A population pharmacokinetic model of intravenous dexmedetomidine after pediatric brain surgery : 소아 뇌수술 후 투여된 덱스메데토미딘의 집단 약동학 모형

Abstract
Dexmedetomidine is a selective alpha-2 adrenergic agonist with concurrent sedative and analgesic effects, and it is being increasingly used in pediatric anesthesia and intensive care. This study aimed to investigate the pharmacokinetics of intravenous dexmedetomidine in mechanically ventilated children in the intensive care unit (ICU) after brain surgery.
Pediatric patients aged 2–12 years, who were mechanically ventilated in ICU after brain surgery, were allocated into a low-dose (n = 15) or high-dose (n = 14) group. The low-dose group received dexmedetomidine at a loading dose of 0.25 µg/kg for 10 min, followed by a maintenance dose of 0.25 µg/kg/h for 50 min, whereas the high-dose group received dexmedetomidine at a loading dose of 0.5 µg/kg for 10 min, followed by a maintenance dose of 0.5 µg/kg/h for 50 min. Serial blood samples were collected for a pharmacokinetic analysis up to 480 min after the end of the infusion. The sedative effect of dexmedetomidine was assessed using the Bispectral Index and University of Michigan Sedation Scale. Adverse reactions, electrocardiography findings, and vital signs were monitored for a safety assessment. A population pharmacokinetic analysis was performed using non-linear mixed effects modeling.
Dexmedetomidine induced a moderate-to-deep degree of sedation during infusion in both groups. The pharmacokinetics of dexmedetomidine were best described by a two-compartment disposition model with first-order elimination kinetics. The parameters were standardized for a body weight of 70 kg using an allometric power model. The population estimates (95% confidence interval) per 70 kg body weight were as follows: clearance of 81.0 (72.9–90.9) L/h, central volume of distribution of 64.2 (50.6–81.0) L, intercompartment clearance of 116.4 (90.6–156.0) L/h, and peripheral volume of distribution of 167 (132–217) L. No serious adverse reactions or hemodynamic changes requiring the discontinuation of dexmedetomidine were observed.
In conclusion, dexmedetomidine had increased clearance in mechanically ventilated children in ICU after brain surgery, thereby indicating the need for frequent dosing to maintain a target plasma concentration.
배경: 덱스메데토미딘은 진정과 진통 효과를 동시에 갖는 선택적 알파-2 아드레날린 수용체 작용제로, 최근 소아 마취와 소아 중환자 관리에 많이 사용되고 있다. 본 연구에서는 뇌수술 후 중환자실에서 기계 환기를 하고 있는 소아에서 정맥 주입 덱스메데토미딘의 약동학에 대해 알아보고자 하였다.
방법: 뇌수술 후 중환자실에서 기계 환기가 예정된 2-12세 소아들을 무작위로 저용량군과 고용량군으로 배정하였다. 저용량군에서는 덱스메데토미딘을 부하 용량으로 10분 동안 0.25 µg/kg 투여 후, 유지 용량으로 50분 동안 0.25 µg/kg/h 정맥 주입하였다. 고용량군에서는 부하 용량으로 10분 동안 0.5 µg/kg 투여 후, 유지 용량으로 50분 동안 0.25 µg/kg/h을 투여하였다. 약동학 분석을 위해 약물 투여 종료 후 480분까지 혈액 샘플을 수집하였다. 덱스메데토미딘의 진정 효과는 Bispectral Index와 University of Michigan Sedation Scale을 이용하여 평가하였다. 안전성 평가를 위해 약물 이상 반응, 심전도 소견 및 활력 징후를 지속적으로 감시하였다. 집단 약동학 분석에는 비선형 혼합 효과 모델링을 사용하였다.
결과: 두 군 모두 덱스메데토미딘이 투여되는 동안 중등도에서 깊은 진정 상태를 나타냈다. 덱스메데토미딘의 약동학은 1차 제거 동역학을 갖는 2구획 배치 모델에 의해 가장 잘 설명되었다. 매개 변수들은 allometric power model을 사용하여 체중 70kg의 성인으로 표준화되었다. 체중 70kg으로 표준화된 집단 추정치(95% 신뢰구간)는 청소율 81.0 (72.9–90.9) L/h, 중심구획용적 64.2 (50.6–81.0) L, 구획간 청소율 116.4 (90.6–156.0) L/h, 말초구획용적 167 (132–217) L로 나타났다. 덱스메데토미 딘 투여를 중단해야 하는 심각한 부작용이나 혈역학적 변화는 관찰되지 않았다.
결론: 뇌수술 후 중환자실에서 기계 환기를 받는 소아에서 덱스메데토미딘을 정맥 주입했을 때, 증가된 청소율을 보였으므로 목표 혈장 농도를 얻기 위해서는 투약 간격을 짧게 해야 한다.
