Abstract
아토바스타틴은 비정상적으로 상승된 콜레스테롤 수치를 낮추기
때문에 이상지질혈증의 치료와 심혈관계 질환의 예방에 쓰인다.
아토바스타틴을 포함한 스타틴계 약물은 이미 상승해 있는 콜레스테롤
수치를 낮춘다. 이는 콜레스테롤 합성의 첫 번째 단계를 매개하는
효소의 작용을 억제하기 때문이다. 따라서 스타틴계 약물은 LDL
콜레스테롤이 상승한 환자에게 주로 1차적으로 사용하는 약물이다. 또한
고혈당 콜레스테롤의 스타틴 치료에 에제티미브를 추가하는 것이
심근경색 및 뇌졸중의 위험을 상당히 감소시켰고 이전에 심장마비가
있었던 환자들의 심장마비 또는 뇌줄중 위험을 낮추는 것으로 나타났다.
최대 내약성 스타틴 단독으로는 LDL 목표를 달성 할 수 없는 일부
고위험군에서 에제티미브를 추가 할 것을 권장하고 있다. 아토바스타틴
및 에제티미브 복합제에 대한 용출규격설정을 위해 경구용 의약품의
용출규격 설정 가이드라인에 따라 예비시험 및 본시험을 수행하여
용출규격을 설정한다. 용출규격 설정을 위한 예비시험 수행 시 패들법,
회전속도 50 rpm으로 pH 1.2, 4.5, 6.8 시험액 및 물에서 실험한다.
만약 예비시험에서 용출률이 기준치에 근접하지 못할 경우 계면활성제를
사용한다. 약물과 계면활성제의 상호작용은 약물과 계면활성제의
물리화학적 특성에 의해 영향을 받는다. 약물에 대하여 비이온성,
음이온성, 양이온성 등 여러가지 계면활성제를 선택하여 실험한다.
아토바스타틴 및 에제티미브의 경우 패들법, 회전속도 75 rpm, pH 6.8
+ 0.2 % w/v PSB 80 시험액에서 용출률이 기준에 적합하였다. 그로
인하여 용출규격을 패들법, 회전속도 75 rpm 및 pH 6.8 + 0.2 % w/v
PSB 80 시험액으로 선정하였으며 시험액 양(900 mL)과
온도조건(37.0 ± 0.5 ℃)은 일반적인 용출시험조건으로 선정하였다.
Because atorvastatin lowers abnormally elevated cholesterol levels,
it is used for the treatment of dyslipidemia and the prevention of
cardiovascular diseases. Statin drugs, including atorvastatin, lower
cholesterol levels that are already elevated. This is because it
inhibits the action of an enzyme that mediates the first step in
cholesterol synthesis. Therefore, statin drugs are mainly used
primarily for patients with elevated LDL cholesterol. Additionally,
the addition of ezetimibe to statin treatment for hyperglycemic
cholesterol significantly reduced the risk of myocardial infarction
and stroke, and was shown to lower the risk of a heart attack or
stroke in patients with a previous heart attack. The addition of
ezetimibe is recommended in some high-risk groups who cannot
achieve their LDL goals with maximally tolerated statins alone. To
set the dissolution specification for the combination of atorvastatin
and ezetimibe, a preliminary test and a main test are performed
according to the Guideline for setting dissolution specifications
for oral drugs to set the dissolution specification. When
performing the preliminary test for setting the dissolution standard,
test in pH 1.2, 4.5, 6.8 test solutions and water using the paddle
method and rotation speed of 50 rpm. If the dissolution rate is not
close to the reference value in the preliminary test, a surfactant is
used. The interaction between drug and surfactant is affected by
the physicochemical properties of drug and surfactant. For drugs,
select and test various surfactants such as nonionic, anionic, and
cationic surfactants as follows. In the case of atorvastatin and
ezetimibe, the dissolution rate in the paddle method, rotation speed
75 rpm, pH 6.8 + 0.2 % w/v PSB 80 test solution was suitable for
the standard. Therefore, the dissolution standard was selected as
the paddle method, rotation speed 75 rpm, and pH 6.8 + 0.2 % w/v
PSB 80 test solution, and the amount of test solution (900 mL) and
temperature conditions (37.0 ± 0.5 ℃) were selected as general
dissolution test conditions. The dissolution standard setting is an
experiment to set the dissolution condition through a preliminary
test and to set the dissolution standard by performing the main test
under the condition. The dissolution rate did not come out when the
preliminary test was performed under the basic conditions. As a
result, the dissolution rate indicated in the CDER was suitable. If the
dissolution standard has been established, it is applied as the
standard and test method for this drug.
