소아병동 간호사가 보호자와의 관계에서 겪는 갈등 경험

Abstract

Pediatric nursing has characteristic caring children including parents. Parents are very important roles for giving nursing to children, pediatric nurse formed relationship with parents. It induces conflict situation between pediatric nurse and parents. However, the majority of studies have focused on the interpersonal conflict of nurse and it is poorly understood a specific aspect of conflict between pediatric nurse and parents. This study was conducted to understand the interaction of pediatric nurse with parents mothers during nursing and evaluate the effects of these experiences to pediatric nurse on the study topics of conflict experience between pediatric nurse and parents This study was conducted after being approved by the medical research ethics review committee of Seoul National University, where authors were affiliated. Research participants were selected at the S University Children Hospital in Seoul. Data was collected until the data was saturated. This study targeted ten pediatric nurses worked more than 1 years. Data were collected from June 2015 to March 2016. Indepth interviews were conducted at a cafe or rest area of nurse dormitory. Collected data was analyzed and interpreted by using the qualitative analysis method proposed by Braun & Clarke (2006). The results of this study drew four themes and 13 sub-themes from total 52 code statements. The first theme was burden working environment. It showed background of confilct between pediatric nurse and parents. The second theme was confusing. It showed pediatric nurses personal internal conflict. The third theme was not become a main of nursing. It showed at the time of conflict. It expressed as three sub-themes, which were parents sensitive attitude, invisible surveillance, shaking the professional area of nursing. The last theme was unresolved conflict. It showed emotions of pediatric nurse, solution process of conflict, and efforts to resolve conflicts. It expressed as four sub-themes, which were become a sinner, not completely resolved conflict, wearing mask and keep a distance,efforts for conflict resolution. The results of this study helped us to understand the background and solution process of conflict between pediatric nurse and parents. These understandings will be the basis to develop the necessary nursing intervention for pediatric nurse.

소아를 대상으로 하는 간호는 환아 뿐만 아니라 환아 보호자 또한 돌봄의 대상이라는 특성을 보인다. 환아에게 간호를 제공하는 데 있어 보호자는 중요한 역할을 하기 때문에 간호사는 보호자와의 관계를 형성하게 되고, 이로 인해 갈등 상황이 야기된다. 하지만 소아병동 간호사와 보호자 간 갈등에 대한 연구는 대인 관계 갈등의 일부로 거론되는 것이 대부분이며, 이들이 겪는 갈등의 구제척인 양상에 대해서는 연구가 미비하다. 본 연구는 '소아병동 간호사가 보호자와의 관계에서 겪는 갈등 경험'이라는 연구 주제를 통해 소아병동 간호사가 환아를 간호하는 과정에서 보호자와 어떤 상호작용을 하며, 이러한 경험들이 간호사에게 어떤 영향을 끼치는지 심층적으로 이해하고자 시행되었다. 본 연구는 연구자가 소속된 서울대학교 생명윤리심리위원회의 심의를 받은 후 2015년 6월부터 2016년 3월에 걸쳐 서울특별시 S대학 어린이병원 1곳을 통해 연구 참여자를 선정하였다. 자료가 포화될 때까지 자료 수집을 시행하였고 1년 이상 근무한 간호사 중 10명의 소아병동 간호사를 대상으로 하였다. 주로 참여자가 선택한 카페나 기숙사 내 휴게실에서 심층 면담이 이루어졌으며, Braun & Clarke(2006)의 질적 주제분석 방법을 이용하여 수집된 자료를 분석 및 해석하였다. 본 연구의 결과 총 52개의 코드 진술로부터 4가지 주제(themes)와 13개의 하위주제(sub-themes)가 도출되었다. 제 1 주제인 버거운 근무환경은 소아병동 간호사가 보호자와의 관계에서 겪는 갈등이 발생하게 되는 배경에 대한 내용이다. 제 2 주제인 혼란스러움은 소아병동 간호사가 보호자와의 관계에서 경험하는 개인적인 내적 갈등에 대한 내용이다. 제 3 주제인 간호의 주체가 되지 못함은 하위주제인 소아병동 간호사가 실무에서 실제로 발생하는 갈등상황 당시에 대한 내용으로 날선 보호자들의 태도, 보이지 않는 곳에서의 감시의 눈, 의료인의 전문적인 영역을 흔드는 보호자로 확인되었다. 제 4 주제인 해결되지 않은 갈등은 보호자와의 갈등의 경험했을 때 참여자들이 느꼈던 감정과 갈등의 해결과정, 그리고 갈등을 해결하기 위해 노력하는 간호사에 대한 내용이다. 이는 죄인이 됨, 갈등이 시원하게 해결되지 않음, 가면을 쓰고 거리를 둠, 갈등 해결을 위해 노력함의 하위주제로 구성되었다. 이를 통해 소아병동 간호사가 보호자와의 관계에서 겪는 갈등이 발생하는 배경과 갈등의 해결 과정에 대해 확인할 수 있었고, 이러한 이해는 나아가 소아병동 간호사를 위해 필요한 간호중재를 개발하는 기초가 될 수 있을 것이다.
