Is the growth for Class III mandibular prognathic patients different from those for the general population?

Abstract

Introduction: To determine if any significant differences existed in the stature growth pattern of Class III subjects with non Class III and general population. Methods: Twelve-year longitudinal stature growth data were collected for 402 randomly selected adolescents in the general population, 55 Class III mandibular prognathic patients, and 37 non-Class III (Class I and Class II) patients. Growth data were analyzed using the traditional linear interpolation method and non-linear growth functions. The six stature growth parameters were measured: age at take off (TO), stature at TO, velocity at TO, age at peak height velocity (PHV), stature at PHV, and velocity at PHV. Comparisons in the stature growth parameters and 15 cephalometric variables among the general population, Class III subjects, and non-Class III subjects were performed with a multivariate analysis. Results: Patients with Class III prognathism did not have different growth parameters compared to Class II subjects or the general population. Conclusions: This study does not allow meaningful conclusions with regard to the relationship of mandibular size and stature growth pattern. The application of nonlinear growth curves versus the traditional linear interpolation method was also discussed.

본 연구의 목적은 누적 신장 자료를 이용하여 III급 부정교합자와 일반인 집단 사이의 신장 성장 양상에 차이가 존재하는지 비교해 보고자 하는 것이다. 서울대학교 치과병원 교정과에 내원한 환자를 대상으로 적합한 누적 신장 자료가 완비된 92명의 환자군(남자 48명, 여자 44명)에서 III급 부정교합자군(남자 36명, 여자 19명)을 선정하여 일반인 집단과 비교하였다. 일반인 집단은 2004년과 2010년에 시행된 한국인 인체 치수 조사사업에서 보고한 횡단적 신장 자료와 한국인 종단 자료(남자 236 명, 여자 166 명)를 이용하였다. 수집한 신장 자료는 자료 정리, 성장 모형 적합, 적합된 모형의 미분 과정을 거쳐 성장 변수를 추출하였고, 환자군의 측모두부계측 방사선사진에서 하악골과 관련된 계측치를 구하였다. 본 연구에 사용한 성장 모형은 (1) 선형 보간법 (linear interpolation), 비선형 성장모형인 (2) Preece and Bainess model 1, (3) Jolicoeur-Pontier-Pernin-Semp model이다. 각 성장 모형에서 도출된 성장 변수들과 측모두부계측방사선사진 계측치는 다변량 분산분석 (MANOVA), 반복측정 분산분석(repeated measures ANOVA), 일원 분산 분석 및 다중비교를 시행하여 분석하였다. 그 결과 III급 부정교합자와 I, II급 및 일반인 집단간의 성장 특성의 차이는 발견할 수 없었다. 선형 보간법의 경우 실험군 간 성장 특성에 차이가 존재한다는 결과를 얻었지만, 이는 실제적인 차이가 존재한다기 보다는 성장 모형의 한계에 의한 오류인 것으로 보인다.
