- 예방 가능한 입원(preventable hospitalization)은 효과적인 만성질환관리와 적절한 일차의료서비스 이용을 통하여 막을 수 있는 입원으로, 입원 전 단계에 발생하는 일차의료 및 예방서비스가 부족하다는 것과 밀접한 관련이 있다. 이러한 예방 가능한 입원을 줄이는 것은 보건의료서비스 이용의 형평성과 자원배분의 효율성을 증진시킬 수 있으며, 불필요한 서비스 이용을 줄이고 의료비를 절감할 수 있을 것으로 기대된다. 본 연구에서는 이러한 예방 가능한 입원에 영향을 미칠 수 있는 요인들 중 상용치료원(usual source of care)의 역할에 주목하였다.
환자가 자주 방문하는 의료기관에서 서비스를 제공하는 상용치료원은 양질의 일차의료원으로서의 역할인 지속성, 포괄성, 조정기능을 가져야 한다. 본 연구에서는 의원급 의료기관에서 주치의를 보유한 경우를 상용치료원으로 정의하고, 상용치료원의 보유뿐만 아니라 양질의 일차의료 속성인 지속성, 포괄성 및 조정기능을 충족한 상용치료원을 이용하는 것이 예방 가능한 입원에 어떠한 영향을 미치는지 살펴보고자 하였다.
본 연구에서는 한국의료패널 2012년, 2013년, 2016년 연간데이터를 활용하여 상용치료원 보유여부가 예방 가능한 입원에 미치는 영향을 로지스틱 패널회귀모형으로 분석하였으며, 상용치료원의 지속성, 포괄성 및 조정기능을 주요독립변수로 고려하여 이들이 각각 예방 가능한 입원에 미치는 영향을 합동 로지스틱 모형으로 분석하였다.
그 결과, 상용치료원 보유 및 양질의 일차의료 속성을 갖춘 상용치료원은 예방 가능한 입원에 대하여 유의한 영향을 미치지 않았다. 비록 상용치료원 보유여부와 양질의 일차의료속성을 갖춘 상용치료원 보유는 예방 가능한 입원에 미치는 영향이 통계적으로 유의하지 않았으나, 본 연구에서는 이러한 결과를 바탕으로 국내 일차의료 강화에 대한 시사점을 제공하고, 이를 위한 정책적 제언을 수행하고자 한다.
Preventable hospitalization is the admission that could be prevented through effective chronic disease management and proper use of primary care services, thus closely related to the lack of primary care. This study considered the role of a usual source of care as a quality primary care provider. Quality primary care attributes include first contact, longitudinality, comprehensiveness and coordination. This study explored how preventable hospitalization is affected by not only having a usual source of care but also having it with quality primary care attributes.
With 2012, 2013, and 2016 Korean Health Panel, this study used logistic random-effects model and pooled logistic model to examine the effects of having a usual source of care with primary care attributes on preventable hospitalization. The results show that having a usual source of care and quality primary care attributes had no significant impact on preventable hospitalization, which seem to result from the perverse incentive and payment systems for health providers, e.g., fee-for-service, and the over-supply of hospital beds in Korea.
