근거이론적 분석을 통한 장애인구강진료체계 개선

Abstract

1. 목 적 장애인은 치주 질환이나 치아우식 등의 구강질환 발병률이 높으며, 치료를 받더 라도 이를 잘 유지해 나가기가 어렵다. 장애인 구강진료체계의 경우 진료 중심의 치료보다 예방치료 및 정기적인 검진이 매우 중요하다. 그러나 현재 장애인에 대한 구강진료 접근성은 매우 떨어지는 편이며 장애인은 시의적절한 진료를 받지 못하 고 있다. 정부는 장애인에 대한 구강보건의료서비스 공급을 확대하기 위하여 다양 한 정책을 추진하고 있다. 정책의 일환으로 장애인구강진료센터를 설치하였으나 권역별로만 설치되어 있어 접근성이 매우 낮으며 대기 시간도 매우 길다는 문제점 이 지적되고 있다. 이 연구의 목적은 장애인의 구강진료 접근성 제약에 관한 요소 를 평가하기 위해 의료서비스 수요자인 장애인과 공급자인 치과의사의 관점에서 진 료의 경험을 분석하는 것이다.

2. 방 법 본 연구는 질적 연구 방법의 하나인 Strauss & Corbin(1996)의 근거 이론 (Grounded Theory)를 사용하여 진행하였다. 연구 참여자는 2021년 3월부터 2022년 3월까지 종로 푸르메치과를 이용하는 장애인과 장애인구강진료센터에서 근무하는 의료진을 대상으로 개별 심층 면접을 진행하였다. 선행연구를 바탕으로 질문지를 작성하여 심층 면접을 진행하였고, 녹취 후 전사하여 자료를 정리하였다. 수집된 자 료는 근거 이론에 따라 코딩하고, 패러다임 모형에 따라 핵심 범주를 도출하였다.

3. 결 과 수집된 자료를 범주화하는 개방 코딩 결과-조건, 현상, 대응전략, 결과로 도출되 었다. 장애인은 스스로 구강관리에 어려움을 겪고 있었고, 구강관리에 대한 지식도 부족하였다. 장애에 대한 재활에 주력하다 보니 구강건강관리는 우선순위에서 밀리는 현상이 발생하고 있었다. 조건은 치료받고자 마음먹어도 장애인이 내원할 수 있는 의료기관이 부족하였고 높은 비급여 진료비로 인하여 선뜻 치료받기가 어렵다고 하였다. 연구에 참여한 대다수의 장애인은 이러한 상황에 적극적으로 대처하며 도움을 요청하거나 치료 후 자신감을 되찾는 등 긍정적 경험을 하고 있었다. 일부 장애인은 현 상황에 대한 자구책을 마련하며 체념하고 있었다. 결과적으로 정책적 개선을 요구하였다. 의료진은 장애인 환자가 이미 악화된 치아 상태로 내원함으로 인하여 높은 진료의 난이도를 경험하고 있었다. 조건은 힘든 진료를 수행하고 있지만 낮은 수가와 비현실적인 가산제도로 인하여 수익구조에 어려움을 경험하고 있었다. 한편 의료진은 대응전략으로 이러한 상황 에 대하여 체념하고 적응하거나, 더욱더 적극적으로 자구책을 마련하고 있었다. 그 결과, 정책적 지원을 요청하거나, 수립되는 정책들 속에서 괴리감을 느끼고 있었다. 현재 정부에서 추진 중인 정책은 구강진료센터 설립에 집중되어 있다. 장애인의 구 강진료 접근성 개선을 위해서는 장애인 인구밀도가 높은 지역에 지역장애인구강보 건센터의 설립이 우선적으로 이루어져야 한다. 또한 지역-권역-중앙 장애인구강진료센터의 각 특성에 따른 유기적인 진료 체계의 구축이 필요하다. 마지막으로 치과적 중증 장애인의 비급여 진료비에 대한 선별적인 치료비 지원 정책이 필요하다. 본 연구를 통하여 장애인의 구강진료접근성 개선을 위한 정책 개발의 근거를 마련하고자 한다.

The objective of this study was to explore factors related to obstacles on access to dental care for people with disabilities. The experience of dental treatment was analyzed from perspectives of people with disabilities and the dentists. In-depth interviews were conducted with people with disabilities and the dentists from Regional Dental Center for People With Disabilities. Based on previous research, questions were prepared and interviews were conducted, and the data were recorded and transcribed. The collected data was coded according to grounded theory(Strauss & Corbin, 1996) among the analysis methods of qualitative research, and core categories were derived according to the paradigm model. Conditions, phenomena, response strategies, and results were derived from paradigm model. People with disabilities were having difficulty in oral care on their own. There were not enough oral care centers for people with disabilities to visit and uninsured medical expenses were too expensive. The majority of disabled patients who participated in the study actively asked for help and had a positive treatment experience. The dentists who participated in the study were experiencing difficulty in treatment as the patients visiting the clinic with already severely deteriorated dental conditions. However, they were also having difficulties in financial management of the clinic from long treatment time with low cost. As a result, it is necessary to establish a local dental health center for people with disabilities that can be easily accessed by people with disabilities. It is necessary to establish an associated treatment and referral system of the Local-Regional-Central Dental Care Center for People With Disabilities. Preventive and regular oral examinations are important and encouraged. There is a need for a selective treatment cost support policy for uninsured medical expenses for oral rehabilitation of people with disabilities.
