I급 복합레진 수복물에서 치면열구전색재와 표면전색재 적용시 미세누출에 관한 비교연구

Abstract

본 연구는 복합레진으로 수복한 I급 와동에서 변연부 미세누출에 대한 2종의 치면열구전색재(Ultraseal XT plus™, Clinopro™)와 표면전색재(Fortify™, Biscover LV™)의 효과를 평가하고, 또한 전색재를 적용하지 않은 복합레진 수복물과의 미세누출 차이를 비교하기 위하여 시행하였다. 100개의 발거된 소구치에 I급 와동을 형성하고 복합레진을 충전한 후, 전색재에 따라 다음과 같이 5군으로 분류하였다; 복합레진에 전색재를 적용하지 않은 대조군, 복합레진에 전색재를 적용한 Fortify™ 군, Biscover LV™ 군, Clinopro™ 군 및 Ultraseal XT plus™ 군으로 분류하였다. 각 군의 시편을 5℃와 55℃에서 10000회의 열 순환을 시행하고 1% methylene blue 용액에 24시간 동안 침적시켰다. 각 치아는 저속의 diamond disk를 이용하여 협설측으로 치아 장축에 평행하게 두 번 절단하여 수복물의 단면을 노출시켰다. 각 시편당 변연 4곳의 색소침투도를 입체현미경으로 관찰하고 사진촬영을 하였다. Image J™을 이용해 전색제와 수복물의 총 길이 및 색소가 침투된 길이를 구한 후 변연 미세누출 퍼센트를 구하였다. 치아시편상에서 I급 와동을 복합레진으로 수복한 군과 그 위 치면열구전색재 및 표면전색재를 적용한 군들의 미세누출에 대한 유의성 검정을 시행하여 다음과 같은 결과를 얻었다. 1) 대조군을 비롯한 모든 군에서 미세누출을 보였지만 전색재를 적용한 실험군은 복합레진으로만 수복한 대조군에 비해 통계적으로 유의한 감소 효과가 있었다(P < 0.05). 2) Biscover LV™ 도포군이 실험에 사용된 전색재 중 가장 높은 미세누출을 보였다(P < 0.05). 3) Clinopro™ 군과 Fortify™ 군이 Biscover LV™ 군에 비해 낮은 미세누출을 보였으나(P < 0.05) 서로 간에는 통계학적으로 유의한 차이는 없었다(P > 0.05). 4) Ultraseal XT plus™ 군이 다른 실험군에 비해 통계학적으로 가장 낮은 미세누출을 보였다(P < 0.05).

The purpose of this study was to verify the influence of sealants and to compare the effect of two conventional (Ultraseal XT plus™, Clinopro™) and surface sealants (Fortify™, Biscover LV™) on marginal microleakage of Class I composite resin restorations. Class I cavities were made on the occlusal surface of 100 permanent premolars and restored with composite resin (Z100™). The teeth were divided into groups according to the sealant agent; group 1-control group without the application of sealant, group 2-Fortify™, group 3-Biscover LV™, group 4-Clinopro™ and group 5-Ultraseal XT plus™. Specimens were thermocycled 10000 times, and immersed in a 1% methylene blue solution for 24 hours. Each specimen was sectioned longitudinally in a buccolingual direction with a low speed diamond disk. The percentage of microleakage was calculated and results were submitted to analysis of variance and Tukeys HSD test. The results of this study were as follows: 1) Microleakage was observed in all groups but the experimental groups showed lower microleakage values than the control group, which were statistically significant(P < 0.05). 2) Biscover LV™ group showed the highest values of marginal microleakage among the experimental groups(P < 0.05). 3) Clinopro™ and Fortify™ group demonstrated lower microleakage values than the Biscover LV™ group(P < 0.05) but there were no statistically significant differences between the two groups(P > 0.05) 4) Ultraseal XT plus™ showed the lowest microleakage value among the experimental groups(P < 0.05).
