보건간호사의 재난역량과 영향요인

Abstract

간호사는 재난 발생 시 최초 대응, 중증도 분류, 직접 간호 제공, 현장 조정, 정보 제공, 교육, 정신건강 상담 등 재난의 전 단계에 있어 중요하고 다양한 역할을 수행하는 필수 의료 인력이다. 지진과 홍수, 태풍 같은 자연 재난 뿐 아니라 감염병 역시 사회적 재난에 해당하며, 재난은 즉각적인 대처가 대상자의 생명과 건강에 직결되기 때문에 재난 대응 역량이 필수적으로 요구된다. 코로나19 팬데믹 상황에서 보건소는 일차적 선별진료소이며, 보건간호사는 비상 계획, 선별, 상담, 검체 채취, 환자감시, 역학조사, 보건 교육, 예방접종, 치료 등의 다양한 역할을 수행한다. 보건간호사는 직무 수행 상 중요한 방어선에 있기 때문에 코로나19에 대한 효과적인 대처를 위해 보건간호사의 정신 건강을 돌보는 것은 필수적이며, 보건간호사의 재난대응역량이 뒷받침되어야 한다. 이에 본 연구는 Deci and Ryan의 자기결정성 이론을 바탕으로 보건간호사의 재난역량을 파악하고 재난역량에 미치는 영향요인을 파악하고자 수행되었다.

본 연구는 횡단조사연구로 2021년 3월 27일부터 2021년 4월 6일까지 부산․경남 지역 보건소 소속 간호사 242명을 편의표집하였다. 대상자들의 우울, 불안, 직무만족, 스트레스, 소진, 재난역량, 지식, 감염예방행위, 재난대응참여의지를 구조화된 설문지를 사용하여 조사하였다. 수집된 자료는 SPSS 23.0 프로그램을 사용하여 기술통계, T검정, 반복측정 분산분석, 일원배치 분산분석, Pearson의 상관분석, 다중회귀분석을 시행하였다.

다중회귀분석 결과 재난역량에 영향을 미치는 요인은 재난대응참여의지, 감염예방행위, 신종 감염병 대응 교육 경험, 보건소 경력, 직무만족, 학력(석사 재학 또는 졸업)으로 나타났다. 본 연구의 회귀모형은 유의한 것으로 나타났으며(F= 20.841, p<0.001), 모형의 설명력을 나타내는 수정된 결정계수(Adjusted R²)는 .332로 나타났다. 재난역량에 가장 큰 영향을 미치는 요인으로는 재난대응참여의지(β=.267,p=<0.001)로 나타났다.

본 연구결과를 통해 보건간호사의 재난역량과 심리적 웰빙을 향상시키기 위한 적절한 중재가 필요한 것을 확인하였다. 또한, 보호받을 수 있고 지지적인 근무 환경 조성, 경력자 우선 일선 배치, 정규직 보건간호사의 업무 책임 분산 등의 조직분위기 개선이 필요하다.

Nurses form the core of healthcare professionals and serve in a variety of important disaster response roles, such as initial response, severity classification, direct patient care, site management, and providing information, education, and psychological counseling. Infectious diseases are considered social disasters just as earthquakes, floods, and typhoons are considered natural disasters. Disaster competencies are crucial because an immediate and effective response to disasters directly impacts the life and safety of people. Public health centers have become primary screening clinics in which public health nurses play additional roles, such as emergency planning, screening, specimen collection, surveillance, and epidemiological investigation. Public health nurses are an important line of defense for the society, and therefore, their psychological health must be protected and their disaster competencies augmented for an effective response to COVID-19. This study aimed to investigate the level of disaster competencies of public health nurses in the context of emerging infectious diseases, and identify influencing factors of disaster competencies based on the self-determination theory of Deci and Ryan. This research is a descriptive cross-sectional study. A convenience sample of 242 was selected from public health nurses working in the Busan-Gyeongnam area. Data were collected using a structured questionnaire and analyzed using descriptive statistics, t-test, repeated measure ANOVA, one-way ANOVA, Pearsons correlation, and multiple regression analysis using the SPSS Statistics ver. 23.0. Results showed that the significant factors influencing disaster competencies included willingness to respond to a disaster, preventive behavior, experience of receiving education on emerging infectious diseases response, public health center experience, job satisfaction, and education. This regression model explained 33.2% of the variance in disaster competencies. Willingness to respond to a disaster was the strongest factor affecting disaster competencies. Based on these results, it is concluded that interventions to improve disaster competencies and psychological wellbeing of public health nurses are needed. Additionally, strategies such as creating a supportive work environment, deploying experienced nurses primarily on the front line, and reducing the tasks of permanent public health nurses should be implemented.
