물리치료의 전문화

Abstract

의료의 전문성이 높아지고 의료시장 내 경쟁 구도가 가속화되면서 의료서비스와 서비스를 제공하는 인력의 전문화에 대한 사회적 관심과 요구가 증가하고 있다. 의료서비스의 질적 수준은 서비스 제공 인력의 현실을 반영하는 한편, 서비스를 제공받는 환자들의 건강 결과에도 직접적인 영향을 미친다. 특히 물리치료는 환자의 신체적, 정서적 회복에 많은 영향을 미치기 때문에 서비스의 양적, 질적 관리가 중요하다. 더욱이 물리치료에 대한 수요와 물리치료사의 역할이 점점 더 늘어날 것으로 전망되고 있어 물리치료 서비스와 서비스를 제공하는 인력의 전문화에 대한 보건학적 관심이 더욱 요구되는 상황이다. 우리나라 물리치료는 세계적 흐름과 달리 여러 가지 제도적 제약 속에서 전문화를 요구받고 있다. 또한 물리치료계 내부의 결집력이나 구성원 전반의 전문가 의식도 높지 않아 자체적으로 전문화를 이끌어가는데 한계가 있는 것으로 보인다. 그런데 최근 물리치료의 전문화가 사회 변화와 의료계 내부 요인에 의해 추동될 가능성을 시사하는 움직임들이 나타나고 있다. 이에 본 연구는 물리치료 업무현장의 흐름과 인식, 경험 속에서 물리치료 전문화의 실재를 확인하고 가능성과 한계를 논의하고자 하였다. 연구를 위해 전문직 체계의 역동성을 고려한 전문직 체계론을 바탕으로 세 가지 연구주제를 설정하였다. 첫째, 물리치료 시장의 흐름은 어떤 양상을 보이며 기여 요인은 무엇인가? 둘째, 업무현장에서 인식하는 물리치료 처방 증가의 맥락과 영향은 무엇인가? 셋째, 물리치료 전문화를 추동하는 사회요인은 무엇인가? 다음으로 자료를 구성하고 분석하기 위해 세 가지 연구방법을 사용하였다. 첫째, 연구1에서 물리치료 시장의 흐름을 살펴보기 위해 건강보험 청구자료를 이용하여 2006년부터 2015년까지의 물리치료 처방의 증가 추이를 개괄하고, LMDI(Log Mean Divisia Index) 요인분해법을 적용하여 물리치료 시장변동의 기여 요인을 분석하였다. 둘째, 물리치료 전문성과 관련된 업무현장의 인식과 경험을 탐구하기 위해 의사와 물리치료사 총 18인을 대상으로 개별심층면담을 진행하고 질적 주제 분석을 실시하였다. 셋째, 전문물리치료를 경험한 환자와 보호자 총 20인에 대해 4개 그룹을 구성하여 초점집단면접을 진행하고 질적 주제 분석을 실시하였다. 공급자와 이용자의 인식과 경험에 대한 자료는 연구2와 3의 분석에 사용하였다.

