate the mechanism underlying the PG201-mediated regulation of various inflammatory molecules, the effects of PG201 in Raw264.7 cells were studied at various levels. Treatment with PG201 down-regulated the LPS (lipopolysaccharide)-induced production of IL-6, CCL2 [chemokine (C-C motif) ligand 2], PGE2 (prostaglandin E2), and IL-1β as well as NO. Data from Northern blot analysis indicated that PG201 down-regulated the expressions of IL-1β, IL-6, and CCL2 at the RNA level. A gel retardation assay showed that PG201 inhibited the DNA binding activity of AP-1 and CREB. The expression of cPLA2 (cytosolic phospholipase A2), involved in the production of prostaglandins, was also reduced by PG201 at the protein level (but not at the RNA level), resulting in decreased PGE2 production. These results indicated that PG201 induced anti-inflammatory activities by regulating the expression of inflammatory cytokines and enzymes at multiple levels.
PG201 also showed strong anti-oxidative activities by decreasing the production of ROS and NO. PG201 increased the RNA level of various anti-oxidative genes, including HO-1 (heme oxigenase-1), BlvrB (biliverdin reductase B), GCLM (glutamate cysteine ligase, modifier), SOD2 (superoxide dismutase 2), and catalase. In addition, PG201 inhibited the LPS-induced NO production by decreasing the protein expression of iNOS (inducible nitric oxide synthase). The knockdown experiment involving siRNA specific for HO-1 showed that the induction of HO-1 by PG201 might be an important factor on PG201-mediated suppression of iNOS and NO production.
The molecular mechanisms of PG201-mediated induction of HO-1 expression were further investigated using Raw264.7 cells. Data from reporter gene assays indicated that PG201 induced the HO-1 promoter activity through the StRE (stress response element), located in E1 and E2 enhancers of the HO-1 promoter, to which Nrf2 (NF-E2-related factor 2) binds. When cells from Nrf2 knockout mice were used, the PG201-mediated induction of HO-1 expression was significantly decreased as compared with the level seen in wild type cells, showing that Nrf2 transcription factor plays a critical role in the PG201-mediated HO-1 expression. PG201-mediated induction of HO-1 and Nrf2 expression were suppressed in the presence of LY294002, an inhibitor of PI3K (phosphatidylinositol 3-kinase), suggesting that PI3K/Akt might be involved in the activation of Nrf2 and subsequent induction of HO-1.
Results from various in vitro and in vivo experiments indicated that PG201, containing both strong anti-inflammatory and anti-oxidative activities, might have therapeutic potential for other inflammatory diseases. In this study, it was examined whether PG201 could be applied to the prevention or treatment of multiple sclerosis, a well-known inflammatory autoimmune disease using mouse EAE (experimental autoimmune encephalomyelitis) model. Oral administration of PG201 could ameliorate the characteristics of EAE, such as paralysis and weight loss, and prevent immune cell infiltration of the CNS (central nerve system). Consistent with these observations, down-regulation of various inflammatory cytokines was observed in the CNS, including IFN (interferon)-γ, IL-17, GM-CSF (granulocyte-macrophage colony-stimulating factor), TNF-α, IL-1β, IL-6, and CCL2. PG201 inhibited MOG (myelin oligodendrocyte glycoprotein)-restimulated induction of IFN-γ and IL-17, and the differentiation of Th1 and Th17 cells in splenocytes from EAE animals. Results from the experiments using BV-2 microglia cell line suggest that PG201 could repress the expression of various inflammatory mediators. Altogether, data indicated that PG201 might be used for the prevention or treatment of multiple sclerosis.
In conclusion, the present thesis showed that PG201 has strong anti-inflammatory and anti-oxidative stress effects by affecting multiple molecules at various levels, including the signal transduction pathway (PI3K/Akt), transcription factors (AP-1, CREB, and Nrf2), RNA (IL-1β, IL-6, CCL2, HO-1, and etc.), and protein (iNOS and cPLA2). Taken together, PG201 appears to have great potential as a safe and effective agent for a variety of inflammatory diseases.
