ICT 기반 장애인 구강보건진료서비스모델에서 치과위생사의 역량 개발

Abstract

1. 목 적 이 연구의 목적은 ICT 기반 장애인 구강보건진료서비스 모델에서 치과위생사의 역할을 검토하여, 장애인 구강보건진료발전을 위한 치과위생사 인력 양성방안을 제시하는 것이다.

2. 방 법 ICT 장애인 구강보건진료서비스 모델에서 치과위생사 역할 정의와 장애인 구강보건진료서비스 수행을 위해 필요한 치과위생사 역량을 도출하였다. 더블다이아몬드 프레임 방법을 통하여 치과위생사 역할이 부여된 ICT 기반 장애인 구강보건진료서비스 모델 고안과 타당도 평가를 수행하였고, 의료경영컨설팅, 공급자심층면접 질적분석, 델파이조사를 통하여 치과위생사 역할의 적절성을 평가하였다. 장애인 구강보건진료서비스 수행에 요구되는 치과위생사 역량 평가와 치위생교육 개선 방향 도출을 위하여 장애인 구강보건진료 전문가 13인을 대상으로 심층면접을 수행하였다. 수집된 자료를 치과위생사 역량 기준으로 연역적 방법을 통하여 전문가들이 추구하는 치과위생사 역량에 대한 모델링을 하였다.

3. 결 과 수요자와 공급자 대상 요구도 조사로 비즈니스모델링을 수행하였고, 도출된 핵심 인력인 치과위생사의 역할 정의를 포함한 ICT 기반 구강보건진료서비스 모델을 고안하였다. 고안된 구강보건진료서비스 모델 내 치과위생사 역할에 대한 타당도 평가를 위해 의료경영컨설팅, 공급자 심층면접, 델파이조사 결과, 고안된 현장치과위생사의 역할에 대한 적절성이 높게 분석되었다. 장애인 구강보건진료서비스에서 요구되는 치과위생사의 역량을 전문가 심층면접을 통하여 분석한 결과, 주로 전문가적 행동, 의사소통, 임상치위생, 임상치과의 4가지 분야에서 요구되는 역량들이 나타났으며, 장애인 구강보건진료서비스 개선을 위한 임상과 교육개선 방안을 도출하였다. 결론적으로 장애인 구강보건진료서비스 수행을 위한 치과위생사의 제도적 문제점과 요구되는 전문가적 행동, 의사소통, 장애인 임상실무 역량을 도출해 낼 수 있었으며, 임상과 교육분야의 개선방안을 도출할 수 있었다.

1. Purpose This study aims to examine the role of dental hygienists in the Information communication technology (ICT) based oral healthcare service model for disabled people and propose improvement measures for the dental hygiene system within the development of oral health care for this population in Korea.

2. Methods This study defined the role of dental hygienists in the ICT oral healthcare service model for disabled people and identified the competencies of dental hygienists regarding oral health care service. The ICT-based oral health care service model, including the dental hygienist role, was designed and validated through the double diamond method. The adequacy of the dental hygienist role was evaluated through medical management consulting, in-depth interviews with suppliers, and Delphi surveys. In-depth interviews were conducted with 13 experts in oral health care for the disabled to evaluate dental hygienists competencies required to perform oral health care services for this population and identify improvement strategies regarding dental hygiene education. Based on data collected and identified competencies for dental hygienists, competency modeling was performed using a deductive method regarding the attributes of dental hygienists sought by experts.

3. Results Business modeling based on the assessment of needs and demands of consumers and suppliers was conducted. Later, an ICT-based oral health care service model was designed, which included the definition of the role of dental hygienists, as a key partnership. To evaluate the validity of dental hygienists role in the oral health care service model, health care management consulting, in-depth interviews with suppliers, and Delphi surveys were conducted. Results showed a high adequacy of the designed role of on-site dental hygienists. As a result of analyzing in-depth interviews of experts on dental hygienists competencies required for oral health care service addressed to disabled people, it was found that these competencies mainly involved four areas: professional behavior, communications skills, clinical dental hygiene, and clinical practice. Measures to improve the clinical and educational aspects of oral health care service for disabled people were also derived. In conclusion, this study identified the institutional problems and required dental hygienist capabilities when oral health care services are provided for the disabled. Additionally, improvement measures with clinical and educational applicability were identified.
