장애인의 사회적 배제가 우울과 음주에 미치는 영향에 대한 탐색적 연구

Abstract

Recently, the concept of social exclusion, which approaches the poverty problem in multidimensional way including material scarcity, discrimination, employment and educational accessibility, and social relations, has been draw attention. It is known that disabled people experience exclusion in various areas of life such as education and labor, have a high prevalence of depression, and are vulnerable to drinking problems. Therefore, this study attempts to understand the current status of social exclusion experienced by the disabled, analyze the effect of social exclusion on depression and alcohol use, and explore whether there is a difference in the degree of the influence depending on drinking level and sociodemographic characteristics. This study analyzed 1,022 people with disabilities using data from the 16th year of the Korean Welfare Panel Study. For the measurement of social exclusion, the sub-dimensions of economy, housing, education, social relations, social participation, and labor were set, and the level of comprehensive social exclusion was converted to a value between 0 and 1 using fuzzy set theory. Depression and alcohol use were processed according to the user guidelines of the Korean Welfare Panel Study, and the higher the score, the higher the level. Descriptive statistics, Pearson correlation analysis, t-test, ANOVA, multiple regression analysis, and Chow test were conducted to verify the research model. The results of this study show that disabled people had a high level of exclusion in the order of education, social participation, economy, social relations, housing, and labor, and there was a moderate positive correlation between education and social participation, and economy and education. The level of social exclusion of the disabled differed according to sociodemographic characteristics, and it was found that female, the elderly aged 65 or older, one-person households, and people with chronic diseases experienced high social exclusion. The higher the level of social exclusion of the disabled, the higher depression, and the exclusion of social relations, economy, and social participation dimensions affected. In addition, it was found that female, younger age, and people with chronic diseases increased depression. The disabled with higher level of social exclusion showed higher level of alcohol use, and the exclusion of the economy dimensions affected. In addition, it was found that male, younger age, one-person households and people without chronic diseases increased alcohol use. As a result of testing whether there is a difference in the degree of influence depending on drinking level and sociodemographic characteristics, there is no difference in groups of drinking level and chronic diseases. On the other hand, the groups of female, the elderly aged 65 or older, one-person household, and mild disabilities were found to have a greater influence on depression, indicating that they were more vulnerable to depression by social exclusion. This study attempted to explore the current status of social exclusion of the disabled in situations where there are few prior studies related to social exclusion, depression, and alcohol use of the disabled, and to confirm the relationship between social exclusion, depression, and alcohol use. Through this, it has been proved that social exclusion of the disabled is an overlapping phenomenon, and high exclusion of education, social participation, and economy dimensions suggests the need to strengthen policy support in related fields. In addition, it was found that the exclusion of social relations had a strong direct effect on the depression of the disabled, so social support was important for the depression of the disabled. Furthermore, special management will be needed so that social exclusion does not lead to depression problems, especially for women, the elderly aged 65 or older, and the disabled in single-person households who have been identified as vulnerable groups in this study.

최근 빈곤 문제를 물질적 결핍으로만 한정하지 않고 차별, 고용 및 교육 접근성, 사회적 관계 등 다차원적으로 접근하는 사회적 배제의 개념이 주목받고 있다. 장애인은 삶의 다양한 영역에서 배제를 경험하며, 높은 우울증 유병률을 가지고, 음주 문제에도 매우 취약한 것으로 알려져 있다. 이에 본 연구에서는 장애인이 경험하는 사회적 배제의 현황을 파악하고, 사회적 배제가 우울과 음주에 미치는 영향을 분석하며, 음주수준과 인구사회학적 특성에 따라 영향력의 정도에 차이가 있는지 탐색적 연구를 시도하고자 한다. 본 연구는 한국복지패널 16차년도 자료를 이용해 장애인 1,022명을 분석 대상으로 하였다. 사회적 배제 측정을 위해 경제·주거·교육·사회관계·사회참여·노동의 하위 차원을 설정하였으며, 퍼지 집합 이론을 활용해 종합적인 사회적 배제의 수준을 0∼1사이의 값으로 환산하여 사용하였다. 우울과 음주는 한국복지패널 사용자 지침서에 따라 데이터 처리하였으며 점수가 클수록 수준이 높다. 연구 모형 검증을 위해 기술통계, pearson 상관분석, t-test, ANOVA, 다중회귀분석과 Chow test를 실시하였다. 연구 결과, 장애인들은 교육, 사회참여, 경제, 사회관계, 주거, 노동 차원 순으로 배제 수준이 높았으며, 차원 간에는 교육과 사회참여 차원, 경제와 사회참여 차원, 경제와 교육 차원이 다소 높은 수준의 상관관계를 보였다. 장애인의 사회적 배제 수준은 인구사회학적 특성에 따라 차이를 보였으며, 여성, 65세 이상, 1인가구, 만성질환자는 높은 사회적 배제를 경험하는 것으로 나타났다. 장애인의 사회적 배제가 심화될수록 우울이 증가하였으며 하위차원 중 사회관계, 경제, 사회참여 차원의 배제가 영향을 미쳤다. 또한 여성일수록, 어릴수록, 만성질환이 있는 경우에도 우울이 증가하는 것으로 나타났다. 장애인의 사회적 배제가 심화될수록 음주는 감소하였으며, 하위차원별로는 경제 차원의 배제가 영향을 미쳤다. 또한 남성일수록, 어릴수록, 1인가구인 경우, 만성질환이 없는 경우 음주는 증가하는 것으로 나타났다. 장애인의 사회적 배제가 우울에 미치는 영향을 음주수준별, 인구사회학적 특성별로 영향력 크기에 차이가 있는지 검정한 결과, 음주수준 및 만성질환 여부에 따른 차이는 없는 것으로 나타났다. 반면, 여성, 65세 이상, 1인가구, 경증 장애인들은 사회적 배제가 우울에 미치는 영향력이 더욱 큰 것으로 확인되어 이들은 사회적 배제 관점에서 우울 문제에 더욱 취약한 집단임이 드러났다. 본 연구는 장애인의 사회적 배제, 우울, 음주와 관련된 선행 연구가 적은 상황에서 장애인의 사회적 배제 현황을 파악하고, 사회적 배제, 우울, 음주의 관계를 확인하기 위한 탐색적 연구를 시도하였다. 이를 통해, 장애인의 사회적 배제는 다른 차원과의 연관성을 가지고 다중적으로 나타나는 현상임을 입증하였으며 교육, 사회참여, 경제 영역의 높은 배제는 관련 분야에 정책적 지원이 강화될 필요성을 시사한다. 더불어, 장애인의 우울에 사회관계 차원의 배제가 강한 직접적인 영향을 미친 것으로 나타나 장애인 우울 문제에 사회적지지가 매우 중요하며, 특히 취약한 집단으로 드러난 여성·65세 이상 노인·1인가구 장애인들에게는 사회적 배제가 우울 문제로 이어지지 않도록 각별한 관리가 필요할 것이다.
