Abstract
서론: 발목 관절 주위의 골절 손상에서 비골 외과는 가장 흔하게 골절되는 부위이다. 발목에 대한 외회전 염전력에 의해 주로 발생되는 이 손상은 비골 외과에 길고 비스듬한 골절선을 만든다. 현재까지 많은 수술 방법들이 소개되어 있는데, 가장 표준적인 방법으로 인정받는 것은 골절 골편간 지연 나사 고정 후 외측 금속판 고정법이다. 하지만 이 방법은 수술 절개선이 길고 나사 삽입을 위해 많은 골천공이 필요하며, 외측에서 내측으로 향하는 나사들을 삽입하는 과정에서 발목 관절 연골을 손상시킬 위험성이 있다는 단점들이 보고되고 있다. 그러나 최근 들어, 이런 장사형(長斜形)의 비골 외과 골절에 하나 또는 두 개의 3.5mm 두께의 피질골 나사를 골편간 결합 지연 나사 고정법에 대한 좋은 임상 결과가 보고 되고 있다.
방법: 이를 변형시킨 방법인, 2.7mm 소형나사를 3 개 이상 사용하여 양피질 골절편간 나사 고정법(bicortical interfragmentary screw technique)으로 고정한 임상 치료 결과에서 좋은 결과를 얻었는데, 본 연구는 새로운 골절 고정 방법과 기존의 3.5mm 지연 나사 고정법 사이의 생역학적인 차이를 확인하고자 한다. 실험 전 기존 논문들의 자료를 확인하여 통계적 유의성을 확보하는데 필요한 시료의 수를 검정력 분석을 통해 먼저 구하였다. 그리고 그 수에 맞게 인간 사체의 비골을 채취하여, 준비 기간 동안 재료를 영하 20도 정도에서 보관하도록 하였다. 장사형 골절 (long oblique fracture)을 그대로 재현하도록 하기 위해 표준 전후방 절골을 미세 진동 전기 톱을 이용하여 절골하였다. 종골의 골 밀도를 Dual Energy X-ray Absorptiometry를 이용하여 측정하였다. 비골 조직을 무작위로 2개의 조작 군에 배정하였다. 각 군은 3 개의 2.7 mm bicortical 나사 고정군과, 2개의 3.5 mm lag screws 고정군으로 나눈다. 모든 절골부를 정복하고 bone clamp로 일시적 압박을 유지한 후, 각각의 해당 군의 고정 방법대로 고정한다. bicortical screw fixation은 정복된 절골부를 2.0mm drill bit로 골 천공 후 2.7mm 소형 나사만을 삽입 고정하는 방법이고 지연 나사 고정은 양 피질골을 2.0mm drill bit으로 천공 후 근위 피질골을 2.5 mm drill bit으로 천공하여 고정한다. 표본들을 검사 기기에 단단하게 위치시키고, 외팔보 휨 부하와 염전 부하를 가하여 construct stiffness, average load-to-failure, mode of failure를 분석하고 고정법 사이의 결과 확인을 통해 새로운 고정 방법의 유용성에 대해, 비열등 검증(non-inferiority test) 한다. 모든 시료들은 골밀도 검사에서 골다공증으로 판정되었다.
결과: 두 가지 부하 검사 모두에서 새로운 고정 방법이 기존 방법에 비해 load-to-failure나 torque-to-failure, construct stiffness가 열등하지 않음을 확인할 수 있었다. 골다공성 원위 비골의 단순 사형 골절에 대해, 세 개의 2.7 –mm 소형 나사를 이용한 양피질 골편간 고정 방법(bi-cortical inter-fragmentary fixation)은, 임상적 성과가 이미 소개된 3.5-mm 피질골 지연 나사 고정법에 비해 외팔보 휨 부하나 염전 부하 검사 결과에서 열등하지 않았다.
결론: 새로운 고정 방법은 굵기가 가는 원통형의 비골 에서, 기존의 3.5mm 나사보다, 더 많은 나사를 안전하게 삽입하여 보다 적정한 골절 고정을 하는데 좋은 대안이 될 수 있을 것이다.
Introduction: The fixation of simple oblique fractures of short tubular bones with only inter-fragmentary screws is simple and straightforward, but its stability is dubious.
Methods: This study compared the biomechanical properties of fixation using three 2.7–mm mini-screws and fixation using two 3.5–mm lag screws construct for simple oblique fractures of the distal fibula in osteoporotic bone. Isolated simple oblique fractures of the distal fibula at the level of the syndesmosis were examined with a fibular osteotomy and ligamentous sectioning in 15 paired fresh frozen ankles. The bone mineral density (BMD) was measured. Fractures were fixed with either a mini-screw fixation system using three 2.7–mm mini-screws (group 1) or two 3.5-mm lag screws (group 2) in each pair of ankles. The specimens were examined under a cantilever bending load and an external rotational load to failure. The torque-to-failure, load-to-failure, and construct stiffness of the two fixation groups were compared using a non-inferiority test, with a pre-specified non-inferiority margin. The mode of failure for each specimen was observed. The BMD of calcaneus was assessed as osteoporotic based on reference values for 20- to 29-year-old healthy Koreans.
Results: The torque-to-failure, load-to-failure, and construct stiffness of the group 1 were not inferior to those of the group 2. For the group 1, the construct failure was initiated from the distal-most screw hole in the anterior cortex. Fixation with only three 2.7-mm mini-screws provided biomechanical stability comparable to conventional method for simple oblique osteoporotic fractures in the distal fibula under torsional stress, and is straightforward and simple.
Conclusions: Fixation with three 2.7-mm mini-screws only may be an alternative treatment for simple oblique fractures of the distal fibula, with torsional stability similar to that with the two 3.5-mm lag screws fixation method.
