Abstract
Purpose: This study investigated the psycho-behavioral and biochemical factors moderating the effect of desvenlafaxine on hot flashes in the women with breast cancer patients taking tamoxifen and infers the treatment mechanism.

Methods: I utilized data from clinical trial which had conducted to determine whether desvenlafaxine reduces hot flashes in women with breast cancer taking tamoxifen and performed additional biochemical tests to achieve the aims mentioned above. Participants were randomized to desvenlafaxine 50mg, 100mg, and placebo groups and took the doses of desvenlafaxine and placebo for a total of 4 weeks. (the number of recruited participants: 57) To measure hot flash severity, which is primary endpoint, participants wrote daily a hot flash symptom diary for five weeks, including one week prior to drug administration and four weeks during dosing. I obtained Blood samples from participants before drug administration to measure plasma concentrations of serotonin receptors (5-HT2A), serotonin metabolites (5-hydroxyindoleacetic acid, 5-HIAA), and pro-inflammatory cytokines (IL-1β, IL-6, IL-8, TNF-α). Participants visited the hospital three times (week 0, week 1, and week 4 after study registration) and performed questionnaire on psycho-behavioral factors such as depression, anxiety, sleep quality, and chronotype. I analyzed the moderating effect of psycho-behavioral and biochemical factors on the improvement of hot flashes after administration through a linear mixed model.

Results: Among the psycho-behavioral factors, depression and chronotype had significant moderating roles (depression, p = 0.002; evening type, p = 0.045) on the effect of desvenlafaxine on hot flashes in the desvenlafaxine 50mg group. In the subgroup analyses, desvenlafaxine 50 mg showed a significant effect on hot flashes in the subgroups with depression or with intermediate/evening chronotype compared to the placebo group. All four pro-inflammatory cytokines had significant moderating roles on the effect of desvenlafaxine on hot flashes in the desvenlafaxine 50mg group (IL-1β, p = 0.010; IL-6, p < 0.001; IL-8, p = 0.007, TNF-α, p = 0.016). In the subgroup analyses, desvenlafaxine 50 mg had a significant effect on hot flashes in the subgroups with high plasma concentration of pro-inflammatory cytokines compared to the placebo group (IL-1β, p <0.001; IL-6, p <0.001; IL-8, p <0.001, TNF-α, p <0.001). In the subgroup with high plasma concentration of 5-HT2A receptor, both desvenlafaxine 50mg and 100mg showed a significant effect on hot flashes (p-values were all less than 0.001) compared to the placebo.

Conclusion: The current study identified psycho-behavioral and biochemical factors predicting the effect of desvenlafaxine on the treatment of hot flashes in the women with breast cancer patients taking tamoxifen and provided evidence for dose determination for those patients. Patients with depression or with evening chronotype or with high blood levels of pro-inflammatory cytokines could expect a significant improvement in hot flashes compare to the placebo group with desvenlafaxine 50 mg, which is less than the previously reported effective dose (desvenlafaxine 100mg). As the biochemical mechanism by which desvenlafaxine treats hot flashes, I suggested normalization of serotonin dysregulation in the pituitary thermoregulatory center and reduction of pro-inflammatory cytokines. Of the two mechanisms, I suggested that the former is the main mechanism and the latter is an auxiliary.
연구배경 및 목적: 안면홍조는 유방암 경험자의 삶의 질을 저하시키는 주요한 치료 부작용이다. 본 연구의 목적은 안면홍조를 호소하는 타목시펜을 복용중인 유방암 환자에서 데스벤라팍신 투약이 안면홍조 치료에 효과적인 대상의 정신행동 및 생화학적 인자의 특성을 확인하고, 데스벤라팍신이 안면홍조를 치료하는 생화학적 기전을 유추하는 것이다.

