코로나19 유행 시기 흡연 행태와 마스크 미착용의 연관성

Abstract

연구배경: 코로나19를 비롯하여 호흡기 감염병 유행이 반복되는 상황에서 위험요인으로 작용하는 흡연에 대해 알아볼 필요가 있다. 특히 코로나19 바이러스 전파 차단을 위한 방역 정책으로 실시되었던 마스크 착용은 흡연 시 그 행태적 특성으로 실천되기 어려운 점이 존재한다. 이에 본 연구에서는 코로나19 유행 시기 흡연 행태와 마스크 미착용과의 연관성을 확인하고자 한다.

연구방법: 2020년 지역사회건강조사에 참여한 한국 성인 208,618명의 자료를 이용하였다. 독립변수로는 인구통계학적 특성과 흡연 상태(여부), 담배의 종류, 흡연 변화, 금연 시도 여부 등의 흡연 행태 관련 변수, 그리고 코로나19 관련 염려가 사용되었다. 마스크 미착용과의 연관성을 알아보기 위하여 로지스틱 회귀분석을 이용하였으며 흡연 행태와 코로나19 관련 염려와의 상호작용도 함께 분석되었다.

연구결과: 흡연 상태(여부)와 담배 종류, 흡연 변화, 금연 시도는 모두 마스크 미착용과 연관성이 있었다. 비흡연자 대비 일반담배(궐련) 사용자의 오즈비(OR)는 1.91(95%CI [1.61, 2.26]), 과거흡연자의 오즈비는 1.33(95%CI [1.12, 1.58])이었다. 일반담배(궐련) 사용자는 NCTNPs(신종담배) 사용자에 비해 마스크 미착용 odds가 47% 높았다(OR=1.47, 95%CI [1.07, 2.03]). 또한 코로나19 유행 기간 흡연에 변화가 없는 경우 대비 흡연이 감소한 경우의 오즈비는 0.60이었다(95%CI [0.42, 0.84]). 금연 시도는 있었을 때 대비 없었을 때의 오즈비가 crude model에서 1.23이었다(95%CI [1.02, 1.49]). 마스크 미착용에 대해 흡연 상태(여부)와 염려는 대부분 상호작용이 있었고(p-value <0.05) 담배 종류와 염려 간에는 상호작용이 없었다(p-value >0.05).

결론: 일반담배(궐련) 사용자와 과거흡연자는 비흡연자 대비 마스크 미착용과 연관성이 있었고 흡연의 감소는 마스크 미착용과 연관성이 낮았다. 일반담배(궐련) 사용자는 NCTNPs(신종담배) 사용자보다 마스크 미착용과 관련이 있었고, 금연 시도는 마스크 미착용과 연관성이 낮았다. 또한 흡연 상태와 코로나19 관련 염려는 마스크 미착용에 대해 상호작용이 있어, 염려가 완화되는 시기의 마스크 착용은 더욱더 관찰되고 독려될 필요가 있다. 이를 통해 본 연구는 호흡기 감염병 유행 상황에서 금연 지원 및 흡연 예방사업, 차별화된 방역 정책 전략의 필요성을 제고하고자 한다.

BACKGROUND: It is necessary to investigate smoking which acts as a risk factor for COVID-19 infection in pandemic period. Especially, mask wearing is difficult to practice when smoking due to the behavioral feature. Therefore, this study aimed to examine the association between smoking behaviors and non-compliance with mask wearing in COVID-19 pandemic.

METHODS: Data analysis was conducted using the sample of 208,618 Korean adults who participated in the 2020 Korea Community Health Survey. As the independent variable, Socio-demographic characteristics, fear related to COVID-19, and smoking behaviors were used. Logistic regression was performed to analysis the association between these variables and non-compliance with mask wearing. Furthermore, the interactions between smoking behaviors and fears related to COVID-19 were also analyzed. RESULTS: Smoking status, changes in smoking, tobacco types, and smoking cessation trial were all associated with not wearing a mask. Odds ratio(OR) of cigarette users was 1.91(95%CI [1.61, 0.62]), and OR of former smokers was 1.33(95%CI [1.12, 1.58]) contrary to never smokers. Odds were 47% bigger for cigarette users than NCTNP users(OR=1.47, 95%CI [1.07, 2.03]). Moreover, when smoking decreased during the COVID-19 pandemic, odds ratio was 0.60 compared to no change(OR= 0.60, 95%CI [0.42, 0.84]). When smoking cessation was not tried, the odds ratio was 1.23 in crude model(95%CI [1.02, 1.49]). For not wearing a mask, there were mostly interactions between smoking status and fears(p-value <0.05) and there were no interactions between tobacco types and fears(p-value >0.05).

CONCLUSIONS: Cigarette users and former smokers were less likely to wear masks than never smokers. The reduction in smoking was negatively associated with not wearing a mask. Cigarette users were associated with non-compliance with mask wearing than NCTNP(Non-Cigarette Tobacco or Nicotine Products) users, and smoking cessation trial had a association with mask wearing. In addition, there were interactions between smoking status and COVID-19-related fears about not wearing a mask, so it is necessary to observe and encourage wearing a mask when the fears are alleviated. Through these findings, this study aims to raise the need for smoking cessation support, smoking prevention projects, and differentiated quarantine policy strategies in the midst of an epidemic of respiratory infections.
