역전이를 활용한 즉시성 개입 결정의 고려 요인

Abstract

This study focused on the counselor's immediacy during countertransference. Immediacy during countertransference means that the counselor immediately discloses his/her here and now internal and external experiences in the therapeutic relationship and counseling process to the client for therapeutic purposes. Through this, we can reach the core dynamic of the client, break the stuck in counseling process, and help the client reach self-reflection, awareness, and emotional integration(Ginot, 2007; Hajeong, 2016; Jurist, 2008; Seligman, 2007). However, although immediacy during countertransference is known to be very effective, it can negatively affect therapy outcome if used improperly, and is also an intervention that is particularly difficult for novice counselors to use. Therefore, in order to help counselors to effectively use immediacy during countertransference, it is useful to examine procedural knowledge and structural knowledge which experienced counselors have about using immediacy during countertransference. In other words, it is needed to know what experienced counselors consider when they decide to use immediacy during countertransference and how their knowledges are linked and organized. In this study, ideas about the factors that experienced counselors consider when deciding to use immediacy during countertransference were examined by concept mapping. Participants were experienced counselors with more than 10 years of counseling experience and a high understanding of the concept and utilization of countertransference from the psychodynamic perspective. Through individual interviews with 13 participants, research team generated 532 statements and synthesized them into 78 statements through keyword coding. After receiving ratings of the relevance of the 78 statements and reflecting the feedback, 70 representative statements were finally confirmed. Afterwards, similarity classification and importance rating were performed by 12 interviewees, and multidimensional scale analysis was conducted based on the similarity classification results of the participants to recognize the structure of experienced counselors' perception about decision to use immediacy during countertransference. As a result, three-dimensions solution was adopted. Dimension 1 (X-axis) describes whom to consider and is named by Considering the counselor factors - Considering the client factors. Dimension 2 (Y-axis) describes the judgment for readiness and is named by Judging cognitive readiness - Judging emotional readiness. Dimension 3 (Z-axis) describes the purpose of intervention and is named Pursuing client's insight - Pursuing sincere therapeutic relationship. In other words, experienced counselors decide whether to use immediacy during countertransference through active self-reflection. At this time, experienced counselor examines the factors of the counselor or client, the two subjects of counseling (Dimension 1), and considers whether the two subjects are cognitively or emotionally prepared enough to engage in immediacy using countertransference (Dimension 2), and whether immediacy is appropriate to achieve his purpose to provide insight to client or to provide sincere therapeutic relationship (Dimension 3). In addition, by the hierarchical clustering analysis, using Ward's method, 7 cluster categories were found to be the most appropriate for the meaningful interpretation of clusters. 7 clusters are Cluster 1. Counselor can hold and accept countertransference, Cluster 2. Pursuing sincere therapeutic relationship. Cluster 3. Thinking immediacy using countertransference is useful, Cluster 4. Intervention purposes and directions are established, Cluster 5. Understanding the client through countertransference, Cluster 6. Client can hold and accept his emotion, Cluster 7. Client trusts on therapeutic relationship. As the experienced counselors rated the relative importance of representative statements, It was recognized as the most important factor that countertransference should be identified as a useful material for the client, and that counselor should be able to contain and hold his countertransference emotion while maintaining the focus on the client without being overwhelmed by the countertransference. On the other hand, whether the client trusts the therapeutic relationship and whether the counselor pursue an sincere therapeutic relationship were recognized as relatively the least important factors. This study is meaningful in that it provided an understanding of various factors and contexts that counselors should consider in order to effectively use immediacy during countertransference. How the results can be used to counselor education are discussed in detail in the discussion chapter. It is hoped that this study may contribute to the advancement of the concept mapping methodology in the field of counseling, by conducting very strict procedures to obtain reliability and validity in the data collection and data analysis process and providing the detailed method to implement an 3D interactive visualization for three-dimensional concept map.

