Construction of Integrative Causal Model of Insight and its Validation and Application in Patients with Schizophrenia : 조현병 환자의 병식에 대한 통합적 모델 구축 및 검증

Abstract
The term insight in mental illness has evolved in its definition and conceptualization to favor multi-dimensional approach, but research on insight and its relationship with psychopathology has yielded inconsistent results. Some findings support the direct causal link between the severity of psychopathology and insight, but others have failed to replicate such findings and instead have posited that insight should be considered as a separate domain of psychopathology. However, these studies mostly have not considered the complex inter-relationships among the symptom complexes and how they may affect insight through different stages of illness.
The overall aim of this thesis is therefore to present a series of studies to advance the conceptualization of insight as a symptom in schizophrenia that is 1) divisible from the traditionally defined domains of psychopathological symptoms and neurocognitive deficits, 2) a non-continuous phenomena, in which the relationships between insight and psychopathological and neurocognitive domains characteristically differ according to different stages of illness, and 3) reflected by the accuracy of the patients judgment of not only his/her symptoms but also other clinically related variables, such as subjective well-being and side effects, in relation to the evaluation of the clinicians.
Chapter 1 presents a brief overview of historical conceptualization and definition of insight, etiological models of lack of awareness of illness, and present status of insight research in schizophrenia.
In Chapter 2, it is hypothesized that different symptom domains interacted among each other in relation to insight and that there is a need to better refine such domains in order to construct conceptual model of insight. Hence, in Study 1, five symptom domains derived from factor analytic approach were applied to construct the causal model of insight and psychopathology, along with other feasible alternative models with structural equation modeling method (SEM). Such refinement was deemed necessary to clearly determine the independence between insight and psychopathology. As a result, the hypothesized causal model of insight, in which positive, negative, and autistic preoccupation (cognitive) symptoms were positioned as the primary predictors of insight with the activation factor mediating between positive and autistic preoccupation symptoms and insight, was found to satisfy all indices of goodness-of-fit. Hence, the activation symptoms played not only a mediating role for positive symptoms but also a moderating role for autistic preoccupation symptoms in predicting insight. In Study 2, a subgroup of same patients was followed longitudinally for longitudinal validation of the causal model of insight. Despite the temporal changes in the magnitude of associations among symptoms and their respective predictive strength, our causal model of insight was proven to reliably capture the dynamics between psychopathology and insight in schizophrenia patients at both stabilized (8-week) and chronic (1-year) stages. Also, the post hoc analysis of the causal relationship between depression and insight supported the notion that insight causes depression, rather than the opposite, partially discounting the psychological defense model of insight. In Study 3, the results obtained from the above studies were replicated with another group of patients with schizophrenia using self-reports rather than clinician-assessments of psychopathology. Hence, the causal model of insight constructed with this group also satisfied all indices of goodness-of-fit and self-reported symptoms. The clinician-rated psychopathology significantly correlated only among the patients with insight, and self-reported psychoticism and lack of insight were moderated by self-reported paranoid ideation and hostility. Lastly, in Study 4, differences between the uni-dimensional and multi-dimensional measure of insight in terms of their association with the psychopathology and validity of the model were examined. The results showed that the model constructed with the multi-dimensional measure of insight has relatively more percentage of variance, which can be explained by the contribution from the negative symptoms. The common finding from the above line of studies was that the relationship between insight and psychopathology qualitatively shifted according to the stage of illness and that the level of association between insight and psychopathology was at most moderate. In short, the notion that insight should be considered as a distinct primary symptom has been supported.
Included in Chapter 3 are studies that 1) expanded the present model to include neurocognitive functions by examining both the cross-sectional and longitudinal relationships between insight and neurocognitive variables, and 2) formulated how insight and other clinical variables predict subjective well-being in chronic patients of schizophrenia. Specifically, in Study 1, various domains of neurocognitive functioning, such as general intelligence, executive functioning, verbal and spatial memory, attention, and psychomotor speed, were examined in relation to insight. As a result, COWAT perseverative response, an executive function measure, was found to be the only neurocognitive functioning measure that predicts both cross-sectional level of and short-term longitudinal changes in insight. The cross-sectional model that included this variable as an intermediary variable between psychopathology and insight showed superior goodness-of-fit for both baseline and 8-week assessment. In Study 2, the possibility of expanding the model of insight in chronic patients of schizophrenia was examined by including primary predictors such as psychopathology and executive functioning, and intermediary variables such as insight and subjective side effects in predicting the quality of life. As a result, it was found that insight and executive functions mediate between psychopathology and side effects, and between psychopathology and QOL to determine the cumulative dysfunction in daily life for patients with chronic schizophrenia.