주요 연구 결과는 다음과 같다. 첫째, 지난 10년간 물리치료 처방에서 전문물리치료 영역의 처방이 일반물리치료 영역의 처방보다 더 빠르게 증가하고 있었고, 그중에서도 물리치료사에게 특정 자격기준을 요하는 중추신경계발달재활치료의 증가율이 차지하는 바가 컸다. 이용자 특성별로는 10세 미만과 50대 이상의 환자가 전문물리치료 영역의 비용의 증가에 상대적으로 높게 기여했으며, 공급자 특성별로는 병원급 의료기관, 요양병원 입원이 전문물리치료 영역의 비용 증가에 상당 부분을 차지하고 있었다. 이러한 변화는 고령화와 같은 인구구조의 변화보다는, 질병으로 인해 환자가 증가하거나 의료기관 내에서 환자 1인에게 실시하는 치료의 양이 늘어난 것이 상대적으로 많이 기여한 것으로 확인되었다. 둘째, 전문물리치료 처방 증가의 맥락과 영향에 대해 질적 주제 분석을 실시한 결과 서비스 제공기관의 양적 증가, 물리치료 전문성에 대한 사회적 관심 증가, 물리치료사의 전문적 역량 강화라는 3개의 주제와 12개의 범주가 도출되었다. 의료현장의 경험과 인식에 따르면 전문물리치료 처방의 증가 현상은 요양병원과 같은 서비스 제공기관의 증가에 영향을 받고 있었다. 전문물리치료 중심의 서비스 공급 증가로 많은 환자들이 물리치료를 이용하게 되면서 재활 치료에 대한 인식이 높아졌고 의료기관들도 서비스 질 제고를 위해 노력하고 있음을 확인할 수 있었다. 업무현장에서 의사와 환자는 물리치료사에게 지식, 경험, 소통, 문제해결 능력을 아우르는 독립적인 전문가의 역할을 기대하고 있었으며, 물리치료사 역시 이러한 인식을 공유하고 있었다. 셋째, 질병의 증가나 의사의 처방과 같은 의료 영역 외에 물리치료 전문화에 영향을 미치는 사회요인의 변화에 대해 질적 주제 분석을 실시한 결과 기술의 발전, 환자의 역능화, 지역사회의 요구라는 3개의 주제와 9개의 범주가 도출되었다. 의학기술의 발달은 물리치료의 전문 영역을 확장시키고 있었으며, 과학기술의 발전은 물리치료사의 업무방식을 변화시키고 때로는 새로운 전문성을 요구하기도 했다. 또한 통신기술의 발달로 직역 간 상시 교류가 증가하면서 환자에 대한 세부적인 정보를 공유할 기회가 많아졌다. 한편, 질병에 대한 환자의 주체의식은 강화되었으나 재활에 대한 정보 부족, 치료 기간의 제한, 서비스 제공기관의 부족 등으로 환자들은 양질의 서비스를 충분히 제공받지 못하고 있었다. 물리치료에 대한 요구는 점차 지역사회로 뻗어 나가고 있었으며, 이로 인해 물리치료사에게도 새로운 전문화의 기회들이 제공되고 있는 것으로 확인되었다.

연구를 통해 우리나라 물리치료의 전문화 현상이 다양한 외부 요인들과 이에 맞서는 의료계의 대응 속에서 추동되고 있음을 확인하였다. 그리고 기존 전문화 사례들과 달리 직업집단 내부보다 외부의 동력이 강하게 작용하고 있음을 발견하였다. 본 연구는 전문화를 추동하는 여러 요인 중 몇 가지 측면만을 중점적으로 다루어 물리치료 전체에 대한 총체적 담론을 이끌어내는데 한계가 있다. 그럼에도 지금까지 관심이 저조했던 비의사 영역의 서비스 전문성에 대한 탐구를 시도하였고, 물리치료의 전문화를 이끄는 사회 변화 및 의료체계 내 역동성 속에서 중요한 경험적 정보를 제공하였다는데 의의가 있다.

