신체상에 대한 심리적 경직성이 신체변형장애 증상에 미치는 영향

Abstract

심리적 경직성은 개인이 추구하는 가치나 상황적 맥락에 맞춰 행동을 효과적으로 조율하지 못하는 심리적 특성으로서, 정신 병리의 핵심기제 중 하나로 밝혀지고 있다. 본 연구는 신체상에 대한 심리적 경직성을 신체변형장애 발달 과정에 영향을 미치는 잠재적 취약요인으로 상정하고 관련 변인들과의 관계를 탐색하고자 하였다. 이를 위해 연구 Ⅰ에서는 신체상에 대한 심리적 경직성 정도를 측정하는 신뢰롭고 타당한 평가도구를 마련하고자 하였다. 신체상과 관련된 고통스러운 내적 경험을 온전히 수용하지 못하고 이를 회피하거나 변경하려는 심리적 경직성을 측정할 수 있는 신체상 심리적 경직성 척도(Body Image Psychological Inflexibility Scale: BIPIS)를 번안하고 요인구조, 신뢰도 및 타당도를 확인하였다. 총 329명의 만 18세 이상 성인을 대상으로 수집한 자료를 분석한 결과, 신뢰도는 양호한 수준이었으며 요인구조는 원척도의 단일 요인 구조를 따르는 것으로 확인되었다. 다른 척도들과의 상관관계를 분석한 결과, 외모 수용 척도, 신체상 대처전략 척도와 같이 기존의 유사 척도 및 관련 있는 심리적 특성들과 유의미한 상관을 보였으며, 전반적인 심리적 경직성보다 신체변형장애 증상을 더 잘 예측하여 타당도가 양호한 것으로 나타났다. 아울러 연구 Ⅱ에서는 부적응적 완벽주의, 외모 도구성 및 신체변형장애의 관계에서 신체상 심리적 경직성의 매개역할을 살펴보았다. 총 278명의 만 18세 이상 성인을 대상으로 수집한 자료를 분석한 결과, 신체변형장애의 심리적 취약요인인 부적응적 완벽주의, 외모 도구성 신념이 신체변형장애 증상에 미치는 영향에서 신체상에 대한 심리적 경직성이 완전매개하는 것으로 나타났다. 본 연구는 신체상 심리적 경직성을 측정할 수 있는 척도를 국내에서 사용 가능하도록 번안 및 타당화하고 심리측정적 특성과 역할을 탐색함으로써, 추후 관련 연구에 활용될 수 있는 기반을 마련하였다. 또한 신체변형장애와 관련 변인들 사이의 관계에서 신체상에 대한 심리적 경직성의 역할을 확인함으로써 신체변형장애의 기제에 대한 이해를 넓혔다는 점에서 의의가 있다. 마지막으로 본 연구가 지니는 한계점에 대해 논의하였다.

Psychological inflexibility is characterized by the avoidance of ones own cognitive and affective states at a cost to personal values. Acceptance and Commitment Therapy (ACT) suggests that psychological inflexibility is a central vulnerability that can lead to psychological maladaptation. This study aims to explore the role of psychological inflexibility regarding ones body image in the development of body dysmorphic disorder. Study Ⅰ was conducted to develop the Korean version of the Body Image Psychological Inflexibility Scale (BIPIS: Callaghan, Sandoz, Darrow, & Feeney, 2015). Data was collected from 329 adults to conduct exploratory factor analysis and confirmatory factor analysis. The results of factor analyses supported the one-factor structure of K-BIPIS. Also, the K-BIPIS displayed good reliability and had good convergent and concurrent validities. Moreover, the K-BIPIS predicted variability within body dysmorphic symptoms above and beyond that of the AAQ-Ⅱ assessing general psychological inflexibility. Study Ⅱ was designed to examine a mediation model using structural equation modeling in a sample of 278 adults. In the model, body image psychological inflexibility was expected to exert a mediation effect on the relationship among maladaptive perfectionism, social instrumentality of appearance, and body dysmorphic symptoms. The results were as follows. First, body image psychological inflexibility had a significant positive correlation with maladaptive perfectionism, social instrumentality of appearance, and body dysmorphic symptoms, which was consistent with the research hypothesis. Second, body image psychological inflexibility appeared to have mediating effects between maladaptive perfectionism and body dysmorphic symptoms, and between social instrumentality of appearance and body dysmorphic symptoms. Also, maladaptive perfectionism and social instrumentality of appearance had only indirect effects on body dysmorphic symptoms via body image psychological inflexibility. The present study demonstrates that the K-BIPIS is a reliable and valid tool for assessing psychological inflexibility of body image in Korean adults. Furthermore, it illustrates how psychological inflexibility regarding body image plays a key mediating role between maladaptive perfectionism and body dysmorphic symptoms, as well as between social instrumentality of appearance and body dysmorphic symptoms. Lastly, implications of the findings, limitations, and suggestions for further research were discussed.
