약물 유발 심장 독성의 in vitro 평가법 개발

Abstract
Adherent cellular imaging을 바탕으로 region의 intensity를 측정하는 방법은 약물에 의한 심장 독성 평가에 이용되어 왔다. 부착되지 않은 상태의 isolated single cell을 요구하는 single cell cytometry와 달리, 이는 adherent cell의 isolation과 detachment를 위한 enzyme 처리를 필요로 하지 않으며 세포의 morphological information도 유지할 수 있다. 이를 확인하기 위해 green fluorescent protein (GFP)을 과다발현 시킨 human embryonic kidney (HEK)-293 세포의 isolated single cell과 adherent cell에서의 regional average intensity와 전체 세포 중 발현된 세포의 percentage를 비교하였다. 각 세포의 bright field image에서 defocused image를 얻고, 각 세포의 region을 잡아 형광 image에서의 region overlap 과정을 통해 측정하였다. 그 결과 세포의 isolation 과정 없이 adherent 상태의 세포를 통해서도 실험 결과가 유효하다는 결론을 얻고 human Ether-à-go-go-Related Gene (hERG)를 과다발현 시킨 HEK-293 세포를 이용하여 약물에 의한 심장 독성 평가를 수행하였다. 1980년대 얀센社에서 출시된 cisapride는 위장 운동 촉진제로 시장에 등장하였으나 심각한 심장질환 유발의 부작용으로 인해 기업에서 스스로 철수시킨 약물이다. 본 실험에서는 cisapride와 함께 심장에 독성을 나타내지 않는 것으로 알려진 dexamethasone을 각각 농도별로 처리한 세포에서 potassium ion channel assay를 수행하여 그 결과를 비교하였다. cisapride의 경우 약물의 농도가 높아질수록 세포의 potassium ion channel이 block 되었고, dexamethasone을 처리한 세포의 경우 약물의 농도에 따른 유의적인 변화를 보이지 않았다. 수많은 약물들이 부작용으로 인한 심장 독성의 유발로 인해 시장에서 퇴출되고 있다. 따라서 약물로 인한 심장 독성의 적절한 평가는 drug discovery에서 매우 중요한 문제이다. 본 연구의 결과를 바탕으로 in vivo 단계에서의 동물 모델 이전에 적용할 수 있는 in vitro 단계에서의 drug safety screening 평가로의 이용 가능성을 제시한다.
