Abstract
연구 배경

전 세계적으로 담배 제품 광고 및 홍보 규제가 강화되면서 담배 회사는 제품 판매 촉진을 위해 담뱃갑 포장을 주요 마케팅 수단으로 활용해 왔다. 이 때문에 담배규제기본협약은 흡연의 위험을 알리는 경고 문구를 담배 제품 포장에 의무적으로 부착할 것을 당사국에 요구하고 있으며 이때, 그림(사진)까지 함께 표기하도록 하고 있다.

담뱃갑 경고 그림은 흡연의 폐해를 직접적으로 알려주기 때문에 흡연의 위험성을 경고 문구보다 더 정확하고 직접적으로 전달할 수 있다. 이로 인해 경고 그림은 흡연자들의 금연을 유도할 뿐만 아니라 비흡연자, 특히 어린이, 청소년들의 흡연 시작을 예방하는 데 효과적이다. 이러한 효과 때문에 2001년 캐나다를 시작으로 전 세계 100개 이상의 국가에서 담뱃갑 경고 그림을 도입하고 있으며, 우리나라는 2016년 12월 23일부터 도입하였다.

하지만 우리나라의 경우 경고 그림 도입하려는 시도는 2002년부터 시작되었으나 2016년이 되어서야 비로소 정책이 시행되었다.

담뱃갑 경고 그림 도입을 비롯하여 담배 가격 인상, 금연 구역 확대 등 담배 규제 정책 추진 과정은 서로 유사하고 전 세계적으로 비슷한 양상을 보이기 때문에 정책 과정에 대한 분석은 추후 담배 규제 정책 추진에 도움이 된다. 또한, 정책에 대한 체계적 분석은 물론 담배 규제 정책을 방해하는 담배업계에 효과적으로 대응하기 위해서는 과학적 근거 마련이 필수적이다.



연구 목적

본 연구의 목적은 담뱃갑 경고 그림 도입 과정과 도입 후 효과 분석을 바탕으로 향후 담뱃갑 경고 그림과 담배 규제 정책 추진 방향을 제시하기 위함이다. 우선, 한국의 담뱃갑 경고 그림 도입 입법 과정에서 정책 입안자들 사이에서 대두된 쟁점을 파악하고 이를 해결해 나가는 과정을 통해 담배 규제 정책 저해 활동에 대처하는 글로벌 담뱃갑 정책 및 담배 규제 전략을 제시하고자 한다. 또한, 경고 그림 도입 과정에서 대두된 쟁점 가운데, 경고 그림 위치에 따른 효과 차이를 살펴보고자 한다. 마지막으로 경고 그림 도입 후 경고 그림이 청소년들의 흡연에 대한 인식과 태도 변화에 미친 영향을 살펴보고 향후 청소년 흡연 예방을 위한 효과적인 담뱃갑 경고 그림 및 담배 규제 정책 방향을 제시하고자 한다.

연구 방법

우리나라의 담뱃갑 경고 그림 도입 입법 과정을 통해 정책 대안 선택 과정에 미치는 요인을 파악하기 위해 국회 의안 정보 시스템에서 경고 그림 도입을 요구하는 국민건강증진법 개정 의안 원문, 관련 보고서, 국회 회의록을 수집하였다. 국회 회의록 가운데 경고 그림 도입과 관련된 내용을 논의하는 부분을 발췌해 정리하였다. 또한, 150건의 온라인 신문 기사를 유형(뉴스, 컬럼)에 따라 담배 규제 정책에 대한 관점(긍정, 부정, 혼합 or 중립)을 분류하고 그 내용을 검토하였다.

담뱃갑 경고 그림 위치에 따른 시선의 차이를 살펴보기 위해 아이 트랙킹(eye-tracking)을 활용하여 경고 그림을 보는 시간을 측정하였다. 2종(피부 노화, 독성 물질)의 경고 그림을 위치(상, 중, 하)에 따라 배치하고 성인 54명(남성 30명, 여성 24)은 총 6종의 경고 그림을 각 3,000miliscond(ms) 응시하도록 하고, 각 영역별 응시 시간을 측정하였다.

