의료보험통합일원화 정책결정과정 분석 -권력자원 이론의 확대적용을 중심으로-

Abstract

본 논문은 우리나라의 의료보험통합일원화 정책결정과정(이하 의보통합)의 분석 틀로서 권력자원 이론을 확대 적용함으로써, 의보통합은 국가 권력자원과 노동운동 권력자원의 보상적 상호관계에 따른 상호교환의 형태로서 이루어졌음을 분석하고 있다. 권력자원 이론에 의하면, 두 집단이 서로 상대방의 자원이 필요하며 승리에 대한 확신이 약한 경우에 권력자원의 보상적 교환이 이루어진다고 보고 있다. 먼저 국가는 정치적 권력기반의 약화와 경제위기를 극복하고 노동통제의 새로운 전략을 달성하기 위해 보상적 교환을 선택하였다. 두 번째로 노동운동은 사회개혁투쟁의 일환으로서 의보통합을 추진함으로써 국민적 지지기반을 확보하고 합법화를 토한 내부 투쟁역량의 강화라는 보상적 교환을 선택하였다. 의보통합은 이러한 국가와 노동운동 권력자원의 보상관계를 통해서 설명될 수 있다.

본 논문의 이러한 분석은 2002년 1월부터 시행하기로 했던 의료보험의 재정통합이 다시금 연기되게 된 과정을 분석하는데도 유용하게 적용될 수 있을 것이다. 또한 본 논문은 권력자원론의 권력균형 개념에 보다 초점을 맞춤으로써 노동운동의 힘이 약한 우리나라의 사회복지 제도 변화를 설명하는데도 권력자원론이 유용하게 적용될 수 있음을 보여주고 있다는 점에서 의의가 있다.

This paper is investigates the policy making process of the integrated National Health Insurance(NHI) administrative management system by applying Power Resource Theory(PRT). It argues that the integrated NHI is the result of the exchange between the state and the labour movement.

According to the PRT, interaction where both parties activate pressure resources constitutes manifest conflict, whereas interaction involving the mutual use of rewards traditionally is called eschange. First, the state activated reward resources, for passing the financial crisis and the weak political power. Second, labor movement activated reward resources, for strengthening the support of the people and the internal power of the labor movement.

The significance of this paper are; first, with the emphasis of the actors, this paper overcomes the functional and structural approach for the social welfare institution development. Second, with focusing the concept of power balances, this paper has a good point when PRT is applied to the weak labor movement countires.
