Abstract
배경: 휘발성 유기화합물은 탄소를 기반으로 하며 상온에서도 쉽게 증발하는 다수의 물질을 아울러 이르는 용어로 일상생활에서는 페인트, 살충제, 화장품 등을 통해 쉽게 노출될 수 있다. 휘발성 유기화합물은 발암성을 비롯하여 많은 건강 위해를 초래함이 밝혀졌지만, 내분비계 교란 물질로서의 연구는 많이 시행되지 않았다. 또한 휘발성 유기화합물이 칼슘 농도에 영향을 미친다고 보고된 바는 있지만, 칼슘의 주요 조절 인자인 부갑상선 호르몬과의 관계에 대한 연구는 찾아볼 수 없었다. 따라서 본 연구는 휘발성 유기화합물과 부갑상선 호르몬 간의 관련성을 확인하고자 시행되었다.

방법: 혈액에서 측정한 휘발성 유기화합물 중 벤젠, 톨루엔, m-/p- 자일렌을 노출 변수로 사용하였고, 혈액에서 측정한 부갑상선 호르몬을 결과 변수로 사용하였다. 미국의 일반 인구를 대상으로 시행한 국민 건강 영양 조사 (National Health And Nutrition Examination Survey)의 2003년-2004년, 2005년-2006년 자료를 이용하였고 벤젠의 경우 1,734명, 톨루엔의 경우 2,976명, m-/p- 자일렌의 경우 3,104명이 연구집단에 포함되었다. 벤젠, 톨루엔, m-/p- 자일렌과 부갑상선 호르몬의 관련성을 확인하기 위해 선형 회귀분석과 로지스틱 회귀분석을 시행하였고, 성별, 연령, 인종, 비만도, 음주 상태, 흡연 상태, 연간 가구 소득, 신체 활동, 혈청 비타민 D, 혈청 칼슘으로 보정하였다. 그리고 성별, 비만도, 신장 기능에 따라 휘발성 유기화합물과 부갑상선 호르몬 사이의 관련성이 달라지는지 평가하기 위해 층화 분석을 시행하였다.

결과: 교란 변수들로 보정한 선형 회귀분석을 통해 로그 변환된 휘발성 유기화합물과 로그 변환된 부갑상선 호르몬 사이에는 음의 상관관계가 있음을 알 수 있었다. 벤젠, 톨루엔, m-/p- 자일렌의 회귀계수±표준오차는 각각 -0.072±0.011 (p<.0001), -0.022±0.010 (p=0.0359), -0.035±0.014 (p=0.0159)으로 유의미했다. 하지만 로지스틱 회귀분석을 통해 산출된 벤젠, 톨루엔, m-/p- 자일렌의 오즈비 (95% 신뢰구간)는 각각 1.364 (1.065-1.747), 1.080 (0.807-1.445), 1.318 (0.921-1.886)로 벤젠에서만 유의한 결과가 도출되었다. 성별, 비만도, 신장 기능에 따라 시행한 층화 분석에서 남자, 신장 기능이 좋은 군에서는 음의 상관관계가 대부분 유의하게 산출되었으나 그 외에서는 유의성이 소실되거나 일관성 있게 나타나지 않았다.

결론: 휘발성 유기화합물 중 벤젠, 톨루엔, m-/p- 자일렌은 부갑상선 호르몬의 감소와 관련이 있었으며, 이는 남성, 신장 기능이 좋은 군에서 더 두드러지게 나타났다. 또한 대다수의 연구 대상이 참고 범위에 속하는 혈중 휘발성 유기화합물 농도를 갖고 있었으므로 이는 노출 기준 이하의 휘발성 유기화합물 또한 부갑상선 호르몬 농도의 이상과 연관이 있을 수 있음을 시사한다.
Introduction: Volatile organic compounds (VOCs) refer to various carbon-based chemicals with high vapor pressure. Exposure to VOCs occurs through breathing fumes from paint, insecticide, cosmetics, and so on. Although it has been documented that VOCs might cause a lot of harmful effects on human health, there were only a few reports regarding VOCs as an endocrine disruptor. Though a few studies reported the effects of VOCs on calcium concentration, no study reported whether VOCs might affect parathyroid hormone, which is the primary regulator of calcium. Thus, we investigated the association between VOCs and parathyroid hormone.

Methods: Among VOCs measured in serum, benzene, toluene, and m-/p- xylene were selected as exposure variables. The outcome variable was serum parathyroid hormone. Data were derived from the National Health And Nutrition Examination Survey 2003-2004 and 2005-2006. The numbers of study populations for the three VOCs, namely benzene, toluene, and m-/p- xylene, were 1,734, 2,976, and 3,104, respectively. Linear and logistic regressions were conducted to assess the association between the VOCs and parathyroid hormone after adjusting for gender, age, race/ethnicity, body mass index, alcohol consumption status, smoking status, annual family income, physical activity, serum vitamin D, and serum calcium. Stratified analyses were also done according to gender, body mass index, and kidney function.

Results: After controlling for the covariates, results of linear regression revealed negative associations between the log-transformed VOCs and log-transformed parathyroid hormone which were statistically significant, where beta±standard error for benzene, toluene, and m-/p- xylene were -0.072±0.011 (p<.0001), -0.022±0.010 (p=0.0359), -0.035±0.014 (p=0.0159), respectively. However, logistic regression showed that only benzene was significantly associated with hypoparathyroidism (odds ratio:1.364, 95% CI: 1.065-1.747); the results of others were not statistically significant. When the regressions were conducted in the same manner except for the stratification, most of the significance remained unchanged for male and participants with good kidney function.

Conclusion: The levels of benzene, toluene, and m-/p- xylene were negatively associated with the level of parathyroid hormone and the significance was prominent in male and participants with good kidney function. Also, for most of the study populations, as the serum concentrations of VOCs were in the reference range, this study suggests that exposure to VOCs might be harmful to parathyroid gland even if the exposure was below the exposure limit.
