급성 허혈성 뇌경색 환자에서 초기 기능적 예후인자로서 Cystatin C의 역할에 관한 연구

Abstract

Objective: Cystatin C is a competitive inhibitor of lysosomal cysteine protease. Recent studies suggest cystatin C is an accurate marker of kidney dysfunction, cardiovascular risk and prognosis. We evaluated the association between cystatin C and early functional outcome in acute stroke patients. Methods: We retrospectively evaluated 714 consecutive patients with acute ischemic stroke who were admitted to Seoul National University Hospital between January 2008 and May 2011. We classified patients to each four groups based on the level of cystatin C and estimated glomerular filtration rate. We defined the unfavorable outcome using responder analysis, which outcome was adjusted by initial severity of stroke. Multivariable logistic regression analysis was used to evaluate the association with cystatin C, estimated GFR and clinical outcome. Results: Among 714 patients, 544 patients were classified to the unfavorable outcome (76.2%). A stepwise increase in the proportion of patients with an unfavorable outcome was seen across cystatin C quartiles. The association between cystatin C quartiles and unfavorable outcome remained significant after adjustment of possible confounders [adjusted odds ratio (95% confidential interval) Q1: reference, Q2:2.07(1.00, 4.27), Q3:2.50(1.14, 5.51), Q4:2.87(1.14, 7.18)]. In contrast, estimated GFR did not show any significant association with unfavorable outcome. Conclusions: Our results demonstrate that cystatin C is associated with functional outcome in stroke patients. Cystatin C may be a potent predictor of functional outcome after ischemic stroke.

배경 및 목적: Cystatin C는 리소솜 시스테인 프로테아제(lysosome cysteine protease)의 경쟁적 억제제 중 하나이다. 최근 연구에서 cystatin C는 크레아티닌(creatinine)이나 크레아티닌을 이용한 사구체 여과 비율(estimated glomerular filtration rate)보다 신기능을 정확하게 반영할 수 있는 새로운 지표로 밝혀졌다. Cystatin C는 경미한 정도의 신기능 장애도 민감하게 반영함으로써, 심혈관 질환의 위험도와 예후를 예측할 수 있는 예측 인자로서의 역할도 수행하는 것으로 밝혀졌다. 하지만 뇌졸중 환자에서 cystatin C에 관한 연구는 드물어 이에 급성 허 혈성 뇌 경색 이후 초기 기능적 예후에 대한 예측 인자로서 cystatin C의 역할에 대해서 고찰해 보고자 하였다. 방법: 2008년 1월에서 2011년 5월까지 서울대학교 병원 신경과에 입원한 714명의 급성 허 혈성 뇌 경색 환자를 대상으로 연구를 진행하였다. Cystatin C와 크레아티닌을 이용한 사구체 여과 비율을 기준으로 네 군으로 나누었다. 불량한 기능적 예후 (unfavorable functional outcome)는 처음 입원 당시 신경학적 결손의 정도를 보정한 퇴원 시 기능적 독립 수준 점수 (modified rankin scale at discharge)를 바탕으로 정의하였다. 다 변량 로지스틱 회귀 분석 (multivariable logistic analysis)이 cystatin C와 크레아티닌을 이용한 사구체 여과 비율과 임상적 예후와의 관계를 가늠하기 위해 사용되었다. 결과: 전체 714명 중 544명(76.2%)이 불량한 기능적 예후를 보였고, cystatin C의 농도를 기준으로 사분 위(quartile)로 환자를 나누었을 때, cystatin C 농도가 높은 군일수록 불량한 기능적 예후를 갖는 환자 군의 비율이 높았다. 또한 다 변량 회귀 분석 결과, cystatin C의 경우 임상적인 교란 변수를 보정한 다음에도 가장 낮은 분 위에 속하는 군에 비하여 분 위가 높은 군일수록 불량한 기능적 예후를 갖게 될 오즈 비(odds ratio)가 의미 있게 증가하는 경향을 보였다. 반면에 크레아티닌을 이용한 사구체 여과 비율의 경우 예후와의 중요한 관련성이 있음을 입증하지 못했다. 결론: 본 연구에서는 cystatin C가 허 혈성 뇌 경색 환자의 기능적 예후를 예측하는 신뢰할 만한 예측 인자가 될 수 있음을 시사하였다.
