지역별 응급의료 접근성이 환자의 예후 및 응급의료비 지출에 미치는 영향

Abstract

응급의료 취약지를 개선하기 위해 2012년 이후, 권역응급의료센터 선진화 추진계획을 시행하였고, 2013~2017년의 4년 동안 응급의료체계의 개편을 통해 예방 가능한 사망률을 개선하였으나, 아직도 응급의료 취약지는 다수 존재하고 있으며, 대응과 프로토콜의 차원에서 응급의료체계가 미흡하다는 지적이 나오고 있다. 본 연구의 목적은 지역별 특성이 응급의료의 물리적 접근성, 즉 소요시간에 미치는 영향과 응급의료 접근성이 환자의 예후와 응급의료비 지출에 미치는 영향에 대해 규명하는 것이다. 본 연구는 한국보건사회연구원과 국민건강보험공단이 공동으로 주관하는 한국 의료패널 연간 데이터 13차 beta version 1.6과 국가통계포털(KOSIS)의 지역별 의료이용통계, 시군구별 의료시설 및 의료인력 현황, 도시계획 현황 통계 자료를 활용하였다. 연구대상으로 2013~2017년 동안 응급실에 내원한 환자 8,119명을 대상으로 다수준 분석과 다중 회귀분석, 다항 로지스틱 회귀분석을 시행하였다. 개인 변수와 지역 변수를 모두 고려한 다수준 분석의 결과, 소요시간에 통계적으로 유의한 영향을 미친 지역 변수는 면적 100km2당 응급의료기관 수로, 면적 100km2당 응급의료기관 수가 증가할수록 응급실까지 소요시간은 감소하는 것으로 나타났다. 다중 회귀분석과 다항 로지스틱 회귀분석 결과, 환자의 예후와 응급의료비 모두 소요시간에 유의하게 영향을 받는 것으로 나타났다. 따라서 본 연구를 통해 응급의료의 접근성은 지역별 특성에 영향을 받으며, 접근성이 응급의료비 지출 및 환자의 예후에 영향을 미친다는 것을 규명하였다. 본 연구의 결과는 현 응급의료 시스템의 기초자료로 활용될 수 있을 것으로 기대된다.

In order to eliminate the regions that lack the emergency medical services, the Regional Emergency Medical Center Advancement Promotion Plan has been implemented since 2012 and the emergency medical system was reorganized for four years from 2013-2017. But there still existed many regions that lack the emergency medical services and it was pointed out that the emergency medical care system was insufficient in terms of response and protocol. The purpose of this study was to examine the impact of the regional characteristics on the accessibility of emergency care and the impact of emergency medical accessibility on the patients' prognosis and the emergency medical expenditure. This study used the 13th beta version 1.6 annual data of Korea Health Panel which is jointly organized by the Korea Institute for Health and Social Affairs and the Korea National Health Insurance Corporation Service. For regional characteristics, the statistics on medical use by region, the status of medical facilities and medical personnel by city and district, and the urban planning statistics from Korean Statistical Information Service(KOSIS) were used in this study. The sample included 8,119 patients who visited the emergency centers between year 2013 and 2017 and multi-level analysis, multiple regression, and multinomial logistic regression were performed. The results for the multi-level analysis in both the individual and regional variables showed that the number of emergency medical centers per 100 km2 area had a statistically significant effect on the arrival time. As the number of emergency medical institutions per 100 km2 area increased, the time required to reach emergency centers decreased. Multiple regression and multinomial logistic regression results showed that both the patient's prognosis and the emergency medical expenses were significantly affected by the arrival time. In conclusion, this study suggested that the access to emergency care was affected by regional characteristics and the access to emergency care affected emergency medical expenditure and patient outcomes. The results of this study are expected to be used as valuable evidence to improve the current emergency medical system.