Chapter 4 presents two studies that demonstrate the feasibility of broadening the operational definition of insight and its conceptual paradigm. In other words, they deal with the conceptualization of insight that is based not only on the awareness and labeling of symptoms but also on the understanding of the ones overall level of functioning and physical discomfort, including side effects of medication. Hence, in Study 1, the patients were stratified according to the level of correspondence between self-reported QOL and clinician-rated functioning based on the assumption that those with good insight would show higher levels of correspondence. As a result, those with higher levels of correspondence showed associations between patient- and clinician-rated psychopathology and with other measures that were stronger and more consistent than those of other discordant groups. In Study 2, drug-free patients mostly without insight were followed for 6-weeks after the patients were divided according to the presence of insight, measured for their heart rate variability, and assessed by the clinician for side effects. They also provided self-reports of side effects. As a result, those with insight showed higher and more consistent associations not only between the measures of side effects but also between self-reported side effects and HRV parameters. Such results confirmed our hypothesis that the conceptualization of insight should not be confined to the awareness of illness, but expanded to include the understanding of a broader range of consequences of illness.
Lastly, this thesis concludes with a brief overview of the findings, overall limitations of the studies, and implications for future studies.
정신장애에서의 병식의 정의와 개념화에 대한 연구는 다차원적인 접근을 선호하는 방향으로 전개되어왔다. 하지만 병식과 정신병리간의 관련성에 대한 연구는 일관되지 않은 결과를 보였다. 그 중 일부는 정신병리의 심각도와 병식 간의 직접적인 인과론적인 연결성을 지지하는 반면, 기타 연구에서는 이러한 결과를 재연하는 데에 실패하여 병식이 정신병리와는 구별될 수 있는 영역으로 간주되어야 한다고 주장했다. 하지만 이런 일련의 연구들은 대부분 증상 영역간의 복잡한 상호관계와 그들이 상이한 질병의 단계에서 병식에 미치는 영향의 변화를 고려하지 못했다.
따라서 이 논문의 전체적인 목표는 조현병 환자의 병식을 다음과 같이 개념화하고 그것을 지지하는 일련의 연구를 제시하는 데에 그 목적을 두고 있다: 1) 병식은 조현병에서 전통적으로 정의된 정신병리 영역 및 인지 손상과 구분될 수 있는 증상이며, 2) 병식과 정신병리 및 신경인지 영역간의 관계는 질병의 단계에 따라 특징적으로 다른 비연속적인 현상이고, 3) 병식은 자신의 증상뿐만 아니라 주관적 안녕과 부작용과 같은 임상적 변인들에 대한 환자의 자기보고와 임상가의 평가가 일관되는 것에 의해 반영될 수 있다.
제 1장은 병식의 역사적 개념과 정의, 병에 대한 인식의 결여에 대한 원인론적 모델, 그리고 조현병에서 병식 연구의 현주소 등에 대한 간략한 개관을 제시하였다.
