선청성 세포비대성 바이러스감염에 수반된 Ganglionic Eminence Cyst에 의해 유발된 수뇌증

Abstract

An autopsy case of hydrocephalus whicb is presumably caused by partial obstruction of Foramen Monroe in association with congenital cytomegalovirus infection is reported. This male baby was born to a 23 year old primiparous mother after 36 weeks gestation. The pregenancy ''I:a5 uneventful except for a gradual enlargement of fetal head from the Sth month of gestation. After birth through Cesarian section because of breech presentation, the baby developed marked cyanosis and respiratory distress and died 60 hours later. Postmortem examination revealed right cleft lip and palate, abnormal lobation of lung and hepatosplenomegaly. Coronal sections of brain revealed partial obstruction of foramen Monroe by ganglionic eminence cyst and dilation of lateral ventricles. The 3rd and 4th ventricles were not dilated. Characteristic cytomegalic inclusion bodies were seen III the kidneys, liver, pancreas and lungs, together with a marked extramedullary hematopoiesis in viscera. Interstitial pneumonitis and bronchopneumonia were also noted. The microsopic findings of the brain consisted of periventricular white matter damage, i.e., hypertrophic astrocytosis, rarefaction and capillary proliferation. The ganglionic eminence cyst consisted of multiple trabeculae with wasted matrix cells and some degree of gliosis. No inclusion body was seen in ependymal or glial cells. A small focus of calcification below the ependyma was present.
