소아청소년의 예방접종 시행에 대한 일반적인 권장사항

Abstract

Immunization is the most cost-effective method of preventing many infectious diseases. Recommendations for immunization practices are based on scientific knowledge of vaccine characteristics, the principles of immunization, the epidemiology of specific diseases, host characteristics, and expert opinion of public health officials and specialists in clinical and preventive medicine on vaccine benefit and safety issues. Therefore, recommendations for vaccination practices balance scientific evidence of benefits and risks of immunization programs. Many of the standards for child and adolescent vaccination programs that have been previously published are universal and are applicable to many countries where different public health concerns may exist. However, several updated changes are added to these standards to maximize the benefits from the vaccination services. This report will provide the most recent technical guidance about common vaccination concerns for clinicians who administer vaccines to infants, children, and adolescents. 예방접종은 감염질환의 예방에 비용 대 편익면에서 가 장 경제적인 수단 중 하나이다. 특히, 감염질환이 흔 히 발병하는 영아기, 소아기 및 청소년기에는 백신으로 예방가능한 질환을 예방하여 개인과 사회의 보건 향상에 기여하는 이점을 최대화하고, 개인과 사회에 안전한 예방접종을 시행하기 위해서는 예방접종의 일반적인 권장사항을 준수하는 것이 매우 중요하다. 예방접종 정책은 백신의 특성, 예방접종의 기본원칙, 질환의 역학 및 숙주의 특성에 대한 과학적인 지식과 보건정책 전문가, 임상 혹은 예방의학 전문가의 견해를 바탕으로 하여 수립된다(1). 이러한 결정은 한 지역사회에서 시행되는 예방접종이 개인과 사회에 최대한의 편익을 제공하는 반면, 접종으로 인한 위험은 최소화하는 것을 지향한다. 예방접종을 시행하는 임상가들은 백신의 효과와 안전성, 역학, 비용효과 분석 등의 과학적 증거를 바탕으로 하여 만들어진 예방접종 시행의 일반적인 권장사항을 숙지하고 이를 잘 준수하는 것이 매우 중요하다. 예방접종 시행에 대한 기본적인 권장사항은 이미 여러나라에 보편적으로 활용되고 있으며, 그 기본적인 틀에는 큰 변화가 없다(2, 3). 하지만 새로운 백신 및 백신제제의 개발과 역학의 변화 등의 이유로 예방접종의 권장사항에는 꾸준히 새로운 내용이 추가되거나 개정사항이 반영되고 있다. 따라서 본 특집에서는 백신을 접종하는 임상가들이 백신접종 시행의 기본 원칙을 준수하여 안전하고 효과적인 예방접종을 하는데 도움을 주고자, 외국과 우리나라에서 가장 최근에 개정된 예방접종의 일반적인 지침을 소개하였다(4~6).
