The proposal of a new TNM classification of gastric cancer

Abstract

Purpose Gastric cancer with lymph node metastasis (LNM) more than 15 (N3b) was defined as stage IV until the 6th AJCC system. However, it has been reclassified as a localized disease (from stage IIb to IIIc) since 7th system. Meanwhile, stage IV with peritoneal washing cytology-only positive (CY1-only) patients had better overall survival than patients with overt distant metastasis with or without peritoneal washing cytology-positive. The aim of this study is to demonstrate that the survival of N3b is comparable to stage IV with CY1-only which is systemic disease and to propose a new TNM system interpreting N3b as an eligibility criteria for receiving more intensive chemotherapy regimens for systemic disease. Materials and Methods 1,662 patients who underwent gastric cancer surgery at Seoul National University Hospital from 2007 to 2013 were retrospectively analyzed. The 5-year survival rate (5YSR) and 3-year recurrence-free survival (RFS) were evaluated according to the 7th and 8th system, as well as a new classification based on lymph node metastasis (LNM); N0-2 (LNM<7), N3a (LNM 7-15), or N3b (LNM>15). Furthermore, subgroup analysis within N3b was conducted with a presumption of other cutoff values differentiating overall survival and recurrence-free survival. Results The survival of N3b(T1-2N3b; IIIb, T3-4N3b; IIIc) is comparable to that of CY1-only stage IV (log rank test, p = 0.728) and distinct from that of grossly stage IV (log rank test, p < 0.001). The survival of stage IIIc without N3b (T4bN3a) was also comparable to N3b and CY1-only stage IV. Most N3b patients had significantly shorter 3-year RFS and mean RFS than IIb-IIIc, as if N3b itself was a higher TNM stage. Meanwhile, subgroup analysis within N3b revealed a new cutoff value for overall survival at the level of 25 LNs. Conclusions In terms of survival, T4bN3a, N3b ≦ 25, and CY1-only stage IV is unified as stage IVa, and N3b >25 and grossly stage IV were defined as stage IVb. Even N3b is classified as localized disease in current staging system, the survival of N3b is comparable to either CY1-only stage IV or grossly stage IV, which are systemic disease. Therefore, we suggest N3b as an eligibility criteria for receiving more intensive chemotherapy regimens for systemic disease.

목적 림프절 전이가 15개 초과 (N3b)인 위암은 6차 AJCC 병기체계까지는 4기로 정의되었다. 그러나 이는 7차 병기체계부터 국소질환 (2b기에서 3c기)으로 재분류되었다. 한편 복막 세척 세포진 검사 단독 양성으로 나온 4기 환자 (CY1-only)는, 복막 세척 세포진 검사의 결과와 관계없이 육안적 전이가 있는 4기 환자에 비해 전체 생존율이 좋았다. 본 연구의 목적은 N3b 환자군의 생존율이 전신질환 (systemic disease)인 복막 세척 세포진 검사 단독 양성 4기 (CY1-only) 환자와 유사하다는 것을 입증하고, N3b를 전신질환에 사용하는 더욱 집중적인 화학요법에 대한 자격기준으로 해석하는 새로운 TNM 시스템을 제안하는 것이다. 자료와 방법 2007년부터 2013년까지 서울대병원에서 위암 수술을 받은 환자 1,662명을 후향적으로 분석했다. 5년 생존율 (5YSR)과 3년 무재발 생존율 (RFS)은 7차와 8차 병기체계 및 림프절 전이에 따라 N0-2 (림프절 전이 7개 미만), N3a (림프절 전이 7개 이상 15개 이하), N3b (림프절 전이 15개 초과)로 새롭게 나눈 분류체계에 따라 평가되었다. 또한, N3b 군 내에서 생존율과 무재발생존율을 유의하게 나누는 또다른 컷오프 값이 존재할 수 있음을 가정하고 subgroup 분석을 수행하였다. 결과 N3b 환자군 (T1-2N3b; IIIb, T3-4N3b; IIIc)의 생존율은 CY1-only 4기 (log rank test, p=0.728)에 필적하며, 육안적 전이가 확인된 4기 환자 (log rank test, p<0.001)와는 유의한 차이를 보였다. 3c기 환자군 중 N3b를 제외한 나머지 환자 (T4bN3a)의 생존율은 N3b와 CY1-only 4기 환자군에 필적했다. 대부분의 N3b 환자들은 N3b 자체가 마치 더 높은 TNM 병기인 것과 같은 결과를 보여 나머지 2b기~3c기보다 3년 RFS와 평균 RFS가 유의하게 짧았다. 한편, N3b 군 내에서 수행한 subgroup 분석 결과 림프절 전이 25개를 기준으로 생존율에 대한 새로운 컷오프 값이 나타났다. 결론 생존율 측면에서 T4bN3a, N3b 중 림프절 전이 25개 이하 (N3b ≦ 25), CY1-only 4기를 4a기로 통합하였고, N3b 중 림프절 전이 25개 초과 (N3b > 25) 및 육안적 4기는 4b기로 정의되었다. 현행 병기체계에서 N3b가 국소질환으로 분류되더라도 생존율은 전신질환인 CY-only 4기 혹은 육안적 4기에 필적하였다. 따라서 우리는 N3b를 전신질환에 적용하는 더욱 집중적인 화학요법을 사용하기 위한 자격기준으로 제안한다.