경고 그림 도입 후 우리나라 청소년들이 최근 1달간 해당 경고 그림에 노출된 적이 있었는지와 노출된 경고 그림이 당시에 청소년들의 흡연에 대한 인식에 영향을 미쳤는지를 살펴보기 위해 2017년 청소년 건강 행태 온라인 조사 결과를 활용하였다.



연구 결과

우리나라의 담뱃갑 경고 그림 도입과 세부 사항이 결정되기까지 총 14년이 소요되었다. 경고 그림이 담배 산업에게 피해를 끼치며, 추가적인 비용을 발생시키고, 흡연자의 기본권과 인격권을 침해하기 때문에 지나친 혐오감을 주어서는 안 된다는 주장이 입법 과정에서 제기되면서 시행이 지연되었다. 이 후 경고 그림 세부 사항을 결정하는 과정에서는 담배업계가 입법 과정에 개입하면서 경고 그림의 상단 배치의 효과가 과학적으로 입증되지 않았으므로 경고 그림을 하단에 배치해야 한다는 의견이 제기되면서 또다시 지연되었다. 하지만 담배업계의 주장은 과학적 근거 기반의 연구 결과와 언론의 지지를 바탕으로 해결되었다.

담뱃갑의 경고 그림에 따라 시선의 점유율은 차이가 있었다. 특히, 하단에 위치할 때보다 중간 또는 상단에 위치할 경우 약 30% 시선 점유 시간이 증가하였다. 반면, 상단과 중간의 시선 점유 시간의 차이는 통계적으로 유의하지 않았다.

경고 그림 도입 6개월 후 남학생의 66.6%, 여학생의 72.4%는 최근 한 달간 경고 그림을 본 적이 있는 것으로 나타났다. 경고 그림을 본 후 흡연이 건강에 해롭다고 생각할 가능성과 흡연하지 않아야겠다는 생각을 할 가능성은 고학년보다는 저학년, 최근 1년 내에 흡연 예방 교육을 받은 경험이 있거나 금연 캠페인을 본 적이 있는 경우에 높았다. 반면, 현재 흡연자, 전자 담배 사용자, 간접흡연 노출 경험이 있는 경우 그 가능성은 낮았다.



결론

담뱃갑 경고 그림 도입 과정을 통해 담배 규제 정책 추진에 있어 정부의 노력과 입법 추진 기관(국회 등)의 투명성 및 합의가 요구되며 무엇보다 담배업계의 저해 활동을 효과적으로 차단하기 위해서는 과학적 근거의 지속적인 마련과 여론의 지지가 필요하다. 또한, 경고 그림을 비롯하여 담배 규제 정책 추진의 효과를 제고하기 위해서는 교육과 홍보가 지속적으로 추진되어야 하며 사회적으로 담배의 비규범화(De-normalization)를 위한 노력이 요구된다. 따라서 향후 경고 그림의 면적 확대를 비롯하여 표준화 담뱃갑(Plain packaging, Standardized packaging) 도입이 빠른 시일 내에 도입되어야 한다.
Background

As tobacco advertising, promotion, and sponsorship regulations have strengthened globally, tobacco packaging is an important marketing tool for tobacco companies. Packaging of products is a major source of expanding sales by communicating product information and meeting the changing needs of consumers. Cigarette packaging plays an important role in selecting and changing tobacco products.

The use of graphic health warning labels (GHWLs) is an effective policy for informing and persuading smokers and non-smokers about the negative consequences associated with tobacco products and smoking. GHWLs not only induce smoking cessation among smokers, but also are effective in preventing smoking initiation among non-smokers, especially young people.

For this reason, Article 11 (Packaging and Labeling of Tobacco Products) of the World Health Organization Framework Convention on Tobacco Control (FCTC) and its guidelines emphasize that well-designed health warnings on tobacco product packaging are a cost-effective way to strengthen public awareness of the risks of using tobacco products. In particular, Article 11 of the FCTC states that tobacco packaging and labeling should include warning text and pictures or pictograms, and that parties are obliged to comply within 3 years after ratification of the convention.

Starting from Canada, which implemented GHWLs in 2001, 105 nations around the world adopted the policy by 2016. In South Korea, GHWLs were introduced on December 23, 2016.



Objective

This thesis aims to investigate the implementation of GHWLs on tobacco products in South Korea. Furthermore, this thesis assess to how effective the warnings among Korean, and seek to how to enhance the effectiveness. Based on these results, I will suggest a strategy for implementing tobacco control policies including GHWLs.