As the level of expertise in medical field has been raised and competitive composition in medical market gains speed, concerns and demands for the medical staff with expertise in this filed are also soaring up. Not only does the level of medical service reflect the reality of the current medical staff members providing a service, but also has a direct effect on the health outcome for the patients. Especially, physical therapy has a lot to do with recovery for both physical and emotion parts of the patients, an organized service management on both quantitative and qualitative basis should comes first than others. Its expected that demand for the physical therapy would be bigger and the role of the physical therapist would be broaden, therefore, its being required to draw more attention to the current service reality and care about professionalization on the physical therapist from the perspective of public health. Professionalization in physical therapy at the moment in Korea is ahead of lots of hurdles related to policy constraints which seem out of line with a universal trend. There also seems to be limits to make a professionalization feasible due to little solidarity among the member of physical therapists and comparatively low level of professionalism on their own. However, some positive movements for the professionalization in physical therapy can be seen in recent years thanks to the internal factors in medical field. Therefore, this study focuses a discussion on the professionalization in physical therapy to figure out the current reality and possibilities on it based on the various voices from work places. To begin with, the following 3 questions have been set up in accordance with a professional job considering the dynamic feature of its own professional system. Firstly, where is the current physical therapy market going now and which factor is it contributing to? Secondly, whats causing the increase in demand for prescription of physical therapy from the view of the work place? Lastly, which social driving factor is moving the professionalization on it forward now? To complete the arranging and analyzing on the gathered data for each question, 3 methods of studies have been used in this article. First of all, the health insurance claim data (2006~2015) has been utilized to check the progress of the increase of the prescription for physical therapy for study 1 and figured out the cause thats contributed to the market change through the LMDI (Long Mean Divisia Index). Secondly, qualitative theme analysis has been implemented based on the in-depth interviews with total 18 medical specialists and physical therapist to hear out the exact perceptions and experiences from work places. Thirdly, focus group interviews with 20 patients and their caregivers with 4 divided groups have also implemented for the qualitative theme analysis. And the data on the perceptions and experiences from the service provider and users has been used for the analysis on study 2 and study 3.

The major research results are as follows. For the last decade, its noticed that the number of the prescription for the professional physical therapy has increased rapidly than the ordinary ones. Among them, the demands for neuro-developmental treatment have noticeably increased, for which, its inevitable for the physical therapist to acquire a certain qualification standard. Especially, those patients in their 10s through over 50s contribute to the cost increase in professional physical therapy comparatively. From the characteristics of the service provider, hospitals or inpatient in long-term care hospitals has taken up mostly for the increase. The increased patients or a relatively grown treatment amount per patient in the medical institution has been confirmed as key factor to the cost increase rather than because of the change in population structure due to the aging. Secondly, as a result of the qualitative theme analysis on the increase of the prescription for the professional physical therapy, total 3 themes like Increase of the institutions providing physical therapy in quantity, Increase of the social concern for the quality of physical therapy, Strengthen physical therapists professional ability and total 12 categories have been drawn. According to the perceptions and experiences from the relevant medical work places, its also concluded that the current progress of the increasing prescription for the professional physical therapy could attribute to rising number of institutions like long-term care hospital. It can be seen that medical providers also try to raise the level of medical service under the atmosphere where patients having more experiences with physical therapy started to seek even more professional service based on the different perceptions. Its also confirmed that medical specialist and patients have a high expectation for the physical therapists as professionals in their field and they have also found to share this perceptions. Thirdly, other than the increase in disease and prescription of medical doctors, social factors has also been found to drive the professionalization in physical therapy. As a analysis on the relevant data, total 3 themes like Technology development, Patient empowerment, Demand from community and total 9 categories have been drawn. Due to the advances in medical technology, the professional area for the physical therapy have been broaden and science technology improvement has had work method changed in the field and sometimes demand a higher professional job. Its also gotten feasible for a patient to communicate with medial specialist in a real time to share detail in remote area as well. Although patients became to have sovereignty by themselves when it comes to the disease, its hardly say that patients are being provided with sufficient service due to the lack of information for rehabilitation, limited period of treatment, shortage of institutions. Demands on physical therapy are now being spread over to the local community now and it can be noticed that opportunities of being professionalized are being given to them gradually.

According to this study, its also confirmed that a variety of external factors and the relevant respond from medical field has step forward to the professionalization in physical therapy and also found that external driving factors are affecting stronger rather than ones within physical therapists. This study could have some limitation to draw the entire discourse on the physical therapy since only a few part of the factors driving the professionalization in physical therapy were handled. Nevertheless, it could be meaningful from the perspective of attempting to look into the professional service of the paraprofessionals and of providing the important findings based on the experiences under the changing society and dynamic medical system.
