Abstract
c-MET은 수용체-타이로신 인산화 효소 (receptor-tyrosine kinase) 중의 하나로, ABN401은 c-MET에 대하여 특이적인 저해작용을 가진다. 폐암과 위암에 항암 효과를 가지는 것으로 기대하고 있으며 현재 임상 1상을 준비 중에 있다. 하지만 선행 연구를 통해 ABN401이 rat에서 oral bioavailability(BA)가 낮게 나타나는 것이 밝혀져 있고 따라서 본 연구에서는 이러한 ABN401의 낮은 흡수율에 영향을 미치는 여러 요인들을 살펴보고 특히 solubility, permeability 그리고 intestinal metabolism 측면에서 접근하고자 한다.
먼저 완전히 용해된 ABN401을 SD rat에 정맥 및 경구 투여하여 각각의 약물동태 파라미터를 구하였을 때, 0.5, 2, 4 mg/kg의 용량범위에서 선형 체내동태를 따르는 것으로 나타났다. 이 때 전신 클리어런스(CL)는 12.6~ 16.3 ml/min/kg, 전신 분포 용적(Vss)는 3.32~ 4.85 L/kg임을 알 수 있었다. 정맥 및 경구 투여 결과로 얻은 AUCinf의 비로 계산한 BA는 22.4~ 24.5 %로 나타났으며 실험 용량에서 통계학적으로 유의미한 차이를 보이지 않았다.
ABN401의 인공 장액에서의 용해도는 0.00746 ± 0.000337 mg/ml로, 인공 위액에서보다 1000배 이상 더 낮게 나타났으며 in vivo 상에서 약물의 소장 내 석출 가능성에 대해 생각해 볼 수 있었다. 약물의 용해 및 석출 동태를 살펴보기 위해 in vitro dissolution-precipitation study를 수행하였고, 이를 통해 dissolution rate constant(kdiss), nucleation rate constant(knuc) 그리고 molecularity index(α)를 예측하였다.
Ussing chamber study를 통해 rat 소장에서의 ABN401의 permeability를 살펴본 결과, Papp가 10.5~ 11.3 cm/s × 10-6로 비교적 permeable하였으며, 실험 용량 범위에서 용량 의존적 Papp가 관찰되지 않았다. 대표적인 MDR1 저해제인 verapamil을 ABN401과 함께 투여한 군에서는 ABN401의 투과도가 유의미하게 증가하는 것을 통해 rat MDR1과의 상호작용을 간접적으로 확인할 수 있었다.
Rat intestine homogenates에서 ABN401의 metabolic stability를 평가하였을 때 CLint은 0.0190 ml/min이며 이를 통해 계산한 Fg는 0.828이다. ABN401의 신장 배설이 1% 미만임이 밝혀져 있으므로 systemic CL가 곧 hepatic CL(i.e. non renal CL)이라고 가정하고 계산한 Fh는 0.817이다. 이를 통해 소장과 간에서의 대사는 ABN401의 낮은 흡수율에 크게 영향을 미치지 않음을 알 수 있었다.
Dissolution-precipitation model과 ADAM model을 적용하여 in vivo absorption model을 세우고 이전에 구한 Papp, kdiss, knuc 그리고 α를 대입하여 예측한 Fa는 0.537이다. BA는 Fa, Fg 그리고 Fh의 곱으로 나타낼 수 있고 본 연구를 통해 예측한 값으로 계산한 predicted BA는 36.3 %이다. 이는 observed BA의 약 1.6배로 추정치와 실측치가 크게 차이나지 않는 것으로부터 흡수율 예측에 사용된 model이 비교적 유효하다고 사료되었다.
ABN401의 흡수 과정을 전체적으로 봤을 때, intestine metabolism이나 hepatic first pass 과정은 major한 요인은 아닌 것으로 볼 수 있으며 가장 key step은 약물의 용해도를 개선하여 위장관을 통한 permeation의 기여도를 증가시키는 것이라고 할 수 있다. 약제학적 관점에서 이를 개선한다면 ABN401의 전체적인 흡수를 증가시킬 수 있고 약물의 활용성을 더 높일 수 있을 것이다.
c-MET is one of receptor tyrosine kinases and ABN401 has a specific inhibitory effect on c-MET. ABN401 is expected to have anticancer effect on lung cancer and stomach cancer, and is currently on the stage of preclinical studies. However, previous studies have shown that ABN401 has low oral bioavailability(BA) in rats. Therefore, the objective of this study is to determine major factors governing the oral absorption of ABN401 in rats especially in aspect of solubility, permeability and intestinal metabolism.
ABN401 was dissolved clearly in a dosing solution and administrated intravenously / orally to male Sprague-Dawley rats at the doses of 0.5, 2 and 4 mg/kg for the characterization of the systemic pharmacokinetics. The dose-normalized AUCinf values for the compound were found to be linear in the dose range studied: The oral BA was found to be approximately 22.4 to 24.5%.
The solubility of ABN401 in simulated gastric fluid and intestinal fluis was determined and the solubility was found to be 11.4 and 0.00746 mg/ml, respectively. This results suggest that the precipitation of ABN401 may occur during the absorption process. To evaluate the impact of the precipitation/dissolution on the intestinal absorption of ABN401, in vitro precipitation study was carried out at ABN401 concentrations ranging from 0.0861 to 0.430 mg/ml in simulated intestinal fluid. The rate constant for the precipitation was estimated to be 0.209~ 0.606 min-1 depending on the initial concentration in simulated intestinal fluid. To characterize the kinetics of ABN401 transport in the rat intestinal epithelium, Ussing chamber study was carried out. The effective permeability ranged from 10.5 to 11.3 x 10-6cm/s in 20, 50 or 100 µM of ABN401 in 1% DMSO transport media. In a group of ABN401 treated with verapamil, a representative MDR1 inhibitor, the permeability of ABN401 is significantly increased, suggesting that ABN401 might interact with rat MDR1. Metabolic stability of ABN401 in rat intestinal homogenate was measured to evaluate the contribution of metabolic elimination by rat intestine. The estimated intrinsic clearance was 0.0190 ml/min and accordingly, calculated Fg was 0.817. Since the urinary excretion was minor for ABN401 (i.e., less than 1% of the dose) in rats, Fh, as predicted from the systemic clearance, may be close to 0.817.
The in vivo absorption model is established and the fraction absorbed(Fa) is predicted to be 0.537 using previously obtained parameters. The bioavailability calculated as the product of predicted Fa, Fg and Fh was 36.3% which is approximately 1.6 fold of the observed bioavailability.
