응급중환자실 간호사의 손상된 신체 간호 경험

Abstract

중환자실 환자 대부분은 중증도가 높고 의식이 저하되거나 약물에 의해 진정되어 있어 집중 간호가 필요하다. 중환자실에서는 환자와 관련된 섭생, 투약, 모니터링, 배설 등의 모든 업무를 담당 간호사가 일임하므로 간호사는 환자 신체와의 긴밀한 접촉을 피할 수 없다. 중환자실에 입원한 환자의 신체는 사고 및 질병으로 손상되거나, 수술 및 시술로 치료하는 과정에서 일시적 또는 영구적으로 변형이 발생한다. 이러한 환자의 손상된 신체를 간호하는 경험을 총체적으로 이해하는 것을 목적으로 하였다. 연구 설계는 보고, 듣고, 느끼는 지각적 활동들을 총체적, 복합적으로 이해해야 세계를 이해할 수 있다는 메를로-퐁티(Merleau-Ponty)의 현상학적 질적 연구을 이용하였다. 연구의 대상자는 2021년 8월부터 10월까지 서울 시내에 소재한 상급종합병원의 응급전용 중환자실 근무 경력 1개월 이상이며, 손상된 신체 간호 경험이 있는 간호사이며, 총 18명이었다. 연구 참여를 동의한 자로 반 구조화된 질문으로 면담을 실시하였다. 각 면담은 자료가 포화될 때까지 시행하였으며, 평균 36분 정도 소요되었다. 모든 면담은 녹음 하여 그대로 필사하였다. 연구 결과, 총 네 개의 주제와 여덟 개의 하위 주제가 도출되었다. 첫번째 주제는 심하게 손상된 신체를 간호하게 됨으로 손상된 환자의 신체를 만나기 전의 경험을 다루었다. 두 번째 주제는 환자의 몸을 보고 난 후 감정보다는 이성이 기능함.으로 환자를 대면했을 때를 다룬 내용이다. 세 번째 주제는 손상된 신체를 나아지게 하려고 고군분투함. 네 번째 주제는 손상된 신체 너머 환자의 삶을 만나게 됨으로, 손상된 신체를 나아지게 하기 위해 최선을 다하면서 환자의 과거와 현재를 만나게 되어 이를 통해 자신의 의식 세계가 확장되는 경험을 포괄하고 있다. 결론적으로, 환자의 손상된 몸에서 환자의 과거와 현재를 만나고, 그 한 사람을 통해 의식의 확장을 경험함으로 표현될 수 있었다. 본 연구 결과를 통해 중환자실 간호사가 손상된 신체 간호 경험에 대한 내용을 파악하여 향후 신체 간호와 관련된 간호사의 전문성에 대한 연구의 필요성을 제시했다는 점에서 큰 의의가 있다고 하겠다.

Most patients in the intensive care unit have high severity, decreased consciousness, or calmed down by drugs, requiring intensive care. In the intensive care unit, the nurse in charge of all tasks such as regulation, medication, monitoring, and excretion related to the patient is entrusted, so the nurse cannot avoid close contact with the patient's body. The body of a patient admitted to the intensive care unit is damaged by accidents and diseases, or temporarily or permanently deform during treatment with surgery and procedures. The purpose of this study was to understand the experience of nursing these patients' damaged bodies comprehensively. The research design used Merleau-Ponty's phenomenological and qualitative study that requires comprehensive and complex understanding of perceptual activities to see, hear, and feel the world. The subjects of the study were nurses with more than one month of experience working in the emergency intensive care unit at a senior general hospital located in downtown Seoul from August to October 2021, and with damaged physical nursing experience, a total of 18 people. Interviews were conducted with semi-structured questions that agreed to participate in the study. Each interview was conducted until the data were saturated, and it took an average of 36 minutes. All interviews were recorded and transcribed as they were. As a result of the study, a total of four topics and eight sub-themes were derived. The first topic was 'Nursing a severely damaged body', which dealt with the experience before meeting the damaged patient's body. The second topic deals with the case of confronting a patient by saying, "After seeing the patient's body, reason functions rather than emotion." The third theme is "Struggling to improve the damaged body," and the fourth theme is "Meeting the patient's life beyond the damaged body," which encompasses the experience of expanding the patient's past and present while doing its best to improve the damaged body. In conclusion, it could be expressed as "meeting the patient's past and present in the patient's damaged body and experiencing the expansion of consciousness through that one person." Through the results of this study, it is of great significance in that the intensive care unit nurse identified the contents of the damaged physical nursing experience and presented the necessity of research on the professionalism of nurses related to physical nursing in the future.
