dc.description.abstract | 연구목적: 본 연구는 골유도재생술과 함께 식립된 임플란트와 골재건술 없이 식립된 임플란트의 장기적 누적 생존율을 비교하고, 부가적으로 임플란트의 누적 생존율에 미치는 영향을 분석하고자 한다.
재료 및 방법: 2001년부터 2010년까지 서울대학교 치과병원 치주과에 내원하여 한 명의 술자(Y.K)로부터 임플란트 시술을 받은 178명의 환자, 498개의 임플란트를 대상으로 전산화된 의무기록을 이용한 후향적 연구를 시행하였다. 누적 생존율을 Kaplan-Meier법에 의해 산출하였으며, 누적 생존율에 대한 인자간의 차이는 로그순위 검정법을 통해 통계적 검정을 시행하였다. 이 중 누적 생존율에 유의하게 영향을 미치는 것으로 판단되는 인자를 선별하여 Cox 비례위험모형을 사용하여 다변수 분석을 시행하였다. 결과: 1. 178명의 환자에게 식립된 498개의 임플란트 중 19명 환자의 28개의 임플란트가 실패하여 470개가 생존하였고, 전체 임플란트의 5년, 10년 누적 생존율은 각각 97%, 91%였다. 2. 골유도재생술과 함께 식립된 임플란트의 5년, 10년 누적 생존율은 97%, 89%였고, 골재건술 없이 자연골에 식립된 임플란트의 5년, 10년 누적 생존율은 99%, 93%로, 두 경우 유의한 차이가 없었다(P>0.05). 3. 성별, 고혈압 병력 유무, 흡연 여부, 식립 위치가 임플란트의 누적 생존율에 영향을 미치는 위험요인으로 나타났다(P<0.05). 4. 임플란트 치료에서 흡연자가 비흡연자에 비해 6.41배 높은 위험도를 나타내었고, 상악에 식립한 임플란트가 하악에 비해 3.23배 높은 위험도를 나타내었다. 결론: 골유도재생술을 동반한 임플란트 치료는 안전하고 예지력 있는 치료방법이라고 할 수 있다. | - |
dc.description.abstract | Objectives: The aim of the present study was to compare the long-term cumulative survival rates of dental implants placed with and without guided bone regeneration (GBR) and analyze various factors associated with survival of implants.
Materials and methods: This retrospective study was performed on 498 implants placed in 178 patients by one dentist (Y.K) at the Department of Periodontology, Seoul National University Dental Hospital in the years 2001-2010. The cumulative survival rates were calculated by the Kaplan-Meier method. The correlations between various factors and implant survival were performed using the log-rank test and Cox proportional hazards model. Results: 1. In total, among the 498 implants in 178 patients, 28 implants in 19 patients were lost and the cumulative survival rate up to 5-years, 10-years was respectively 97%, 91%. 2. The cumulative survival rate up to 5-years, 10-years of implants placed with GBR was respectively 97%, 89% and that of implants placed without GBR was respectively 99%, 93%. In both cases, there was no significant difference(P>0.05). 3. Gender, history of hypertension, smoking status and placed location were significantly associated with the implant cumulative survival rate(P<0.05). 4. In the implant treatment, smokers showed higher risk than non-smokers by 6.41 times and implants placed in the maxilla showed higher risk than implants placed in the mandible by 3.23 times. Conclusion: The implants placed with guided bone regeneration is considered to be a safe and predictable therapy. | - |
