만성요통 환자의 한의원 외래이용 지속성이 척추수술에 미치는 영향

Abstract

연구 목적: 만성요통 환자의 한의원 외래이용 지속성이 척추수술에 미치는 영향을 분석하는 것임.

방법론: 국민건강보험 표본코호트 2.0 DB에서 2004년 1월 1일부터 2013년 9월 31일까지 요통으로 진단 받은 환자 중 ①만성요통으로 이환 되었으며 ②한의원을 최빈의료기관으로 활용하는 환자를 대상으로 함. 지속성 측정은 COC를 활용하였으며, 요통 발병 시점부터 환자가 중도탈락하거나 수술을 받을 시점까지 1개월 단위로 측정함. 분석은 시간의존 콕스회귀분석을 활용하였음.

결과: 연구대상은 21,907명임. 다중 분석 결과 지속성이 낮을 수록 척추수술을 받을 위험도가 높아짐(HR:2.08, 95% CI:1.62-2.67).

고찰 및 결론: 만성요통 환자의 한의원 외래 이용 지속성은 척추수술의 위험을 낮추는 것으로 파악 됨.

Object : To investigate the effects of outpatient continuity of care in traditional Korean Medicine clinic on the spinal surgery in patients with chronic low back pain.

Methodology: Among the patients diagnosed with back pain from January 1, 2004 to September 31, 2013 in the National Health Insurance sample cohort 2.0 DB, 1) patients with chronic back pain transition and 2) patients who used the traditional Korean Medicine clinic as most frequently visited provider. The COC was used to measure continuity of care, and it was measured on a monthly basis from the time of the onset of back pain to the time when the patient dropped out or received surgery. The time-dependent Cox regression analysis was performed.

Results: There were 21,907 subjects. Multivariate analysis showed that the lower the continuity of care, the higher the risk of spinal surgery (HR: 2.08, 95% CI: 1.62-2.67).

Discussion and Conclusion: The outpatient continuity of care in traditional Korean Medicine clinic with chronic low back pain patients may reduce the risk of spinal surgery.
