심장이식 수혜자들의 급성기 이후 자가간호 불이행

Abstract

심장이식 후 주로 발생하는 합병증에는 급∙만성거부반응, graft failure, 심장 동종이식편 혈관병증(cardiac allograft vasculopathy, CAV), 감염, 신부전, 악성종양, 고혈압, 당뇨, 골절, 이상지질혈증 등이 있다. 이렇듯 심장이식 후 생존율이 증가함에 따라 발생할 수 있는 다양한 합병증을 예방하고 관리하기 위해서 심장이식 수혜자들은 면역억제제를 포함한 약물복용, 운동, 금연, 과음 자제, 자외선 차단, 식이 준수, 외래방문 등의 자가간호가 필수적이다. 그런데도 면역억제제 복용과 추천된 생활양식의 불이행과 이로 인한 이식거부반응의 위험성 등이 보고된다. 이에 따라 심장이식 수혜자의 자가간호 이행도 증진을 위한 중재의 필요성이 대두되었고, 중재의 근거가 되는 자가간호 불이행과 그 영향 요인에 대한 파악이 요구되었다. 따라서, 본 연구는 급성기 이후 심장이식 수혜자의 면역억제제 복용과 비약물적 자가간호의 불이행 정도를 파악하고, 면역억제제 복용 불이행에 영향을 미치는 요인을 탐색하기 위해 시행되었다. 본 연구는 서술적 상관관계 연구이다. 서울 시내 소재한 종합병원 심장이식 수혜자 129명을 대상으로 구조화된 설문지를 활용하여 자료를 수집하였다. 자료는 SPSS version 28.0 프로그램을 이용하여 서술통계, T-Test, Chi-square test, Fisher exact test, multiple logistic regression으로 분석하였다. 연구 결과 대상자의 면역억제제 복용은 복용 차원의 불이행률 24.0%, 시간 차원의 불이행률 34.1%로 전반적인 면역억제제 복용 불이행률 44.2%이었다. 그리고 비약물적 자가간호 행위의 불이행률은 자외선 차단의 경우 92.2%, 운동의 경우 72.1%, 식이 준수는 56%, 정기 암 검진은 37.2%, 독감 예방접종은 15.5%, 치과 치료 시 예방적 항생제 복용은 13.2%, 금연은 5.4%, 과음 자제는 4.7%, 외래방문은 0%로 조사되었다. 면역억제제 복용 불이행은 학력과(p=0.038) 직업 유무(p=0.009), 우울 정도에 따라(p=0.016) 차이가 있었다. 다중 로지스틱 회귀분석 결과 대상자의 면역억제제 복용 불이행에 영향을 미치는 변수는 직업 유무(OR=2.57, p=0.030)와 우울 정도(OR=1.11, p=0.028)이었고, 이들 회귀모형의 설명력은 19.8%였으며, 통계적으로 유의한 것으로 확인되었다(p=0.024). 결론적으로 심장이식 수혜자의 면역억제제 복용 불이행에 학력, 직업 유무, 우울 정도가 유의한 영향을 미치는 것으로 확인되었다. 본 연구 결과는 심장이식 수혜자의 자가간호 이행 증진을 위한 중재 개발에 근거가 될 수 있을 것으로 기대된다.

Backgroud: Complications that occur mainly after heart transplantation include acute and chronic rejection, graft failure, cardiac allograft vasculopathy(CAV), infection, renal failure, malignant tumors, hypertension, diabetes, fractures and dyslipidemia. In order to prevent and manage various complications that can occur as the survival rate increases after heart transplantation, it is essential for heart transplant recipients to take drugs including immunosuppressants, exercise, stop smoking, do not drink, block UV, follow therapeutic diet compliance, and visit outpatient clinic. Nevertheless, The risk of nonadherence with immunosuppressive medication and the recommended lifestyle resulting transplant rejection is reported. Accordingly, the need for intervention to promote the self-care of heart transplant recipients emerged, and it was required to understand the self-care nonadherence and influencing factors. Purpose: This study aimed to identify the nonadherence of immunosuppressive medication and non-pharmacologic self-care of post-acute heart transplant recipients and to explore the influencing factors. Methods: A cross-sectional design was employed to survey 129 heart transplant recipients in Korea using survey questionnaires. Results: Overall nonadherence rate of immunosuppressive medication was 44.2%. Nonadherence rate with the taking dimension was 24.0% and nonadherence rate with time dimension was 34.1%. In addition, nonadherence rate of non-pharmacologic self-care was 92.2% for sun protection, 72.1% for exercise, 56% for therapeutic diet, 5.4% for smoking cessation, 4.7% for alcohol consumption, and 0% for outpatient clinic visits. The lower education level(p=0.038), having no job(p=0.009) and the lower depressive symptoms(p=0.016) were related to the lower nonadherence of immunosuppressive medication. A multiple logistic regression analysis showed having no job (OR=2.57, p=0.030) and lower depression (OR=111, p=0.028) were significantly predicted the nonadherence of immunosuppressive medication. Conclusion: This study highlights the importance of self-care adherence. This could be a foundational body of research to act as evidence to develop self-care promoting intervention program for heart transplant recipients. Findings from this study will inform future interventions, to improve self-care in heart transplant recipients.
