- Abstract
- For the purpose of reviewing clinical charcteraistics
of mediastinal tumors, 80 patients treated in the 20
year period. 1958 through 1977, were studied. Cancers
of unidentified primary site of origin, nonneoplastic
lesions and the cases with clinical diagnosis only were
excluded from the study. There were 45 males and 35
females, and their ages ranged from 2 months to 65
years, with the average age of 32 years. Twenty per
cent of patients were younger than 15 years of age.
The most frequently encountered tumors were tcratodermoids
(30.0%), followed by neurogenic tumors,
thymic tumors, malignant lymphomas, benign cysts
and miscellaneous tumors in order of decreasing
frequency. Seventy per cent of the 80 mediastinal
tumors were benign and 30 per cent were malignant.
Thoracic symptoms were common when they present.
and 18.8 per cent of the patients were asymptomatic
at admission. All patients exclusive of 11 with malimalignant
lymphoma had thoracotomy, and the tumors
were removed in 66 cases. There were 2 postoperative
deaths with 2.9 per cent of operative mortality rate,
and 2 other deaths among patients with lymphoma who
were not operated upon, showing overall hospital
mortality rate of 5.0 per cent. Postoperative follow-ups
were possible in 41 cases of varying periods, and
there were 4 late deaths, 3 of whom were the patients
with malignant tumors not resected. Two cases of
benign tumors had reoperation for the tumor recurrences
with good results, while 4 patients of malignant
tumors revealed progress of diseases in the follow-up
period.
Histopathologic characteristics and complex difficulties
in preoperative as well as tissue diagnoses of the
specific types of the mediastinal tumors were discussed
along with the stresses on the important roles of
thoracotomy for its diagnostic and therapeutic merits.
