측두하악관절의 하악과두 관절면에 이환된 골관절염의 발생 양태

Abstract
The aim of this study was to evaluate occurring patterns of arthritic changes on the surfaces of the mandibular condyles and its relationship with clinical signs and symptoms in the patients with osteoarthritis (OA) of the temporomandibular joint (TMJ).
The computed tomographic (CT) images and clinical records of 466 patients (86 males and 380 females whose mean ages were 30.5 ± 15.6 and 33.6 ± 16.1 years, respectively) who had been diagnosed by historical, clinical, and CT examination with TMJ OA without any systemic disease possibly related to joint diseases such as rheumatoid arthritis were retrospectively reviewed.
To determine the occurring patterns of OA changes, the condylar surface was divided into nine imaginary sections; antero-medial (AM), antero-central (AC), antero-lateral (AL), centri-medial (CM), centri-central (CC), centri-lateral (CL), postero-medial (PM), postero-central (PC), and postero-lateral (PL) surface. The occurring patterns of OA were determined based on the distribution of sections involved by OA changes and analyzed by hierarchical cluster analysis. The differences of clinical signs and symptoms among clusters were analyzed.
OA changes were observed the most frequently on AC (63.5%) followed by AM (57.5%), CC (47.6%), AL (43.3%) and CL (43.3%) in the right TMJ and AC (61.3%)
followed by AM (52.3%), CC (48.9%) and AL (42.6%) in the left TMJ. Based on the hierarchical cluster analysis, the patterns of OA changes were classified into 6 clusters in each side of the joint. While frequency of clinical pain and noise of TMJ and mean of maximum mouth opening range and painless mouth opening range were not significantly different among clusters, the gender differences among clusters were observed in the left TMJ (P = 0.025). The difference of craniomandibular index was significant in just one pair of clusters of the right TMJ (P = 0.020) and no pair of clusters of the left TMJ and the difference of mean age was significant in no pair of clusters of the right TMJ and just one pair of clusters of the left TMJ (P = 0.001).
Collectively, OA changes were more likely to occur on the anterior and central surface of the mandibular condyle in both side of the TMJ. Meaningful differences of the clinical signs and symptoms of the TMJ involved by OA were not observed according to the occurring patterns of OA changes classified by hierarchical cluster analysis.
본 연구는 측두하악관절의 골관절염에 이환된 환자들에서 하악과두 표면의 골변화 양상과 임상적인 증상간의 상관관계에 대한 자료를 제공하고자 한다.
환자의 병력, 임상적 증상 및 전산화 단층촬영(CT)을 근거로 측두하악관절의 골관절염으로 진단받고 류마티스 관절염 등 관절에 이환 가능한 다른 전신질환이 없는 466명의 환자(86명의 남자환자 및 380명의 여자환자로 구성)의 전산화 단층촬영(CT)과 임상적 기록을 조사하였으며, 이들의 평균연령은 각각 남자 30.5 ± 15.6세, 여자 33.6 ± 16.1세였다.
골관절염의 발생양상을 나타내기 위하여 관절표면을 가상적인 9개의 분할면(내측부의 전방면, 중앙부의 전방면, 외측부의 전방면, 내측부의 중앙면, 중앙부의 중앙면, 외측부의 중앙면, 내측부의 후방면, 중앙부의 후방면, 외측부의 후방면)으로 나누었다. 골관절염이 이환된 면의 분포를 계층적 군집분석으로 분석하였으며, 각 군간 임상적 증상의 차이를 분석하였다.
골관절염의 발생빈도는 오른쪽 측두하악관절에서 중앙부의 전방면이 63.5%로 가장 높게 나타났으며, 다음으로 내측부의 전방면(57.5%),
중앙부의 중앙면(47.6%), 외측부의 전방면(43.3%) 및 외측부의 중앙면(43.3%) 순으로 높게 나타났다. 왼쪽 측두하악관절에서는 중앙부의 전방면에서 61.3%로 가장 높게 나타났으며, 내측부의 전방면(52.3%), 중앙부의 중앙면(48.9%), 외측부의 전방면(42.6%) 순으로 높게 나타났다. 계층적 군집분석의 결과, 골관절염 발생양상은 오른쪽과 왼쪽의 측두하악관절 모두에서 총 6개의 군(cluster)로 나뉘어졌다. 통증과 관절잡음, 최대개구량은 각각의 군간 유의한 차이를 나타내지 않았으나 왼쪽 측두하악관절에서는 성별에 따라 각각의 군간 유의한 차이를 나타내었다(p=0.025). CMI값은 오른쪽 측두하악관절의 한쌍의 군에서만 유의한 차이를 나타내었으며(p=0.020) 왼쪽 측두하악관절에서는 유의한 차이를 나타내지 않았고, 평균연령은 오른쪽 측두하악관절에서는 유의한 차이를 나타내지 않았으나 왼쪽 측두하악관절에서는 한쌍의 군에서만 유의한 차이를 나타내었다(p=0.001).
이상의 결과로부터 측두하악관절의 골관절염은 오른쪽과 왼쪽 모두에서 중앙부의 전방면에 호발하고, 골관절염의 발생양상을 계층적 군집분석으로 분류한 발생양상과 임상적인 증상간에는 유의한 상관관계가 없음을 알 수 있다.
