dc.description.abstract연구 배경: Vitamin D 결핍증과 활동성 결핵의 연관성에 대한 외국의 보고는 있지만, 아직 논란의 여지가 있는 상태로 본 연구의 목적은 한국인 결핵 환자에서 vitamin D 결핍증의 빈도를 대조군과 비교하고, 항결핵 치료에 따른 vitamin D 농도의 변화를 알아보고자 하였다.
방법: 2010년 4월부터 2011년 3월까지 진단된 결핵 환자를 전향적으로 모집하여 치료 시작 전 혈청 25-hydroxyvitamin D (25-OHD) 농도를 측정하여 이들과 나이, 성별이 비슷한 대조군 과 차이가 있는 지 확인하였다. 환자군은 치료 시작 한달 후 및 치료 종결 시에 25-OHD 농도 변화를 측정하였다. Vitamin D 결핍증은 혈청 농도가 10 ng/mL 이하로 정의하였다.
결과: 총 116명의 결핵 환자와 86명의 대조군이 연구에 포함되었다. 결핵 진단 시 환자군의 25-OHD 값은 대조군과 차이가 없었으며 (13.9 ng/mL vs. 13.2 ng/mL; p = 0.97), vitamin D 결핍증의 빈도 또한 차이가 없었다 (36.2% vs. 27.3%; p = 0.21). 환자군에서 항결핵 치료를 시행하였을 때 혈청 25-OHD 값은 치료 한달 후 12.5 ng/mL, 치료 종료 시 11.0 ng/mL로 유의하게 감소하였다 (p = 0.01).
결론: 한국인에서 vitamin D 농도는 결핵의 발생과 관련이 있지 않았으며 항결핵 치료에 따라 25-OHD 농도는 감소하였다.

dc.description.abstractIntroductions: Vitamin D deficiency has been reported to be associated with the development of active tuberculosis (TB), but many discrepancies exist among studies. The aims of this study were to compare the frequency of vitamin D deficiency between TB patients and controls and to monitor the changes of vitamin D levels during treatment of TB in a Korean population.
Methods: We prospectively enrolled patients with newly diagnosed TB. In addition, we enrolled healthy volunteers or patients with diseases other than TB as controls. Baseline serum 25-hydroxyvitamin D (25-OHD) levels were measured in both groups and compared. In TB patients, the measurement of serum 25-OHD was repeated 1 month after treatment initiation and after completion of treatment.
Results: In total, 116 patients with TB and 86 controls were recruited. The median 25-OHD level was not different in TB patients at diagnosis (13.9 ng/mL; IQR 8.80-21.8) and in controls (13.2 ng/mL; IQR 9.6-19.3) (p = 0.97). The frequency of vitamin D deficiency (≤10 ng/mL) was not different also in TB patients (36.2%) and controls (27.3%) (p = 0.21). During treatment in TB patients, the median 25-OHD level decreased significantly to 12.5 ng/mL at 1 month and 11.0 ng/mL at treatment completion (p = 0.01).
Conclusions: In Koreans, the vitamin D level does not appear to be associated with the development of TB, and median level of 25-OHD decreased after treatment for TB.