연구 방법: 본 연구에서는 타목시펜을 투약 중인 유방암 환자에서 데스벤라팍신이 안면홍조의 중증도를 낮추는 효과가 있는지 확인하기 위한 임상연구의 자료를 활용하고 추가적인 생화학적 검사를 수행하여 상기 목적의 분석과 고찰을 시행하였다. 임상시험을 위해 모집된 대상자는 데스벤라팍신 50mg, 100mg 및 위약군에 무작위 배정 되어 해당 용량의 데스벤라팍신 및 위약을 총 4주 동안 복용하였다. (모집 대상자 수: 57명) 대상자는 안면홍조 증상을 측정하기 위해 투약 전 1주와 투약 기간 4주를 포함한 5주 동안 매일 안면홍조 증상 일기를 작성하였다. 투약 전 대상자의 혈액을 채취하여 serotonin 수용체 (5-HT2A receptor) 및 serotonin 대사체 (5-hydroxyindoleacetic acid, 5-HIAA), pro-inflammatory cytokines (IL-1β, IL-6, IL-8, TNF-α)의 농도를 측정하였다. 대상자는 세 차례 (연구 등록 후 0주, 1주, 4주) 병원을 방문하여 우울, 불안, 수면의 질, 일주기 유형 등 정신행동 인자 대한 설문 평가를 수행하였다. 투약 중 안면홍조 호전 경과에 대한 각 정신행동 인자와 생화학적 인자의 조절효과를 선형 혼합 모형을 통해 분석하였다.

연구 결과: 정신행동 인자 중 우울과 일주기 유형이 데스벤라팍신 50mg군에서 안면홍조 호전에 유의한 조절 효과(depression, p = 0.002; evening type, p = 0.045)를 나타냈다. 우울이 높은 집단, 그리고 중간형 혹은 저녁형 일주기 유형의 집단에서 데스벤라팍신 50mg 투약 시에도 위약군에 비해 유의한 안면홍조 감소 효과가 있었다(depressed group, p < 0.001; intermediate type, p = 0.007; evening type, p < 0.001). Pro-inflammatory cytokines 4종류 모두 데스벤라팍신 50mg군에서 안면홍조 호전에 유의한 조절 효과(IL-1β, p = 0.010; IL-6, p < 0.001; IL-8, p = 0.007; TNF-α, p = 0.016)를 나타냈다. Pro-inflammatory cytokines의 혈중 농도가 높은 집단에서 데스벤라팍신 50mg 투약 시에도 위약군에 비해 유의한 안면홍조 호전을 보였다(IL-1β, p < 0.001; IL-6, p < 0.001; IL-8, p < 0.001; TNF-α, p < 0.001). 탐색적 분석 결과, 데스벤라팍신 100mg 투약군에서는 우울, 일주기 유형, 그리고 pro-inflammatory cytokines의 혈중 농도와 농도에 상관없이 위약군에 비해 유의한 안면홍조 감소 효과를 보였다. 5-HT2A 수용체의 혈중 농도가 높은 집단에서 데스벤라팍신 50mg 및 100mg 투약 시 위약군에 비해 유의한 안면홍조 감소 효과(p-value는 두 투약군 모두에서 0.001 미만)를 보였다.

결론: 본 연구는 타목시펜을 투약 중인 유방암 환자에게 발생한 안면홍조에 데스벤라팍신을 투약할 때 치료 효과를 예측하는 정신행동 및 생화학적 인자를 발굴하여 임상 현장에서 해당 환자에 대한 투약 용량을 결정하는 근거를 제공하였다. 우울을 호소하거나 저녁형 일주기 유형의 대상자, 또는 pro-inflammatory cytokine의 혈중 농도가 높은 대상자에서는 기존에 알려진 유효 용량인 데스벤라팍신 100mg보다 적은 데스벤라팍신 50mg 투약으로 위약군에 비해 유의한 안면홍조의 호전을 기대할 수 있었다. 데스벤라팍신이 안면홍조를 치료하는 생화학적 기전으로써 뇌하수체 체온조절 중추에서 serotonin dysregulation의 정상화와 pro-inflammatory cytokines의 저하를 제시하였다. 두 가지 기전 중 전자가 주된 치료 기전이며, 후자는 보조적인 치료 기전임을 제안하였다.
