Abstract
STAT4 다형성 rs7574865는 전신 홍반성 루프스, 류마티스 관절염과 같은 다양한 자가면역질환과 관련된 유전자이다. STAT4 rs7574865 T 대립유전자는 STAT4 mRNA와 단백질 발현을 증가시키는 것으로 알려져 있고, 이는 표준보다 왜곡된 면역반응을 일으킬 가능성을 시사한다. 자가면역질환 환자 또는 일반인을 대상으로 STAT4에 대해 많은 연구가 이루어졌으나 면역억제 환자를 대상으로 하는 연구는 많이 부족한 실정이다. 특히 조혈모세포이식 후와 같이 면역반응의 조절이 매우 중요한 특수 상황에서 STAT4의 역할을 발견하는 것은 큰 의미를 가진다. 이에 저자들은 공여자의 rs7574865 T 대립유전자의 존재가 동종조혈모세포이식 후의 면역반응과 임상결과에 미치는 영향에 대하여 연구하였다.

2001년 11월부터 2014년 4월까지 서울대학교 어린이병원에서 급성백혈병으로 조혈모세포이식을 시행 받았으며, 이식 후 공여자의 조혈세포로 완전히 치환된 결과를 보이는 161명의 환자를 대상으로 하였다. 유전형질분석을 위해 중합효소 연쇄반응(PCR) 및 제한단편길이다형성 분석을 시행하였다.

TT 유전자형을 가진 경우 자가면역질환에 취약하다는 것이 알려진 바 있어, TT 유전자형을 가진 그룹과 나머지 GG와 GT 유전자형을 가진 환자의 두 그룹으로 나누어 합병증, 재발, 전체 생존율에 대해 비교분석을 시행하였다. 비교분석에 포함된 조혈모세포이식의 합병증은 간정맥폐쇄질환, 급성 및 만성 이식편대숙주병, 사이토메갈로바이러스 (CMV) 감염이었다. 분석 결과, TT 유전자형을 가지는 환자군에서 CMV감염 발생률이 다른 유전자형을 가진 그룹에 비하여 높았다. 다변량 분석을 통하여 TT 유전자형은 CMV 감염의 발생과 독립적으로 유의한 관계가 있음을 밝혀내었다.

이는 조혈모세포이식을 시행 받는 면역 저하 환자에서 rs7574865 에 대해 시행된 첫 번째 연구이다. 기존 연구들에서 TT 유전자형이 자가면역질환과의 연관되어 있음이 밝혀진 것과는 달리, 본 연구의 결과는 면역 억제자들에서는 역설적으로 STAT4 가 다른 역할을 할 가능성이 있음을 시사한다. 상기 결과를 보여준 근거에 대해 밝혀내고, 조혈모세포이식의 성적을 향상시킬 수 있도록 추가적인 연구가 필요할 것으로 사료된다.
STAT4 polymorphism, rs7574865 is linked to various autoimmune diseases such as systemic lupus erythematosus and rheumatoid arthritis. Its T minor allele is associated with higher STAT4 mRNA and protein expression, indicating a stronger skewed immune response than the norm. Although widely studied in autoimmune disease patients and the general population, its effect on immunocompromised subjects is still unknown. Especially in situations, i.e. post-hematopoietic stem cell transplantation (post-HSCT), where control of the immune response is crucial. Hence, this study investigates if the presence of the T minor allele in donors would affect immunological response and clinical outcomes post-HSCT. Samples from 161 pediatric patients who underwent allogeneic HSCT for acute leukemia and showed complete chimerism by donor cells were obtained. Six clinical outcomes were investigated
hepatic veno-occlusive disease, acute graft-vs- host disease, chronic graft-vs-host disease, cytomegalovirus (CMV) infection, relapse and overall survival. The TT genotype was found to be significant in the occurrence of CMV infection (P=0.049), showing higher incidence of CMV infection compared to the others. Multivariate analysis confirmed that association of the TT genotype is independent from other variables in CMV infection occurrence (P=0.010). This is the first study on STAT4 polymorphism rs7574865 in allogeneic HSCT as well as immunocompromised patients. As the TT genotype is associated with autoimmune diseases, our results seem at a paradox with current evidence hinting at a different role of STAT4 in normal circumstances versus immunocompromised patients. Further investigation is needed to elicit the reason behind this and discover novel applications for better post-transplant outcomes.
