근거이론을 활용한 치과병원 원내생 진료센터 진료경험에 관한 탐색적 연구

Abstract

연구 목적 치과병원 원내생 진료센터는 학생들의 임상 역량을 기르고 졸업 후 치과의사로서 갖추어야할 자질을 함양하는 데 중요한 역할을 하는 공간이다. 하지만 환자의 진료 참여와 동의가 점차 줄어들고 있는 실정이며, 이에 따라 학생들이 직접 환자를 섭외하기에 나서는 것을 비롯하여 원내생 진료센터 임상 교육 운영과 관련하여 여러 문제가 발생하고 있다. 질적 연구를 통해 원내생 진료센터 경험 환자를 비롯한 여러 구성원들 간의 상호작용을 살펴보고 이를 다각적 그리고 다원적으로 분석하여 원내생 진료센터 임상 교육의 수월성 확보 방안을 모색해보고자 한다.

연구 방법 질적 연구 방법 중 하나인 근거이론 방법을 사용하여 연구를 진행하였다. 원내생 진료센터를 이용한 환자, 원내생 진료센터에서 근무하는 재학생과 외래 교수 및 직원을 포함하여 총 30명을 대상으로 심층 면접을 실시, 녹취 후 이를 전사하여 자료를 정리하였다. 원자료를 바탕으로 개방 코딩, 축 코딩, 선택 코딩을 실시하여 결과를 도출하였다.

연구 결과 개방 코딩을 통해 139개의 개념을 추출하였고 43개의 하위범주와 12개의 상위범주로 묶어낼 수 있었다. 축 코딩을 통해 패러다임 모형을 제시하였으며, 선택 코딩을 통해 핵심범주 환자 의식 변화와 구성원 제 역할 다하기를 통한 원내생 진료센터 활성화를 상정할 수 있었다. 가설적 정형화를 통해 유형 분석을 이끌어냈고, 치석 제거형, 레진형, 크라운형, 그리고 근관치료형 상호작용 유형으로 나눌 수 있었다. 이론적 모형을 기반으로 원내생 진료센터 활성화를 위해서는 원내생 자질 갖추기, 의료진 소임 다하기, 대학 병원 역할 기대 등 내부적인 노력으로 원내생 진료에 대한 환자의 의식 변화가 필요하다는 것을 도출하였다.

결론 치과병원 원내생 진료센터는 치과의사로서 갖추어야할 임상 능력을 기를 수 있는 좋은 교육의 장이다. 또한 진료라는 매개 사건을 둘러싸고 여러 구성원이 소통하는 공간이다. 본 연구를 통해 현재 원내생 진료센터 임상 교육과 관련한 여러 문제를 확인하고 원내생 진료센터 임상 교육 수월성 확보 방안을 위해 임상 및 전 임상 교육 과정 검토, 교육기관의 환자 수급 명문화, 의료진 처우개선, 그리고 진료 수가 현실화를 포함한 여러 정책적 지원방안이 함께 이루어져야 한다는 것을 제언하였다.

Research Purpose The purpose of this study is to investigate the dynamics and interactions within the predoctoral treatment center in a dental hospital, involving both patients and staff members. The research aims to explore diverse perspectives and conduct a comprehensive analysis to identify strategies for improving the effectiveness of clinical education in the pre-doctoral treatment center. The ultimate goal is to enhance the development of clinical competencies in dental students and cultivate the necessary qualities for their future careers as dentists.

Methodology This research utilized a qualitative approach known as grounded theory. In-depth interviews were conducted with a total of 30 participants, including patients who utilized the predoctoral treatment center, as well as students and visiting professors working within the center. The interviews were recorded and transcribed for data analysis. The collected data underwent open coding, axial coding, and selective coding to derive meaningful results.

Results Through open coding, a total of 139 concepts were identified, which were then organized into 43 subcategories and 12 overarching categories. Axial coding was employed to propose a paradigm model, while selective coding led to the identification of the core category, activating the predoctoral treatment center through patient awareness and fulfillment of members' roles. Hypothetical formalization facilitated typological analysis, resulting in the identification of various interaction types, such as scaling, resin restoration, crown restoration, and root canal therapy. Based on the developed theoretical model, it was inferred that bringing about a change in patient awareness regarding predoctoral treatment center requires internal efforts, including the development of students' qualities, fulfillment of medical professionals' responsibilities, and recognition of the role played by university hospitals.

Conclusion The predoctoral treatment center in dental hospitals plays a crucial role as an excellent educational environment for fostering the clinical skills of future dentists. It serves as a space where multiple stakeholders engage in communication centered around medical treatments. However, based on the findings obtained from exploring the experiences within the center, it is recommended that efforts to ensure effective clinical education should be accompanied by the evaluation of clinical and preclinical educational courses, establishment of a patient supply system for educational institutions, improvement of medical professionals' welfare, and implementation of policy support measures, including enhancements in treatment fees.
