중증 만성질환 아동의 청소년기 형제의 심리사회적 경험

Abstract

중증 만성질환은 완치 불가능하거나 일상생활에 제한을 받아 특별한 도움이 필요한 질병으로, 이를 진단받은 아동의 수가 증가하고 있는 실정이다. 아동기에 만성질환을 진단받는 것은 장기적인 치료를 필요로 하는 질병의 특징으로 인해 가족에게 스트레스로 작용하며 구조적 변화를 일으킨다. 가족구성원들은 서로에게 영향을 주며 살아가지만, 중증 만성질환 아동으로 인해 건강한 형제가 겪는 심리사회적 경험에 대해서는 알려진 바가 많지 않다. 본 연구는 우리나라 중증 만성질환 아동의 청소년기 형제의 심리사회적 경험은 어떠한가?라는 연구 질문을 통해 청소년기 형제의 경험과 변화, 경험이 형제에게 주는 영향에 대해 심층적으로 이해하고자 하였다. 중증 만성질환 아동의 청소년기 형제의 심리사회적 경험을 알아보기 위하여 2020년 2월부터 2020년 4월까지 7명의 형제를 대상으로 면담을 진행하였다. 주로 참여자의 집 근처 또는 화상회의 앱을 이용하여 개인별 1회의 면담이 이루어졌으며 평균 소요 시간은 45분 가량이었다. 질적 주제 분석 방법을 이용하여 면담 내용을 분석하였고 총 80개의 코드 진술로부터 4개의 주제, 16개의 하위 주제가 도출되었다. 첫 번째 주제는 아픈 형제로 인한 복잡한 내 마음으로, 이는 형제의 질병 진단으로 인한 참여자의 심리적 경험 및 대처 방법과 관련된 내용이다. 두 번째 주제는 사람이 가장 먼저 맺는 관계인 부모 및 가족과의 관계에서 이뤄지는 사회적 경험과 관련된 주제로 가족과 함께 고난에 대처하며 가족의 일원으로 성장함으로 도출되었다. 세 번째 주제는 청소년에게 가족 이외의 중요한 관계이며 익숙하게 접하는 사회인 학교 및 또래와의 관계를 통한 사회적 경험을 다룬 학교에서는 아픈 형제의 존재에서 벗어나고 싶음으로 도출되었다. 네 번째 주제 익숙해졌으나 편하진 않은, 아픈 내 형제와 함께 하는 삶은 아픈 형제로 인한 참여자의 삶의 변화를 설명한 것으로 앞날에 대한 소망을 포함한다. 결론적으로 중증 만성질환 아동의 청소년기 형제의 심리사회적 경험은 형제의 질병을 마주하며 외로움이나 돌봄의 대물림에 대해 염려하였으며, 상대적으로 줄어든 부모님의 관심을 필요로 하면서도 자신을 둘러싼 가족과 또래를 통해 사회적 관계를 맺으며 성장하며 삶의 변화를 이뤄가는 것으로 볼 수 있었다. 본 연구는 중증 만성질환 아동의 청소년기 형제의 경험을 심층적으로 파악한 것에 의의를 둘 수 있다. 본 연구를 통해 중증 만성질환 아동이 있는 가족의 돌봄의 부담을 감소시키기 위해 가족 전체를 대상으로 한 중재 및 지원의 개발 및 확대 필요함을 확인할 수 있었다. 본 연구의 결과는 추후 간호 실무에서 만성질환 아동의 형제 경험에 대한 개념 측정 도구 개발 및 형제 지원 중재 개발이나 지역사회와 정부 차원의 프로그램 개발에 기초가 될 수 있을 것이다.

Severe chronic disease refers to either an incurable or a disease that requires special assist due to limitations in daily life, and the number of children diagnosed with severe chronic disease is increasing. Being diagnosed with severe chronic disease in childhood, it not only serves as a stress but also causes structural change to their family members because such diseases require long-term treatment. Although family members live by influencing each other, little has been known about the psychosocial experiences of healthy siblings of children with severe chronic disease. This study aims to thoroughly understand not only the experience and change in adolescent siblings of patient but also the impacts of their experiences on them through a study question, What is the psychosocial experience of adolescent siblings of children with severe chronic disease in Korea? In order to identify the psychosocial experiences of adolescent sibling of children with severe chronic disease, from February 2020 to April 2020, the interview was conducted with 7 siblings once for each. Each interview was conducted at a place near the house of a participant or through the video conferencing application, and each session lasted for about 45 minutes in average. The contents of interviews were analyzed using qualitative thematic analysis, 4 themes and 16 sub-themes were derived from a total of 80 code statements. The first theme My uneasy mind due to a sick sibling is related to psychological experience and coping strategies caused by the sibling diagnosed with a severe chronic disease. As sub-themes, there are Meet my siblings sickness gradually or in shock, Standing the harsh situation by being dependent on time, Feeling all alone despite having a sibling, and Concerns about handing down a care. The second theme Growing up as a member of the family, managing the hardships together with the family is associated with the social experience that occurs in relationships with parents and family, which is the first relationship having as a human being. The sub-themes include Feeling uncomfortable with my parents expectations for me, Still need attention from parents, A mom who has hardships, a dad who is taking a step back, Economic and physical difficulties faced by the family, Family unity even under difficult situation. The third theme Desiring to escape from concerns about the sick sibling in school deals with the social experience through a peer relationship, which is an important relationship for adolescents, in school that is a society intimately contacting the others besides family members. The sub-themes include Dont want to let others know about my sick sibling, Making friends aside from my sick sibling, Trust in a teacher who can understand my family. The fourth theme A life being together with my sick sibling, whom I am accustomed but not comfortable explains the changes in life of a participant caused by a sick sibling and also includes a hope with regard to the future. The sub-themes involved in this theme are Accepting a care for a sick sibling as a daily routine, My view to others has widened, Wish for happiness in the name of mediocrity, and Wish for independence to a place where doesnt need to care for a sick sibling. In conclusion, the psychosocial experiences of adolescent siblings of children with severe chronic disease appeared to be in connection with the concerns about loneliness or handing down a care when encountering the disease of a sibling. Furthermore, while requiring attention from parents that was relatively reduced, the adolescent siblings tend to show a change in life by establishing social relationships with their families and peers around them as they grow. This study has significance in that it had an in-depth understanding on the experience of adolescent siblings of the children with severe chronic disease. It was able to verify, through this study, that development and expansion of intervention and support for the whole family are necessary to reduce the burden of care for families having a child with severe chronic disease. The result of this study would be fundamental data for developing the conceptual measurement tools, sibling-assisted interventions, or the program of community level or government level for the experience of siblings of children with severe chronic disease in the future.
