Abstract
비알콜성 지방간질환(Nonalcoholic fatty liver diseases, NAFLD)은 간 내에 지방이 과도하게 축적되면서 발생하는 지방간을 시작으로 염증을 동반하는 비알콜성 지방간염 (Nonalcoholic steatohepatitis, NASH)과 간 섬유화가 나타나는 간경변증(Cirrhosis) 까지를 아우른다. 특히 비알콜성 지방간염은 비알콜성 지방간에 비하여 간경변증 혹은 간암 (Hepatocellular carcinoma, HCC) 으로까지 악화되는 가능성이 훨씬 높으며, 이로 인한 사망률도 지방간 환자에 비해 높다. 따라서 비알콜성 지방간염의 병리 기전 및 치료제 개발 연구가 활발하게 이루어지고 있다. MLL4는 포유류에서 히스톤 H3 단백질 Lysine-4 잔기의 메틸화를 유도하는 메틸기 전이효소로서 그룹 특이적 단백질과 함께 복합체를 이루어 작용한다. MLL4-복합체는 지방세포 형성 및 배아 발달 과정, 암 발생 및 억제 기전에 관여하는 것으로 알려져 있으며, 그 중 마우스의 간 내 지방증 형성에 있어서 중요한 후성유전학적 조절 기능을 가진다. 그러나 MLL4가 비알콜성 지방간염의 진행 과정에서 어떠한 역할을 하는가에 대해서는 아직 밝혀진 바가 없다. 비만으로 인한 간 내 산화적 환경은 Nuclear Factor-kappaB (NF-κB) 소단위체인 p65/RELA의 활성을 증가시키며, 이로 인해 T 세포의 침윤 작용과 비알콜성 지방간염 및 간 섬유화가 촉진된다. Methionine and choline deficient (MCD)식이를 8주간 급여한 야생형(wild type, WT) 마우스와 Mll4+/- 마우스를 비교한 결과, MCD 식이를 급여한 Mll4+/- 마우스가 WT 마우스에 비하여 간 내 지방증, 염증, 경화증이 모두 현저히 감소하였다. 또한 MLL4가 p65/RELA과 상호작용하여 NF-κB 신호전달체계가 활성화되었을 때 나타나는 염증성 인자의 발현을 조절하는 것을 확인하였다. 결론적으로, 본 연구는 MLL4가 p65/RELA의 전사활성조인자로써 비알콜성 지방간염의 진행을 심화시킬 수 있음을 규명하였으며, 이러한 조절 기작에 근거하여 MLL4-복합체는 항 비알콜성 지방간염 치료제 개발의 새로운 약물 표적으로의 가능성을 제공해 주었다.
Non-alcoholic fatty liver disease (NAFLD) is associated with excessive fat in the liver and non-alcoholic steatohepatitis (NASH) is a growing problem of liver diseases with liver inflammation. We have reported that MLL4, the histone H3 lysine 4 methyltransferase, is a critical epigenetic mediator of overnutrition-induced murine steatosis. However, it remains unknown whether MLL4 is also involved in the progression of NASH. Oxidative hepatic environment in obesity increases the activity of NF-κB subunit p65/RELA that promotes T cell infiltration, NASH, and fibrosis. We subjected wild-type and Mll4+/- mice to methionine and choline-deficient (MCD) diet feeding for 8weeks and found that MCD-fed Mll4+/- mice were resistant to hepatic steatosis, fibrosis, and inflammation compared with wild type mice. We also found that MLL4 interacts with p65 and regulates the expression of proinflammatory factors, induced by activation of NF-κB signaling pathway. Thus, our results reveal a new role of MLL4 in promoting NASH as a trans-coactivator of p65 and this regulatory axis can serve as a new therapeutic target of anti-NASH drug development.