본 연구에서는 상담사의 즉시성(immediacy) 개입의 여러 방식 중에서 역전이를 활용한 즉시성 개입에 주목하였다. 역전이를 활용한 즉시성 개입이란 상담사가 지금-여기에서의 상담관계 및 상담과정에 대한 상담사의 내적·외적 경험을 치료적 목적에서 내담자에게 즉시적으로 개방하는 것을 의미한다. 상담사는 이러한 개입을 통해 내담자의 핵심에 다가갈 수 있고, 상담이 꽉 막힌 교착상태를 풀어갈 수 있으며, 내담자가 자기성찰, 알아차림, 정서적 통합에 이를 수 있도록 도울 수 있다(하정, 2016; Ginot, 2007; Jurist, 2008; Seligman, 2007). 그러나 역전이를 활용한 즉시성 개입은 매우 효과적이면서도 잘못 사용되면 상담성과에 부정적인 영향을 끼칠 수 있고 초심상담사들이 특히 사용하기 어려워하는 개입이기도 하다. 따라서 상담사들이 역전이를 활용한 즉시성 개입을 효과적으로 수행할 수 있도록 조력하기 위해서는 숙련상담사들이 역전이를 활용한 즉시성 개입 시 무엇을 고려하여 언제 어떻게 사용하는지에 대한 절차적 지식과 그 지식들이 서로 연결되고 조직화된 구조적 지식을 밝히는 것이 유용하다. 이에 본 연구에서는 숙련상담사가 역전이를 활용한 즉시성 개입을 사용하기로 판단하는 순간적인 의사결정에 있어 고려하는 요인들이 어떤 절차적 지식과 구조적 지식으로 구성되어 있는지를 개념도 연구방법을 통해 확인하였다. 본 연구에서는 상담경력 10년 이상이면서 정신역동적 관점에서의 역전이 개념과 활용에 대한 이해가 높은 숙련상담사들을 대상으로 하였다. 숙련상담사 13인과의 개별 면담을 통해 역전이를 활용한 즉시성 개입 결정의 고려 요인에 대한 아이디어를 산출하였다. 연구자를 포함한 연구팀 3인이 합의를 통해 면담 전사자료에서 532개의 기초진술문을 추출하였고, 핵심어 코딩 및 분류를 통해 78개의 대표진술문으로 종합하였으며, 숙련상담사 10인으로부터 대표진술문에 대한 적절성 평정을 받아 피드백을 반영한 후 최종적으로 70개의 대표진술문을 확정하였다. 이후 면담 참여자 13인 중 12인과 대표진술문에 대한 유사성 분류 및 중요도 평정 절차를 진행하였고, 결과를 바탕으로 다차원척도 분석을 실시함으로써 역전이를 활용한 즉시성 개입 결정의 고려 요인에 대한 숙련상담사들의 인식 구조를 분석하였다. 그 결과, 본 연구에서 숙련상담사의 역전이를 활용한 즉시성 개입 결정의 고려 요인에 대한 인식의 차원은 3개 차원으로 확인되었다. 1차원(X축)은 고려대상 축으로 상담사 요인 고려 ― 내담자 요인 고려로 나타났고, 2차원(Y축)은 준비도 판단 축으로 인지적 준비도 판단 ― 정서적 준비도 판단으로 나타났으며, 3차원(Z축)은 개입목적 축으로 내담자 통찰 추구 ― 진솔한 상담관계 추구로 나타났다. 즉, 숙련상담사들은 적극적인 성찰을 통해 역전이를 활용한 즉시성 개입을 할 것인지에 대한 의사결정으로 나아가게 되는데, 이때 숙련상담사들은 상담의 두 주체인 상담사 또는 내담자 요인을 살펴보고(1차원), 상담의 두 주체가 각자 역전이를 활용한 즉시성 개입을 할 수 있을 만큼 인지적 또는 정서적으로 준비된 상태인지를 고려하며(2차원), 셋째로 즉시성 개입이 개입 목적을 달성하기에 적절한지, 내담자의 통찰을 추구하거나 진솔한 상담관계 자체를 추구하기 위해 타당하고 효과적인 개입인지를 고려하여(3차원) 즉시성 개입을 하기로 의사결정을 내린다고 할 수 있다. 또한 다차원척도 분석을 통해 산출된 각 진술문들의 좌표값을 사용하여 계층적 군집분석을 실시하였다. 3차원 및 2차원 공간상에 표현한 개념도를 통해 각 군집의 의미를 해석한 결과, 7개 군집은 각각 군집 1. 상담사가 역전이 홀딩 및 수용 가능함, 군집 2. 진솔한 상담관계를 추구함. 군집 3. 개입이 유용하다고 판단함, 군집 4. 개입목적 및 방향성이 수립됨, 군집 5. 역전이를 통해 내담자가 이해됨, 군집 6. 내담자가 정서적 홀딩 및 수용 가능함, 군집 7. 내담자가 상담관계를 신뢰함으로 명명되었다. 숙련상담사들이 70개 대표진술문 목록에 대한 상대적 중요도를 평가한 결과를 전체 내용별 및 군집별로 살펴보면, 숙련상담사들은 역전이를 활용한 즉시성 개입을 사용하기로 결정할 때는 상담사의 역전이가 내담자에게 도움이 되는 유용한 자료로 판단되어야 함을 가장 중요한 요소로 인식하고 있었고, 상담사가 역전이 감정에 압도되지 않고 내담자에게 초점을 유지하면서 역전이 감정을 조절하고 담아낼 수 있는 상태여야 함을 중요하게 인식하고 있었다. 반면 내담자가 상담관계를 신뢰하는지 여부와 진솔한 상담관계를 추구하는 개입목적은 즉시성 개입 결정에 있어 상대적으로 가장 덜 중요한 요소로 인식되고 있었다. 본 연구는 상담사가 역전이를 활용하여 즉시성 개입을 효과적으로 사용하기 위해 고려해야 하는 다양한 요인과 맥락에 대한 이해를 숙련상담사들의 생생한 인식을 통해 제공하였다는 점에서 의의가 있고, 연구결과를 상담사 교육에 활용할 수 있는 구체적인 방안을 논의에서 상세히 다루었다. 또한 본 연구는 자료수집 및 자료분석 과정에서 신뢰도와 타당도를 확보하기 위해 매우 엄격한 절차를 수행함으로써 향후 상담학 분야에서 개념도 연구방법론이 진전되는 데 기여할 것으로 기대하며, 개념도 결과를 R을 사용하여 3차원 공간상에 3D 인터랙티브 시각화로 구현함으로써 기존의 3차원 평면의 지각적 왜곡 문제를 해결하였다는 점에서 앞으로 3차원 개념도 연구가 활발히 수행되는데 기여할 것으로 기대한다.
