노인 구강건강 활동 요인이 구강건강 상태와 구강건강 관련 삶의 질에 미치는 영향

Abstract

1. 목 적 이 연구 목적은 노인의 구강건강 활동 요인을 평가하여 구강건강 관련 삶의 질과 구강건강 상태에 미치는 영향을 조사하고, 이를 통해 노인의 구강건강증진 프로그램 개발을 위해 필요한 기초자료를 확보하는 데 있다.

2. 방 법 서울대학교 치의학대학원 기관윤리심의위원회 승인을 받아, 2019년 10월부터 2020년 3월까지 노인 요양 시설 이용 65세 이상 노인을 대상으로 구강검사와 전신 건강 상태를 조사하였고, 반구조화된 설문지를 이용하여 일대일 면담법으로 인구통계학적 요인, 구강건강 활동, 일반적인 건강 상태 및 구강건강 관련 삶의 질(Oral health impact profile-14, OHIP-14)에 대한 자료를 수집하였다. 구강건강 관련 활동 요인 분석을 위해 구강건강 관련 삶의 질과 구강건강 상태, 공변량 변수들을 활용하여 SPSS 26.0 (IBM Co., Armonk, NY, USA)로 통계분석을 유의수준 0.05로 하였다.

3. 결 과 노인의 구강건강 관련 삶의 질 점수 차이는 칫솔질 여부, 잔존 치아 수, 치주 질환 유무에 따라 나타났다(P<0.05). 구강건강 관련 삶의 질과 노인 인구통계학적 요인, 전신 건강 상태, 구강건강 습관 및 상태, 구강건강 관리 도움 필요로 다중 로지스틱 회귀분석을 수행한 결과, 구강건강 관련 삶의 질은 허벅지 둘레나 악력과 같은 일반적인 전신 건강 상태가 좋지 않은 대상자들과 칫솔질 시 도움이 필요한 대상자에서 낮게 나타났다(P<0.05). 구강건강 활동으로 구개설측면 칫솔질 유무, 칫솔질 빈도, 치간 칫솔 사용, 식이 패턴 및 구강건강 활동 의존성을 포함한 구강건강 활동은 탐침 시 출혈률과 치주 질환과 상관관계가 있었다. 로지스틱 회귀 분석 결과, 칫솔질 시 도움이 필요한 군에서 치주 질환 유병률이 14배 높게 나타났으며(P<0.05), 구개설측면을 잘 닦지 않는 군에서 치주 질환의 유병률이 약 18배로 높은 것으로 나타났다(P<0.05). 또한 탐침 시 출혈률도 치아의 구개설측면 칫솔질을 한 군에서 약 0.001배 낮게 나타났다(P<0.05). 결론적으로 노인의 구강건강 관련 삶의 질과 구강건강 상태는 구강건강 활동과 밀접한 관련이 있음을 도출해 낼 수 있었고, 노인 대상 구강보건서비스 교육 과정에서도 구강건강 활동 요인을 보다 체계적이고 심도 있게 교육해야 함을 도출할 수 있었다.

1. Purpose To understand the factors that influence the oral health-related quality of life of the elderly, we investigated the oral health status and oral health behavior of elderly individuals in a metropolitan city in Korea.

2. Methods Based on a structured questionnaire, we collected data on demographic factors, oral health behaviors, general health questions, and oral health-related quality of life (Oral Health Impact profile-14; OHIP-14). Data were collected from October 2019 to March 2020 after receiving approval from the institutional review board. Oral examinations and questionnaires were distributed to 150 elderly individuals using an elderly welfare center. The grip strength and upper arm and calf circumference were measured to assess the general health condition. Multiple logistic regression analysis was used to determine the correlation between oral health-related quality of life and other variables.

3. Results Differences in OHIP-14 scores were found depending on whether brushing was performed, the number of remaining teeth, and the presence of periodontal disease (P < 0.05). OHIP-14 and demographic factors in the elderly through multiple logistic regression analyses with general health, oral health habits and conditions, and oral health as needed to help with daily life. Correlation between OHIP-14 and general health variables, such as thigh circumference and grip strength, revealed a low oral health-related quality of life in participants who needed help during brushing (P < 0.05). Oral health activities including lingual and palatal site toothbrushing, frequency of toothbrushing, use of an interdental toothbrush, dietary patterns, and activity dependence correlated with bleeding on probing (BOP) and periodontitis. High BOP was significantly associated with brushing the lingual and palatal surfaces of teeth. After adjusting for covariates, the prevalence of periodontitis was lower in the group where the lingual and palatal surfaces of the teeth were partially or wholly cleaned than in the group without such cleaning. These results confirmed the association of OHIP-14 and oral health and habits of elderly individuals using welfare facilities for the elderly with socioeconomic status, general health, and oral health, and each variable affects the relationship. Further analysis and additional epidemiological studies are needed to understand the real inter-relationships.
