클리어 레진의 첨가가 다이렉트 레진의 반투명도 및 형광특성에 미치는 영향에 관한 연구

Abstract

자연치아의 색은 법랑질과 상아질의 빛 흡수와 사란에 의한 내적인 요인과 외부 색소의 침착에 의한 요인에 의해 결정된다. 다이렉트 레진을 이용한 임시 수복물 제작 시 자연치아와 색조를 재현할 수는 있다 하더라도, 투명도 및 형광성 등의 광학적인 특성에서 기인하는 자연치의 느낌을 표현하기는 어렵다. 일부 임상적 상황에서는 전치부 임시수복물의 투명도를 재현하기 위해 교정장치나 스플린트 제작에 사용되는 클리어 레진을 첨가하여 수복물을 제작하기도 한다. 본 연구에서는 임시수복용 다이렉트 레진에 클리어 레진을 첨가가 다이렉트 레진의 반투명도 및 형광특성을 변화시키지 않는다는 귀무가설을 검증하기 위해, 클리어레진을 첨가한 다이렉트 레진시편의 반투명도 지수와 형광특성을 측정하여 이들의 상관관계를 비교함으로써 클리어레진의 첨가가 광학적 특성에 미치는 영향을 알아보고자 하였다. 세 가지 색상 군(A2, A3, D3) 의 임시수복용 다이렉트레진(Jet tooth shade powder, Lang Dental, IL, USA)과 클리어레진(Ortho-Jet, Lang Dental, IL, USA)을 무게비(첨가비율) 3:1(25%), 2:1(33%), 1.5:1(40%) 1:1(50%), 1:2(66.67%)로 균일 혼합하여 중합 후 연마하여 최종 시편(직경10mm, 두께1mm)을 완성하였다. 시편의 수는 혼합 비 군 당 5개로 하였다. 시편의 반투명도 지수를 간접적으로 구하기 위해 Reflection spectrophotometer(CM-3500d, Minolta, Osaka, Japan)를 이용하여 흰색 배경과 검은색 배경 하에 색을 CIE L*, a*, b*값으로 측정하였으며(SCE, CIE diffuse/10 geometry), 시편 당 3회 측정 후 평균값을 계산하였다. 반투명도 지수(TP: translucency parameter)는 흰색 및 검은색 표준 배경에 따른 color 측정값 차이를 계산하여 얻었다: TP=[(LW*-LB*)2+(aW*-aB*)2+(bW*-bW*)2]1/2(W: white backing, B: black backing). 시편의 형광 지수 측정을 위해 Reflection spectrophotometer(Color-Eye 7000A, GretagMacbeth, Regensdorf, Swiss)를 이용하여 UV(100%)와 UV(0%)조건에서 색을 CIE L*, a*, b*값으로 측정하였다. 형광지수(FL)는 D65발광체의 자외선 유무에 따른 색상차이로 계산하였다: FL=[(L100*-L0*)2+(a100*-a0*)2+(b100*-b0*)2]1/2(100: UV 100%, 0: UV 0%). 클리어 레진의 첨가와 색조에 따른 TP, FL의 변화에 대하여 각각 이원분산분석(significance level: 0.05)을 시행하였다. TP 및 FL 분석에서 사후검증은 Tukey test를 사용하였고, 모든 통계 분석은 95% 신뢰수준에서 검증하였다. 클리어레진의 첨가량과 TP, FL 의 상관관계는 Pearson product moment correlation 방법으로 구하였다. 클리어 레진 첨가 후에 다이렉트 레진의 반투명도 지수 및 형광특성은 클리어레진의 첨가에 따라 통계적으로 유의한 차이를 보였으며(p<0.05, 2-way ANOVA), 클리어레진의 첨가량과 통계적으로 유의한 양의 상관관계를 보였다. 형광지수의 경우 클리어레진의 첨가량과 통계적으로 유의한 음의 상관관계를 보였다. 즉, 클리어 레진을 첨가하는 경우 다이렉트 레진의 반투명도는 증가하나 형광특성을 감소하므로 임상 적용시 이를 유념해야 할 것으로 사료된다.

Null hypothesis of this study is translucency and fluorescence of provisional direct resin is not affected by adding clear resin. One brand of provisional crown and bridge resin powder(Jet tooth shade powder) and one brand of clear resin powder(Ortho-Jet) were investigated. A series of specimens of 10 mm in diameter and 1 mm in thickness were made by mixing the direct resin powder and clear resin powder in the ratio of 3:1, 2:1, 1.5:1, 1:1, 1:2 by weight. Five specimens were made for each group. For translucency, color was measured in the CIELAB scale using spectrophotometer(CM-3500d, Minolta, Osaka, Japan). Translucency parameter(TP) values were calculated by the equation TP=[(LW*-LB*)2+(aW*-aB*)2+(bW*- bW*)2]1/2 . For fluorescence parameter(FL), color was measured with a reflection spectrophotometer(Color-Eye 7000A, GretagMacbeth Instruments Corp.). A UV filter was positioned to 0% UV or 100% UV condition. Fluorescence parameter(FL) were calculated by the equation FL=[(L100*-L0*)2+ (a100* -a0*)2+(b100*-b0*)2]1/2. FL and TP values were compared by two way ANOVA by shade and clear resin ratio group combination. Both TP and FL values of direct resin were influenced by adding clear resin(P<0.05). There were significant difference in both TP and FL value by shade and clear resin ratio group combination(p<0.05), and For TP and FL, pearson correlation coefficiency was 0.878 and -0.641 respectively.