Methods

First, a content analysis was conducted on Assembly records, meeting documents from the 15th (1995–2000) to the 19th (2012–2016) Assemblies, and online newspapers from April 22, 2016 to May 13, 2016.

Second, a cross-over design with health warning position (top, middle, and bottom) and image concept (skin aging and toxic constituents) as within-subject factors, and smoking status (smoker and non-smoker) as between-subject factors was conducted to measure the fixation duration of eye movements towards both pictorial warnings and branding on cigarette packs using eye-tracking equipment. Participants (30 daily smokers and 24 non-smokers) were shown six cigarette packs in random order with different position combinations (top, middle, and bottom) and image concepts (skin aging and toxic constituents).

Third, the cross-sectional analysis was conducted the responses of 62,276 students—31,624 boys and 30,652 girls—from data collected for the 2017 Korean Youth Risk Behavior Web-based Survey. Complex samples logistic regression was applied to explore the relationship between GHWLs and perceived effectiveness among students who seen GHWLs on tobacco products in the past 30 days.



Results

In South Korea, it took 14 years to implement GHWLs and determine the associated details. The implementation of GHWLs was delayed in the legislative process, because of the argument that the warnings should not be overly disgusting, as this would infringe on the basic rights of smokers. In the process of determining the technical details, another form of interference from the tobacco industry emerged, namely, the argument that the warnings should be placed at the bottom of cigarette packs, because the effectiveness of placing them at the top had not been sufficiently proved. However, this was overcome through scientific evidence and media support.

Participants fixated longer on the health warning area than on the tobacco branding area (p < 0.05). Mean fixation duration on the health warning area was significantly higher at the top or middle positions compared to bottom, by 28% (mean difference = 340 ms, p = 0.006) and by 30% (mean difference = 368 ms, p = 0.002), respectively. By contrast, mean fixation duration on the branding area showed higher with the warning at the bottom compared to top or middle positions by 25% and 36%, with mean differences of 156 ms (p = 0.100) and 212 ms (p = 0.026), respectively. No significant difference in fixation time was observed between the top and middle positions (p > 0.05).

Six months after GHWLs were implemented, 66.6% of boys and 72.4% of girls had been exposed to the warnings in the previous 30 days. Using complex samples logistic regression, I found that younger adolescents were significantly more likely to decide not to start smoking and to think smoking is dangerous to their health after seeing GHWLs. In addition, the positive effect of warnings was greater for adolescents who had experienced smoking prevention education or who had been exposed to anti-tobacco advertisements. However, those who smoked, used e-cigarettes, or experienced second-hand smoking were significantly less likely to decide not to smoke or see smoking as dangerous.



Conclusions

The implementation of the GHWL is a meaningful component of tobacco control policy by itself. Despite the delays and weakening strategies of the tobacco industry, it was possible to place pictorial warning at the top of the packaging, but it seem to the effect of the policy was not sufficient. These results suggest the need for more enhanced packaging policy of the tobacco products. Therefore, I suggest to consider as soon as possible the implementation of the plain packaging on tobacco products.

The plain, standardized packaging of tobacco products is an important component of the tobacco endgame strategy when expanded on by GHWLs. Plain packaging is effective in inducing smoking cessation among smokers and preventing smoking in non-smokers, particularly youth. Furthermore, plain packages reduce the appeal of tobacco products and tobacco use and limit false perceptions about tobacco. These package alterations have the desired effect on attitudes, beliefs, behaviors, and intentions about smoking, and could play a role in reducing the acceptance of smoking throughout society.

To achieve this policy, it would be necessary to prohibit the tobacco industry from directly or indirectly contacting members of committees involved in the legislative process of regulating tobacco control. Research to establish the scientific basis of such legislation should be carried out continuously, and public-friendly policies are required that can attract media attention and support. Moreover, to maintain the negative perception of smoking from childhood through adolescence, nation should implement comprehensive tobacco control including smoking prevention education in schools and encourage a smoke-free environment at home. In the end, we should strive for a tobacco endgame, a strategy to de-normalize of tobacco use and permanently remove tobacco products from society from certain time that will go beyond tobacco control.
