dc.description.abstractIt is challenging to predict fluid responsiveness, that is, whether the cardiac index or stroke volume index would be increased by fluid administration, in the pediatric population. Previous studies on fluid responsiveness have assessed several variables derived from pressure wave measurements, plethysmography (pulse oximeter plethysmograph amplitude variation), ultrasonography, bioreactance data, and various combined methods. However, only the respiratory variation of aortic blood flow peak velocity has consistently shown a predictive ability in pediatric patients. For the prediction of fluid responsiveness in children, flow- or volume-dependent, noninvasive variables are more promising than pressure-dependent, invasive variables. This article reviews various potential variables for the prediction of fluid responsiveness in the pediatric population. Differences in anatomic and physiologic characteristics between the pediatric and adult populations are covered. In addition, some important considerations are discussed for future studies on fluid responsiveness in the pediatric population.
소아에서 수액 반응성, 즉 수액 투여를 함으로써 심박출량지수가 증가하는지 예측하는 것은 쉽지 않다. 수액 반응성 예측에 관한이전 연구들은 혈압 파형, 체적변동기록(plethysmography, 호흡에 따른 맥박산소측정기 체적변동기록 진폭 변화), 초음파로 측정한 자료, 바이오리엑턴스(bioreactance) 자료, 다양한 복합적 기법으로 얻은 여러 변수들을 평가하고 있다. 하지만, 초음파로 측정한 호흡에 따른 최대 대동맥 혈류속도의 변이만이 소아에서 일관되게 수액반응을 예측할 수 있는 것으로 보고되고 있다. 소아의 수액 반응성 예측에 있어서, 혈류나 혈액량에 의해 결정되는 비침습적 변수가 압력에 의존하는 침습적 변수보다 더 유용할 것이다. 본 논문은 소아의 수액 반응성 예측에 잠재적 가능성이 있는 여러 변수들을 고찰하였다. 수액 반응성을 해석할 때 고려해야 하는소아와 성인의 서로 다른 해부 및 생리적 특징도 다루었다. 또한, 소아 수액 반응성에 대한 향후 연구와 관련된 중요 고려 사항을논의하였다.