제 2장에서는 병식과 관계되는 상이한 증상 영역들 간에 서로 상호작용이 있는 것으로 알려져있기 때문에 병식에 대한 개념적 모델을 구축하기 위해 증상영역을 기존보다 더 정교하게 정의할 필요가 있다는 가정을 토대로 다음과 같은 일련의 연구를 제시하였다. 연구 1에서는 요인분석 기법을 통해 5가지 증상 영역을 추출했고, 구조방정식 모델링 기법을 사용하여 이를 병식과 정신병리의 인과적 모델 및 대안적 모델을 구축하였다. 이러한 정교화 작업은 병식이 정신병리 영역들과 심각도와 구분될 수 있는지를 명확하게 판단하기 위한 목적 하에 진행되었다. 그 결과, 우리가 제시한 병식의 가설적 모델이 (즉, 양성증상, 음성증상, 그리고 자폐적 몰입(인지) 증상영역이 병식의 일차적 예측변인이고 활성화증상이 양성증상 및 자폐적 몰입증상 영역들과 병식 사이에서 이차 예측변인으로 작용하는 모델) 모델의 적합성을 판단하는 모든 기준치를 충족시키는 것으로 나타났다. 연구 2에서는 동일한 환자군의 일부를 1년간 추적하여 병식의 인과적 모델의 종단적 검증을 실시하였다. 그 결과, 시간에 따른 증상들 간의 상관성의 강도 및 병식에 대한 각 증상의 예언력 등의 변화에도 불구하고, 병식의 인과론적 모델은 안정적인 단계 (연구 시작 후 8주) 및 만성적인 단계 (연구 시작 후 1년) 모두에서 조현병 환자의 병식과 정신병리간의 역동을 높은 신뢰도로 기술하는 것으로 나타났다. 또한 사후 분석을 통해 우울증상이 병식에 영향을 주는 것보다는 병식이 우울증을 유발한다는 단서를 얻어 병식의 심리적 방어 모델을 부분적으로 반박하는 증거를 얻었다. 연구 3에서는 위의 연구들에서 얻어진 결과를 또 다른 조현병 환자 집단을 대상으로 정신병리에 대한 임상가의 평가가 아닌 환자의 자기보고형 척도를 사용하여 재검증하였다. 그 결과, 이 집단의 자료를 사용하여 구축한 병식의 인과적 모델 또한 모든 적합도 측정치 기준에 부합하였고, 병식이 있는 환자들만 정신병리에 대해 자기보고와 임상가의 평가간에 유의미한 상관을 보였고, 자신이 보고한 정신병적 증상과 병식의 결여 사이를 자신이 보고한 편집증적 사고와 공격성이 중재한다는 결과를 얻었다. 마지막으로, 연구 4에서는 병식의 일차원적인 측정치와 다차원적인 측정치간에 정신병리와의 관련성 및 모델의 타당도 측면에서 어떤 차이가 있는지를 확인하였다. 분석결과, 다차원적인 병식 척도를 적용하여 구축한 모델이 병식에 대해 상대적으로 더 많은 변량을 설명하였는데, 이것은 대부분 음성증상들이 더 많은 예측력을 보였기 때문이었다. 이런 일련의 연구들에서 얻어진 공통된 결과는 병식과 정신병리간의 관계가 질병의 단계에 따라 질적인 변화를 보였고 병식과 정신병리간의 상관이 높지 않아서 병식이 여타 증상들과 구별될 수 있는 일차적 증상이라는 가설을 지지했다.제 3장에는 위의 모델을 확장시켜서 신경인지기능을 모델에 포함시킨 횡•종단적 연구를 실시하였고 병식 및 다른 임상적 변인들을 토대로 만성 조현병 환자들의 주관적 안녕을 예측하는 모델을 구축하였다. 구체적으로 연구 1에서 일반 지능, 집행기능, 언어 및 공간 기억, 주의력, 그리고 심리운동(정신운동?) 속도와 같은 신경인지 기능의 다양한 영역이 병식과 어떠한 관련성이 있는가를 살펴보았다. 그 결과, 단지 집행기능을 측정하는 통제된 단어 연상 검사의 (Controlled Oral Word Association Test) 보속 반응이 병식의 횡단적 수준 및 단기적 종단적 변화를 예측하는 변인으로 밝혀졌고, 이 변인을 정신병리와 병식 사이의 중간 변인으로 지정한 기저 및 8주의 횡단적 모델은 모두 뛰어난 수준의 적합도를 보였다. 연구 2에서는, 현 모델을 만성 조현병 환자들의 주관적 안녕감을 예측하는 모델에 적용하여 확장시키는 시도를 하였는데, 일차적 예측 변인들로 정신병리와 집행기능, 그리고 이차적 중간 변인들로는 병식과 부작용에 대한 자기보고를 포함시켰다. 그 결과로, 병식과 집행기능이 정신병리와 부작용 사이와 정신병리와 주관적 안녕 사이를 매개하여 만성 조현병 환자들의 일상 생활에서의 역기능을 결정하는 것으로 나타났다.
제 4장에서는 병식의 조작적 정의와 개념적 파라다임을 확장시키는 것에 대한 가능성을 검증하는 두 개의 연구를 제시하였다. 따라서 연구 1에서는 병식이 있는 환자는 자신이 보고한 삶의 질과 임상가가 평가한 기능이 높은 수준의 합치(일치?)를 보일 것이라는 전제하에 합치도(일치도?)에 따라 환자군을 나누어서 두 군간의 차이를 살펴보았다. 그 결과, 높은 합치도를 보이는 환자집단이 환자와 임상가가 평가한 정시병리 척도 간에도 유의미한 상관을 보였고, 이 척도들은 다른 측정치들과 더 강하고 일관된 상관관계를 보였다. 연구 2에서는 약물을 먹지 않은 병식이 없는 환자들만을 대상으로6주간 치료 추적하였을 때 환자의 병식 여부에 따라 환자군을 구분하였다. 각 환자군에서 심장 박동 변화를 측정하고, 임상가에 의한 부작용 평가와 환자들에 의한 부작용 자기보고를 실시하였다. 그 결과, 병식이 있는 환자들은 부작용 측정치들 간에 더 높고 일관적인 상관을 보였을 뿐 아니라, 자신이 보고한 부작용과 특정한 심장 박동 변화 측정치들과도 상관을 보였다. 이러한 연구 결과들은 병식의 개념을 질병에 대한 인식에만 국한하지 말고 질병의 결과에 따르는 다양한 영역에 대한 인식을 수반하는 개념으로 확장시켜야 한다는 본 연구의 가설을 지지하였다.
