Background: Sunken upper eyelids, characterized by hollowing in the upper orbital region, can contribute to an aged or fatigued appearance. We aim to report on the surgical technique and its effects, involving the release of the arcus marginalis of the upper eyelid and the precise distribution of orbital fat.

Methods: From December 2021 to April 2024, a total of 102 eyelids from 51 patients who underwent surgical correction for sunken upper eyelids, utilizing the upper arcus marginalis release and precision fat distribution technique, were included in this study. Preoperative and postoperative sunken depths were measured and statistically analyzed. Aesthetic satisfaction was assessed through patient questionnaires. After the surgical method was established, analysis was conducted on a separate group of 9 patients for whom 3-dimensional photogrammetric and ultrasonographic data were collected.

Results: Preoperative and postoperative sunken depths measured 9.1 ± 2.3 mm and 5.8 ± 2.4 mm, respectively. The mean improvement was 3.3 mm, a change of statistical significance. In the multi-analysis using 3-dimensional imaging, improvements were observed on average in both depth and volume. Aesthetic outcomes and patient satisfaction yielded favorable results. No major complications were observed during the follow-up period.

Conclusion: The upper arcus marginalis release and orbital fat distribution technique demonstrated favorable outcomes in correcting sunken upper eyelids. This procedure ensures stable placement of orbital fat at the deepest sunken point, resulting in aesthetically pleasing and enduring results. This technique serves as a valuable alternative for patients with moderate to severe sunken eyelids.

배경: 상안와부위의 꺼짐으로 특징되는 함몰안검으로 인해 노화된 외형이 더 부각된다. 이 연구에서는 한계궁 박리 및 지방재배치를 이용한 함몰안검 수술적 교정과 그 효과에 대해서 분석하고자 한다.

방법: 2021년 12월부터 2024년 4월까지 총 51명에서 102개의 안검을 한계궁 박리 및 지방재배치를 통한 교정을 시행하였다. 수술 전과 수술 후의 함몰 깊이를 측정하고 통계적 분석을 시행하였다. 환자 설문조사를 통해 미적만족도를 조사하였다. 수술적 방법을 수립한 후에 별도의 9명의 환자에게 3차원 사진측정 및 초음파 분석을 시행하였다.

결과: 수술 전 함몰 깊이는 평균 9.1 ± 2.3 mm 이었고 수술 후 함몰 깊이는 5.8 ± 2.4 mm로 측정되었다. 평균 개선치는 3.3mm였으며 통계적으로 유의하였다. 3차원 사진측정을 통한 다분석을 시행하였고 깊이와 부피에 있어서 모두 개선되었다. 미적결과 및 환자만족도도 유의한 결과를 얻었다. 추적관찰기간 동안 특별한 합병증은 관찰되지 않았다.

결론: 한계궁 박리 및 지방재배치를 통한 수술적방법은 함몰안검의 교정에 있어서 만족할만한 결과를 보였다. 이 방법은 함몰의 가장 깊은 부위에 안와지방을 견고하게 배치함으로써 오래 지속가능한 만족할만한 결과를 얻었다. 이 기법은 중등도 및 중증 함몰안검환자들에 있어 유용한 대안으로 역할을 할 것이다.
